While driving on the highway under normal conditions with no road hazards present, the engine under cover (part #51441-42210) detached from my vehicle and the bumper clips broke, causing both the under cover and bumper to become damaged and requiring replacement. There was no collision, no debris, no pothole, or any other road hazard involved. The failure occurred solely under routine highway driving conditions. Upon taking the vehicle to my Toyota dealership, the service team acknowledged this is a known and recurring issue with the bZ4X model. Research and owner forums confirm multiple 2023 bZ4X owners have experienced the identical failure — undercarriage cover and bumper detachment under normal driving and rain conditions. Toyota dealers have repeatedly told affected owners ‘this happens all the time,’ yet Toyota refuses to cover repairs under warranty, claiming it is not a manufacturing defect. I believe this represents a systematic design or manufacturing defect in the 2023 Toyota bZ4X. The undercarriage cover and bumper mounting system fails under normal driving conditions, which poses a safety risk to the driver and other vehicles on the road. I am requesting a formal investigation into this issue and urge NHTSA to consider a recall to protect other bZ4X owners.

Premature and abnormal tire wear on my 2023 Toyota bZ4X, confirmed by two independent tire shops, despite consistent Toyota dealer maintenance every 6,000 miles. Beaverton Toyota dealer dismissed the condition as “normal,” but both [XXX] and Discount Tire documented unsafe tire wear caused by alignment. --- Details of Problem: Vehicle serviced exclusively at Toyota dealer (Beaverton Toyota, OR) every 6,000 miles, with multipoint inspections and rotations. On 9/22/25, Toyota dealer inspected the vehicle and reported that alignment and tires were “normal.” On 9/25/25, [XXX] inspected the vehicle and documented severe outside shoulder wear on the right-side tires. They recommended immediate tire replacement for safety. Alignment was also adjusted at that time to bring values into balance (report attached). On 9/27/25, Discount Tire replaced all four tires at 27,562 miles. Their written notes confirm edges were worn on all four tires due to alignment (right side more prominent). They declared the tires unsafe for continued use and recommended full replacement. Safety Concern: This abnormal alignment-related wear created unsafe driving conditions well before the rated tire life (55,000 miles). Uneven edge wear and shoulder failure increase the risk of blowouts, hydroplaning, or sudden loss of control. Attachments available: Toyota dealer report stating “normal” Les Schwab alignment printout and written safety note Discount Tire invoice with technician safety comments Tire photos (all four corners) Full Toyota dealer service history INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I drove the car at 40mph through about 2 inches of standing water. This caused the undercarriage splash plate to become loose. It has not fallen off, but it will need to be repaired. The streets sometimes flood and none of the cars I’ve owned ever had this happen.

AC Compressor. The dealer is saying I need to repair the AC compressor. When this happened my heater and mileage weren't showing and a lot of message errors were showing.

I am writing to register our complaint and urgently request your support in resolving a serious health issue related to what we believe is a factory defect with the Toyota BZ4. Timeline of events: In April 2023, I purchased a brand new BZ4 from the Downtown Oakland Toyota dealership. In July 2024, I noticed a foul smell in the car coming from the air conditioning vents. The car was difficult to drive in due to the severity of the smell. On September 11, 2024, we took the BZ4 to the dealer for maintenance and asked for them to look into the foul smell. The dealer reported they were unable to find the cause of the odor. They offered us a car fragrance. The odor continued to be overwhelming in nature. On [XXX] we inspected the car and removed the floor mats. We discovered the undersides of both left and right front floor mats were covered in mold. The floor carpets were soaking wet with water. Both the mats and carpets smelled very badly. On October 31, 2024, we filed a customer service complaint with Toyota. On November 1, 2024, Daniel at Downtown Oakland Toyota responded that the dealer had found a spider web which had blocked the tube, causing the air conditioning system to be blocked. This caused the blocking of the HVAC tube and the water to pool and mold. I am concerned for several reasons: Toyota sold me a car which, according to them, can have one of its major operating systems (HVAC) taken down by a mere spider web. This is irresponsible and negligent design and the company should be liable for the defect in manufacturing. The car is less than two years old, was purchased new and has been driven for just 11,000 miles and is well within the warranty period. It’s shocking to us that Toyota has refused to repair this defect which we believe to be their responsibility as it falls under a design flaw. A safely designed HVAC system should be able to resist the presence of a tiny spider. The Toyota service staff should be made aware of this defect INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATI

