Tire delaminated while traveling with family on I-10, 20 minutes from home. I filed a incident report with firestone and sent them the tire as they requested. When tire delaminated, it damaged the rear quarter panel of my van along with damaging exhaust and fuel filler tube. Firestone responded to my incident report claiming no fault and that the failure was due to an underinflated tire. I have evidence and can prove under oath that this tire has never been underinflated, since I check the tire pressure of all the vehicles tires at minimum every 60 days. I have photographs of the tire after it failed along with pieces of its tread. The tire had only 36,220 miles on it and was visually inspected just a few weeks prior to it's failure. The tire had no visual signs of adverse treadwear; regular alignments and wheel balancing were part of my preventative routine maintenance.*ak (dot number: vd1l1xd308 tiresize: 225/75/r16)

NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
Mr. Larry a. Sackey submitted a petition to the national highway traffic safety administration (nhtsa) by letter dated april 18, 2003, requesting that nhtsafurther investigate the defective collapsible steering shaft system on 1987-1995 vehicles manufactured by the daimlerchrysler corporation.in part because of 49 cfr ? 30120(G), nhtsa has decided to limit the scope of the petition vehicles to 1993-1995 models, excluding the 1993 jeep grand cherokee and 1994-1995 dodge RAM series trucks that were recalled (93v210 & 96v230) for steering column failure.if the analysis of these vehicles had identified a potential problem, the scope could have been expanded in an investigation.a review of odi's data base shows no recent complaints and a lack of a complaint trend concerning the subject issue on the subject vehicles as a whole or by individual models.therefore, the petition IS denied.for details, see federal register notice volume 68, no. 130, july 8, 2003.
