FORMAL VEHICLE SAFETY COMPLAINT Vehicle: 2024 Mazda CX-50 Subject: Repeated Unintended Emergency Braking, False Collision Warnings, and Persistent Braking-System Malfunction I am submitting a formal safety complaint concerning serious and recurring braking-system malfunctions involving my 2024 Mazda CX-50. The vehicle initially began displaying false collision and braking warnings despite there being no apparent hazard or obstruction. These warnings occurred repeatedly and eventually progressed to incidents in which the vehicle activated its automatic emergency braking system without justification. During one particularly dangerous incident, I was traveling approximately 75 mph on the freeway when the dashboard suddenly displayed a red braking warning and the vehicle automatically applied its brakes with extreme force. There was no apparent vehicle, pedestrian, or obstruction requiring emergency braking. The sudden and unexpected deceleration nearly resulted in a serious collision. These problems are not isolated. The vehicle continues to experience false collision alerts or unintended braking on an almost daily basis, even when no hazard is present. Additionally, the vehicle repeatedly displays the following warning: “Braking System Malfunction. Stop the Vehicle in a Safe Location.” This malfunction warning typically appears during morning operation and does not resolve simply by shutting off and restarting the vehicle. I have photographic documentation of the warning. Although the brake pedal generally feels normal during manual braking, the vehicle’s unexpected automatic braking is extremely forceful and creates a substantial danger to occupants and surrounding motorists. I am aware that Mazda issued Service Alert SA-007/24 concerning unintended Smart Brake Support operation involving certain Mazda vehicles, including the CX-50. I request that this information be considered when reviewing my complaint.

Component Failed & Availability: Goodyear tire structure/tread separation and delamination across all four tires; technician states delamination, belt separation on all four tires, structural failure, tire separating from the internal belt structure (steel belts showing, chunks of tread missing); available for inspection. Safety Risk: Severe risk of sudden blowout, loss of vehicle control, and highway accident due to structural failure and internal belt separation. Dealer/Center Confirmation: Confirmed as a manufacturer defect by American Tire Company during a routine maintenance rotation on July 3, 2026 (fully documented by their shop). Inspections: Inspected and documented by American Tire Company; Goodyear management reviewed the documentation and refused full replacement under the guise of wear and mileage. Warning Lamps / Symptoms: No warning lamps; structural delamination and internal belt separation discovered during routine maintenance. Detailed Summary Description: On July 3, 2026, a routine maintenance rotation at American Tire Company revealed a dangerous structural failure across all four Goodyear tires. The technician states delamination, belt separation on all four tires, structural failure, tire separating from the internal belt structure, with chunks of tread missing and steel belts exposed. When the defect was submitted to Goodyear, July 6, 2026) they refused full warranty replacement, demanding $48.57 per tire under the guise of "wear and mileage." When escalation was requested, company supervisor representative stated there was no higher authority than the supervisor and admitted that internal complaints are dead-ended in their files and never forwarded or reviewed. This stonewalling on a dangerous manufacturing defect poses a severe public safety risk.

I'm curious how many other Mazda owners are dealing with the same battery drain issue. I'm now on my third battery since 2024: Factory battery replaced under warranty in 2025. Second replacement purchased in March 2026. Within a month, the "Battery Low - Drive Vehicle to Recharge" warning was back. The dealer kept my vehicle for four days to monitor it, but they couldn't find anything abnormal. This started long before I had a dashcam professionally installed, so that's not the cause. I've read numerous posts on Facebook from other owners mentioning: A door module that can get wet after heavy rain and cause a parasitic battery drain. The key fob constantly communicating with the vehicle when it's within about 20 feet, potentially contributing to battery drain. I mentioned the door module issue to my dealer, but they said they had never heard of it. At this point, it's becoming incredibly frustrating. A modern vehicle shouldn't be going through batteries like this, especially when the dealer can't identify the root cause. I started to document the times that my dashboard warning pops up that the battery is low.

I was attempting to mount my bike rack. To get the clips to properly fit I needed to partially open the liftgate. Upon triggering the door release the power liftgate and began opening the door. The anti pinch system did not detect the pinch and it crumpled the roof and liftgate at the seam. I tried to stop it but there was too much pressure and by the time it stopped lifting under my body weight significant damage had occurred. If a finger had been there it would have been chopped off.

On 02/27/2026, a 2024 Mazda CX-50 (20700 miles) exhibited a severe burning smell following a 3-hour highway drive and a subsequent steep, 2-mile mountain climb to a restaurant in Jasper, TN. The smell was strongly observed by multiple occupants upon exiting the vehicle. On 03/02/2026, a technician at Mazda of Knoxville confirmed via video a 'flat grease ring' slung onto the undercarriage, indicative of a centrifugal lubricant leak from the driveshaft/CV assembly. The proximity of this flammable grease to the hot exhaust system during high-torque operation (mountain climbing) presents a critical fire hazard. Despite visual confirmation of the slinging grease, the dealership issued a blank service invoice, omitting the defect from the official record and returning the vehicle in an unsafe condition. This represents a documented safety defect being suppressed by the manufacturer, posing an unreasonable risk of vehicle fire or catastrophic driveline failure.

