Hankook optimo H724 tire p205/75r-14 dot T7XC hbh. Inside tire wall (back driver side) separated on 04-16-14 traveling I10 west at arizona high way speed limit of 75 mph causing accident. Nothing was run over on highway. Severe, roll-over accident. Mini-van totaled. Purchased at america's tire, san ramon, ca 07-20-13. Tire had ~ 10,000 miles wear on 70,000 mile tire. Tires and pressure checked 8 days prior. Inside wall separations are extremely rare. Suspected manufacturing defect. manufacturing quality control IS not hard. Any manufacturing defect that puts human life in danger IS not acceptable. Will not purchase hankook tires again. There are reported problems with inside wall cracking on the hankook optimo H725 to the national highway safety transportation administration. I had reported the problem directly to hankook. [Xxx], hankook tire, contacted me 04/21/14. he IS clearly not interested in the serious nature of a potential manufacturing defect in 2 phone conversations on 04/22/14. I clearly stated several times that my intent in contacting them was from a safety perspective. [Xxx], from hankook tire, did not know which manufacturing plant had manufactured the tire even though I had provided the dot number or if there were other known problems with the same model number. If there had been a car next to me or a car behind me or a tree on the side of the highway, there might have been serious injuries or death with that type of inside wall tire separation. Again, let me reiterate how rare and dangerous inside tire wall separations are. There IS no ability to control the vehicle in this type of situation. The vehicle jerked to the left, then jerked to the right, and then jerked to the left again before it started to spin 180 degrees and roll. There were no tell-tale warnings (rough ride) of the potential inside tire wall separation like an outside tire wall separation. A tire manufacturer who IS lackadaisical about safety IS deeply troubling. information redacted pursuant to the freedom of information

1993 toyota previa. sudden brake failure at about 30 mph. I tried to stop and there was no braking. parking brake was also not operable. I turned the key off and the vehicle continued to run. putting it in park or reverse did not stop or slow it down. I went down gulley, across lawns and parking lots and did circles in parking lot until it finally stopped (?ran out of gas). terrifying!! I notified toyota to see if they were interested in finding out why there was this dangerous failure and I was informed my car was out of warranty and they really didn't care. overall it had been a great vehicle and all six of my children used it to learn to drive. I was about to give it to my youngest son. thank goodness it happened with me and not him. I was very disappointed that toyota really didn't care that this could have resulted in serious injuries and there are still alot of previas on the road. it had fairly new brakes and had been well maintained. it will now be sold for scrap. *tr

In january 2004, the steel belt in one of our tires frayed through the tire tread while driving. the tire was replaced, and the company that sold it to us returned it to the manufacturer with a request for explanation of the defect. there was no reply. in june 2004, the same problem occurred with another tire from the same set. the mechanic replacing this second faulty tire pointed out that a third tire was bulging, because it had the same defect, and the steel belt would soon fray through the tread if not replaced. the tires were originally purchased new as a set of 4 in november 1999. they had about 37,000 miles wear when the first one had to be replaced, and about 41,000 miles wear when the next two had to be replaced. the brand IS toyo, p215/65r15 800 ultra 95T bl. the dot number for the second tire that failed IS CX kw 49 v 088. *ak

I am currently attempting to get air conditioning (a/c) fixed on previa. 6 weeks ago a new condensor and associated parts were replaced due to failure. these new parts were replaced to meet epa requirements for r-134a coolant (original coolant was R-12). a/c on system failed on july 2, 2003. previa was taken to motorcars toyota in cleveland heights (not the same place as where r-134a conversion was performed) and the system checked. system was recharged with no indication that the dealer made any type of checks for leaks. previa was returned the next day to dealership with strange "harmonic" noise that was heard by service manager but not corrected. rear a/c unit IS not operating and front a/c unit IS not capable of cooling entire van. I will be notifying toyota of this issue, but feel that with the number of previa's still on the road, there will be continuing issues with the switch over from R-12 to r-134a. dealership claims that the noise IS noticeable but too difficult to locate. their claim IS that rear a/c IS cooling normally, even though service manager admits that it IS not doing so. he blames the issue on differences between R-12 and r-134a. in reading information on the web, all parts of system need to be checked for leaks and that r-134a runs at a higher pressure, compressor needs to be checked. from your website, toyota recommends in their service bulletin 080498 that an improved rear a/c magnetic valve IS required to keep internal valve corrosion to a minimum. there IS a high probability that I still have the old valve designed to run under R-12 installed. this will be a continuing problem as there was no effort to look for any leaks or other problems. Recharging the system every month IS not a viable option. I will be making an additional appointment next week to give the dealership one more opportunity to correct this issue. I'll contact the bbb, aaa and the next management level at toyota if it isn't fixed then. *jb
