Vehicle Information: Year / Make / Model: 2022 Volkswagen ID.4 VIN: [XXX] Current Mileage: ~58,000 miles Affected Component(s): Electrical System, Power Steering, Structure: Door Assembly (NHTSA Recall 24V-651 / VW Recall 57J9) Incident Description: Immediately following a car wash, the vehicle suffered a catastrophic electrical and CAN bus network failure. A cascade of sensor and system failure warnings illuminated on the dashboard, total loss of power steering occurred, and the vehicle entered a restricted limp mode limiting speed to under 1 mph, rendering the vehicle completely inoperable. This issue is identical to the safety defect described in NHTSA Campaign 24V-651 (VW Recall 57J9), wherein water intrudes past exterior door handle seals and shorts out the vehicle's electronic control network. Door handles became inoperable. Safety Hazard: Sudden, complete loss of power steering and propulsion creates an immediate crash hazard and leaves vehicle occupants stranded in potentially dangerous locations without critical vehicle control functions. Dealer / Manufacturer Refusal to Repair: Upon delivering the vehicle to an authorized Volkswagen dealership, both dealership staff and Volkswagen of America refused to perform the recall repair or waive diagnostic fees. They claimed the recall remedy had already been performed over 12 months prior and argued that any warranty on the recall repair had lapsed. Volkswagen corporate is actively refusing to remedy an unresolved, recurring federal safety defect that renders the vehicle unsafe to operate. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
A center brake light module burned up, contaminating the passenger compartment with toxic fumes and making the vehicle undriveable. The fumes caused burning throat, headache, and nausea with potential worse long-term health implications. Volkswagen replaced the faulty unit but has refused to mitigate the toxic fumes or replace the vehicle under warranty. A web search indicated this isn't an isolated problem with at least this vehicle model and year.
PRIMARY SAFETY DEFECT — SUDDEN STOP IN ACTIVE TRAFFIC: The vehicle suffers from severe, uncommanded electrical/drive-system shutdowns. While driving in live traffic, the ID.4 panics, flashes error alerts on the display, and abruptly stops within seconds. This sudden, violent stopping in active lanes creates an extreme safety hazard, leaving the EV dead in traffic with no warning and creating an immediate, severe risk of high-speed rear-end collisions, loss of control, and serious occupant injury. SECONDARY SAFETY DEFECT — ELECTRICAL & CHARGING SYSTEM FAILURE: Immediately upon picking up the EV post-warranty service on Aug 31, 2026, the dash displayed persistent error messages: "Your Charging Socket is not working." A VW Master Tech/Engineer at the dealership evaluated the vehicle, witnessed the failure, and explicitly warned that driving long distances poses a severe risk of complete vehicle disablement and becoming stranded on public roads due to total charging failure.REPAIR HISTORY & DAYS OUT OF SERVICE (OOS): The safety issues first occurred around Feb 2025 (towed to dealer). Since then, I returned the EV to the authorized dealer (Jim Ellis VW Kennesaw) at least 5 separate times for the same unresolved flaws. The vehicle has been OOS for warranty repairs for over 90 cumulative days, including 60+ continuous days during the last visit. Dealer diagnostic logs confirm central control unit (CCU) replacements and extensive electrical repairs, yet the hazardous conditions remain uncorrected. MANUFACTURER RESPONSE: VW Group of America acknowledged this history under cases 07530936, 07526164, 07386099, 07334984, and 07709898. VW offered a $4.5k cash settlement, which I rejected as insufficient to address the extreme safety risks. VW has since refused a repurchase, ended negotiations, and closed the case without resolving the defect. This EV is dangerous and unroadworthy. My family and I have lost all confidence in this car.
This July (2026) I found the power steering to fail on two separate hot days, a similar issue also occurred in July of 2025. Using an OBDII reader it appears that it may have been related to a torque sensor.