The vehicle had the original OEM battery fail at 15 months. It dropped to 7v under light load. It's a bad battery. Upon being jump started, the power steering went in and out while driving and the dashboard blinked on and off...throwing a lot of errors that are unrelated to the battery problem. The failure state of the vehicle with a bad battery is unlike any vehicle I have ever seen and it is extremely dangerous. Furthermore, Mazda roadside assistance does not know about this issue and only offers jumpstarts, putting customers into danger by suggesting that the vehicle is safe to drive with a battery that is not stable.

This update pertains to the original complaint (11613144) regarding windshield integrity issues on a 2024 Mazda CX-50. A new crack has occurred on [XXX], with cracks consistently radiating from the driver's A-pillar side. This pattern suggests stress fractures rather than impact damage, as initially assumed. The vehicle was taken to a Mazda dealer in Seekonk, MA, but the Mazda Service Manager denied warranty coverage without an in-person inspection, relying solely on photos taken by a trainee service advisor. These images clearly show a recurring pattern of cracks in the same area, which is concerning. Research into Mazda forums reveals numerous reports from other owners experiencing similar windshield failures, including unexpected cracking, chipping, waviness, visibility issues, and delamination. Many report quick denials of warranty assistance, similar to this case. The frequency and similarity of these reports suggest a potentially systemic issue that may be more widespread than the recent hitch recall. The manufacturer's reluctance to investigate raises serious concerns about windshield safety. After three cracks and two replacements, Mazda has not physically inspected the issue. This situation warrants a thorough investigation by NHTSA, including major glass providers and installers, to ensure occupant safety and address potential systemic failures. The lack of proper reporting and inspection by Mazda underscores the need for NHTSA intervention to protect consumers and ensure that safety standards are met. Furthermore, the consistent denial of warranty claims without adequate inspection may indicate a broader problem in how these issues are handled, potentially affecting many more vehicles than reported. This pattern of failures and lack of action by Mazda necessitates a comprehensive review to safeguard public safety. Collision Prevention System is dangerous. I was nearly rear ended going 75 mph on the PA Turnpike. I was passing a truck that was in the right hand lane when all of a sudden the brakes automatically engaged hard and I nearly was rear ended by another vehicle that was right behind me. It braked so hard it made the tires squeal. I contacted two dealerships and told them what happened and both told me there haven't been any problems with the system that they were aware of and maybe a software update would help. Neither dealership checked if a update in fact was available. I turned the system off and will never turn it back on because it is so dangerous!

Fuel issues P0460 fuel level sensor circuit. Engine malfunction light comes on with fuel sensor issue. The car is full of fuel or not even close to be empty but the sensor reads off triggering a chain of event. Killing car mid of the highway when it got enough fuel. Average full tank ranges 280mi, this instance it only drove 202mi and the sensor reads 14mi to go with quarter Guage left in the tank. Then the needles drops by a notch. I can't upload the video but attached at the screenshot. Contacted dealer and they won't diagnos till 90days out. This is safety hazard and the car comes to halt in the mid of the drive. Where it's highway, parking lot, or anywhere; as if it's out of gas but the tank still good. Wait for few minutes and restart the car the garage read normal. The dealer doesnt want to listen or even consider this as safety issue. Multiple dealer reject to even make an appointment and request that I go to the dealer I purchased the car from.

I purchased my vehicle early August 2024 at Knudson Foothills Mazda in Spokane, Washington. Within the first five hundred miles of use, I noticed that my steering wheel would not maintain center alignment. I took it into the shop explaining my issue and they were fast to acknowledge the alignment was off and that they realigned the vehicle. The steering was off center alignment within 20-40 miles so I again, contacted the dealer and told them I expect a more thorough inspection. They took a brief inspection and realigned the vehicle again. At pickup, the service advisor said "the suspension is settling, this is normal. If it happens again, don't bring it back for 5K miles." They also proceeded to tell me they could not determine the cause, so they called the Mazda Technician hotline which is where they claimed to have got their answer for me. The center alignment did not last more than 20ish miles and has gradually gotten worse. If this were "normal", they would have been able to tell me on the first visit instead of two and having to call their own tech hotline. If this were "normal", I would be able to find reputable sources online confirming this. If this were "normal", I would not have been told to endanger my life for an extended 5K miles before asking for this to be fixed again. I do not feel safe in my car. Each time I call the dealer they tell me to wait until 5K miles. I have filed a complaint with Mazda USA but have not heard anything since. I'm currently looking into attorneys to go after Mazda USA as I feel the vehicle is UNSAFE to drive and they are being NEGLIGENT on their approach with total disregard to consumer safety. Searches online indicate this is likely a result of faulty parts used when the vehicle was manufactured. I do not know what else do to besides file this complaint so a large entity can question Mazda on this SAFETY ISSUE. 1st service/realignment: 9/16/24 - approx 750 miles 2nd service/realignment: 9/25/24 approx 1251miles
