Concern 1: My 2022 Toyota Sienna intermittently experiences an unexpected forward surge while braking. When I press the brake pedal to slow or stop the vehicle, I sometimes feel a sudden forward pushing or acceleration sensation before the vehicle slows down. The behavior is unexpected and concerning, particularly when approaching another vehicle, intersection, or obstacle. I am concerned that this condition could increase the risk of a collision Concern 2 – Clicking/clunking from driver's side There is also a repetitive clicking or clunking noise from the driver's side of the vehicle during stop-and-go operation. The noise can be heard when the vehicle stops and then begins moving again. I reported this condition to Nalley Toyota in Roswell and requested that it be inspected as a potential warranty issue. I was advised that I would need to pay a diagnostic fee.
I am reporting an ongoing braking-safety problem with my 2022 Toyota Sienna XSE Hybrid. During low-speed braking, at the transition from regenerative to conventional hydraulic/friction braking, the expected deceleration can change abruptly. The vehicle can briefly surge forward even though I continue braking. This reduces the gap to the vehicle ahead, requiring significantly harder and faster brake-pedal input to avoid a collision. This affects the predictability of stopping distance. I purchased the vehicle on June 17, 2024 as a Toyota Certified Used Vehicle. On July 8, 2024, during a service visit — 21 days and 577 miles after purchase — I personally reported to Toyota a noticeable change/jerk in braking force at low speed during the transition between regenerative and conventional braking, and Toyota officially documented my complaint. T-SB-0047-24 already existed and provides for reprogramming the brake actuator control module. The update was not performed then and still has not been performed. In 2026, I again returned to Toyota with the same complaint. Seeger Toyota did not perform a road test because the technician considered the mechanical brakes unsafe. The dealer recommended rotor replacement because of corrosion/pitting, but the final RO contains no separate rotor-thickness measurements, runout measurements, quantitative pitting/corrosion data, or specific Toyota limits/criteria. After my written escalation, the Service Manager agreed to perform T-SB-0047-24 at no charge, but the update remains unperformed and the problem recurs regularly. I ask NHTSA/ODI to evaluate this case as a potential safety defect related to the regenerative-to-hydraulic brake transition. Because of the 2,000-character limit, the full description and chronology are attached as a separate PDF. Key evidence is being uploaded with this complaint; additional service records, correspondence, and photographs/measurements are preserved and can be provided upon request.
I own a 2022 Toyota Sienna with approximately 67,680 miles. The vehicle began displaying warnings for **Blind Spot Monitor Malfunction, Rear Cross Traffic Alert Malfunction, and Parking Support Brake Unavailable**. A Toyota dealer diagnosed the problem and found significant corrosion at **both rear blind spot monitor connections**. The wiring harness connector pins had corroded/broken off inside the blind spot monitor sensors. The dealer recommends replacing both blind spot monitors and the rear bumper wiring harness and recalibrating the system. The estimated repair cost is **$3,945.84**. I am concerned that both sides of this safety-related system experienced severe corrosion on a vehicle that is less than five years old. These components are not scheduled maintenance or wear items. I have also found reports of other 2021–2022 Toyota Sienna owners experiencing blind spot monitoring failures involving corrosion of the rear sensor/connectors, including another 2022 Sienna at approximately 65,000 miles. Toyota's Brand Engagement Center has documented my complaint but declined to review the repair for goodwill assistance because the vehicle is outside the basic warranty and has not been serviced exclusively at Toyota dealerships. I am filing this complaint because the corrosion simultaneously disables multiple driver-assistance/safety functions and appears unusually premature. I believe NHTSA should investigate whether corrosion or inadequate protection of the rear blind spot monitor connections is affecting other 2021–2024 Toyota Siennas.
When apply the brake, the car will have sudden forward pushing.
I am reporting a potential safety issue involving the passenger-side power sliding door on a 2022 Toyota Sienna. On XXX, my [XXX] child was outside the vehicle in a wagon while the passenger-side sliding door was closing. He reached toward the vehicle, and his fingers became trapped near the front edge of the sliding door, closest to the front passenger door. The power door did not stop or reverse after contacting his fingers. It continued closing, clicked shut, and fully latched with his fingers caught in the door. We immediately opened the door and removed his hand. Thankfully, he was not seriously injured, but the incident had the potential to cause a severe hand or finger injury. I am concerned that the obstruction-detection or pinch-protection system did not detect an infant’s fingers in this location. I am submitting this complaint so the incident is documented and can be compared with any similar reports involving Toyota Sienna power sliding doors. Stitching on the driver seat is tearing. Under normal wear. The vehicle is just out of the 3 year/36,000 warranty period. If the seat continues to degrade it will not be safe for future use as it worsens.
The car will occassionally lurch forward while braking. It's somewhat random but most often happens when going down a slight hill. My best guess is that there is an issue with the transition from regenerative braking to mechanical braking. It feels like the vehicle momentarily looses contact with the brakes and that the brake pedal has to be pushed down another inch or two to reengage. My concern is that if this happens at the wrong time that I will get in an accident. This has happened ever since I bought it. It's not very often, maybe once a week. Speeds have been variable from under 25 to highway speeds.
We brought our 2022 Toyota Sienna in for service January 2024 for the driver's seat. The seat would make really loud popping noises that sounded like something was loose when the seat moved forward and backward. We brought the vehicle in, and the service technician discovered that the driver's seat assembly was loose and cracking. The technician removed and replaced faulty components of the seat assembly under warranty. The seat starting having the same issues recently, and we are concerned of an on going safety issue given the instability of the driver's seat. The dealership informed us they will not repair the seat under warranty.
Rear windshield shattered between 4/16/2026, 10:04 AM and 4/16/2026, 6:43 PM without reason, no one near it to cause this broken windshield. I have Simplisafe camera that showed no one was near the car. See attached pictures.
I was pulling out onto a main road, pressed the accelerator and "not much" happened. The car accelerated to maybe 5 mph and no faster. Took my foot off the accelerator and pushed it to the floor. No response. It was like the vehicle only ran on EV mode and did not turn the engine on. In a second or more returned to normal and could not reproduce what happened. On coming traffic had to break. There was enough room to pull out under normal situations.
I am reporting a recurring safety-related defect involving driver-assistance and steering-wheel controls on my 2022 Toyota Sienna Limited, VIN [XXX] ry 8, 2026, I brought the vehicle to Parkway Toyota in [XXX] bec [XXX] tion” warning appeared and cruise control stopped working. The dealer diagnosed the issue as related to the 12V battery, replaced the battery, and advised that the condition was resolved. The same malfunction later recurred. I returned to Parkway Toyota on March 18-19, 2026 for the same problem. I reported that the issue had reappeared after driving in cold conditions in Canada and that when the vehicle was warm there were no warning lights. The dealer diagnosed DTC U042381 (“invalid data received from instrument panel cluster”) and replaced the hybrid vehicle control ECU. I was told the repair was complete. Within about two days, the identical “LTA Malfunction” warning returned again and cruise control was again disabled. In addition, a broader failure then developed involving the steering-wheel-mounted controls: the steering-wheel buttons stopped responding, the instrument display became stuck on one screen and could not be changed, the cruise-control buttons stopped functioning, and the radio volume could no longer be changed from the steering wheel. This appears to be an intermittent but recurring defect affecting driver-assistance functions and vehicle controls while driving. The vehicle has now had two repair attempts for the same general condition, and the problem has not been resolved. Repairing dealer: Parkway Toyota, [XXX] . Repair dates: January 8, 2026 and March 18-19, 2026.