On [XXX] I was involved in a collision with another vehicle on [XXX] , while driving my 2023 Toyota bZ4X. Despite the impact's severity, the airbags in my vehicle did not deploy, even though the other vehicle’s airbags did, suggesting that the impact was sufficient to trigger airbag deployment in similar conditions. After the accident, the Toyota app displayed a warning message indicating that the airbags required inspection, raising concerns about a potential malfunction in the airbag system. The failure of the airbags to deploy posed a significant risk to my safety, as the airbags are a critical safety feature designed to reduce injury in collisions. The vehicle has not yet been inspected by Toyota or a certified service center to determine the cause of the airbag failure. Prior to the collision, there were no visible warning lights or error messages on the dashboard indicating any issues with the airbag system. The vehicle is available for inspection upon request. 235/50/R20. Found driver side rear tire completely flat in personal garage yesterday morning and passenger front tire had low psi warning on as well. Used the fix-a-flat kit that came with my car to fill driver rear and able to get to shop. Car/tires have 5000 miles on them. Tire shop could not find anything wrong with the driver rear tire but stated a puncture was found in passenger front tire well and did patch that. I see this has been reported before on a Toyota Bz4x same year and same tire with mileage the same as well. I am concerned this could be an issue going forward and am scared to drive this car now any distance especially since there is no spare and I have to purchase a replacement cylinder for the fix-flat.

I drove my 2023 TOYOTA BZ4X home from work after a heavy rainfall in Galveston, Texas in January 2024, had about 2-3 inches of rain on the road, driving about 15-20 miles per hour on my way home. Both front wheel wells/mud guard liners came off. I drove over one of the liner, stopped and picked it up. Also a liner in the back of the car's undercarriage was hanging out. Took the car to the dealership the next morning and was told since it's due to external force/rain. The warranty on a 6 months old car is not covered. Called Toyota cooperate office and was told the same thing. I was quoted $4000 for the repairs. This should not be happening on a new car while driving 15 miles per hour on 2 inches of water on the road. I know it's 2 inches because I stepped out to pick up the fallen piece and checked the depth of water on the road. There should be a recall on this parts. Too many similar complaint on a brand new car/model from Toyota and Subaru owners.

I submitted this form earlier, but I’m unsure whether it went through. If this is a duplicate, please disregard. An incident of sudden, unintended acceleration occurred while pulling into a gravel parking area on a hillside at dusk. While turning, the vehicle suddenly lunged forward and struck a tree stump. It would have been avoided without the sudden acceleration. The driver slammed on the brakes, and the vehicle came to rest right at the edge of a downward slope. Five seconds earlier, and a family member, who had come out of the house to show where he wanted the car parked, could have been killed, or seriously injured. The car was towed to Toyota Of Erie, where it was inspected by a corporate inspector. It remains there until a decision is made. The adaptive cruise control, which was used on the trip, had been canceled, and the brakes were used several times after exiting the freeway. There was apparently no warning from the front camera and the collision avoidance system about the tree stump, though the driver had seen it prior to making the turn. The incident happened very quickly, but there was no noticeable red warning flashing on the screen. This time there is only damage to a car fender, and a tree stump. What about next time? Suppose the vehicle is slowing into a parking space, there is a child exiting nearby van and the car suddenly accelerates? No thanks!!! This problem needs to be solved! The floor mats in the BZ 4X are secured to the floor so they are not an issue. The driver of the vehicle can play an instrument with foot pedals and is therefore very aware of foot placement-so operator error is not the problem. Perhaps there is just too much autonomous driving technology and software layered over the basic EV platform. Can’t we just have the choice of a basic EV until these other problems are worked out? I signed up to be a pioneer in the adoption of electric vehicles, but not a guinea pig for autonomous driving.

- 11007455Some 2005 2024 Toyota vehicles that have undergone water intrusion may exhibit a condition in which a musty odor is present. Follow the procedures in this bulletin to remediate the odor and address this condition. The purpose of this Service Bulletin is to provide general guidelines and procedures for odor remediation. This Service Bulletin provides a guide on how to prepare and treat the interior of the vehicle for odor remediation. Refer to the applicable model and model year Repair Manual and the EPA (Environmental Protection Agency) website for the most up-to-date safety and precautionary guidelines. This Service Bulletin is to provide a general overview and direction for the odor remediation process.
- 11007445The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
- 11007444To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
- 11007443Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: Minor damage: requires only buffing to repair. Moderate damage: usually requires neutralizing, color sanding, and buffing. Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.

It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
- 11006150To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
- 11006149Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere.

- 11005153Toyota vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination.
- 11005149Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid.

- 11003649The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. While Toyota paint quality, durability, and resistance to environmental factors is continuously being improved, it remains the responsibility of the dealer to protect and maintain the vehicles paint finish from the time of arrival at the dealership until retail delivery to the customer. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