The issues I am having are exactly the ones that were subject to an earlier recall. NHTSA: 24V344 and Volkswagen 919A. From the text of that recall: "Due to a software issue, affected vehicles may experience center displays and instrument panel displays that do not boot, or that sporadically reset. This can result in loss of speedometer inforation or loss of rearview camera image. As such, these vehicles fails to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 101, "Control and Displays" and 111, "Rear Visibility." Displays that do not show critical information, such as vehicle speed or the rearview camera image, increase the risk of a crash." These symptoms are recurring in my vehicle and I have video documentation. Volkswagen is refusing to deal with me because "their diagnostic tools do not show any error message."
What happened: On XXX at approximately 6:49 PM, shortly after departing, the vehicle displayed error and warning messages, entered neutral, and would not engage Drive or Regenerative Brake mode. The vehicle remained inoperable in this condition for approximately one hour near the intersection at approximately [XXX] , during rush hour traffic. The vehicle recovered without mechanical intervention. This is the third occurrence of this failure mode within approximately 90 days. Safety risk: Loss of propulsion at or near a major intersection during rush hour traffic created a severe collision risk to vehicle occupants and surrounding motorists. The vehicle was unable to move under its own power for approximately one hour in high-density traffic conditions. Dealer/manufacturer confirmation: The vehicle was brought to Deel Volkswagen, Miami, FL on XXX and is currently under active diagnostic evaluation. This is the third reported occurrence of the same failure mode. A related drive error was previously documented at a XXX dealer service visit. Other inspections: UNKNOWN. Prior warning lamps or symptoms: Error and warning messages appeared on the vehicle display immediately prior to loss of drive mode. Identical failure modes were previously documented on 02/23/2026 and 05/11/2026. The vehicle has a history of repeated software campaigns and recalls including 97HB, 919A, 57J5, and 57J9 suggesting a pattern of unresolved electrical and software nonconformities. Component/System: Electrical system / electric drive control system. Available for inspection upon request. What happened: On XXX at approximately 3:43 PM, shortly after departing, the vehicle displayed electrical system warning and error messages, entered neutral, and would not engage Drive or Regenerative Brake mode. The vehicle remained inoperable in this condition for approximately one hour at approximately [XXX] . The vehicle recovered without mechanical intervention. Safety risk: Loss of propulsion in active traffic created an immediate collision risk to the vehicle occupants and surrounding motorists. The vehicle was unable to move under its own power for approximately one hour. Dealer/manufacturer confirmation: The failure was reported to Deel Volkswagen, Miami, FL. A related drive error service visit occurred on XXX at the same dealership. The vehicle is currently in service at Deel Volkswagen as of XXX for diagnosis of recurring power loss events. Dealer has confirmed the vehicle is under active diagnostic evaluation. Other inspections: UNKNOWN. Prior warning lamps or symptoms: Warning and error messages appeared on the vehicle display immediately prior to the loss of drive mode. History of prior software campaigns (97HB, OTA 3.2.12, OUE7 3.8.11) and Recall 919A (instrument panel display malfunction) suggest a recurring underlying electrical/software fault. A drive error warning was documented at a XXX dealer service visit prior to this event. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
On 5 separate occasions the vehicle locked up the brakes while at highway speeds. Nothing around to trigger any systems
I brought my car to the Volkswagen garage because there was a problem with the software, mainly the GPS was off. The location shown was 1 mile away. After 10 days in the garage, I asked if I could take it back and the case remained open because the problem was not resolved. I was pulling back my car in front of the house to unload groceries and it did not stop, damaged the garage door. Several people complained about this problem on the web, when the car is in reverse, and it seems that a software update can fix it.
At 43k miles, 90F outside temps, battery SOC at 20%, charged the car at a fast charger getting 140kw. Daytime Running Lights, DRL, were on. After the charging session was over at 80%, the driver's side DRL went and stayed dim. Dealership replaced the entire headlight under warranty. At 77k miles, 94F outside temps, battery SOC at 20%, using a fast charger and getting 145kw with DRL left on. After the charging session was over at 80%, the passenger's DRL went and stayed dim. I normally turn off the DRL and forgot this one time.
The front wheel assembly detached from the vehicle while I was driving. The vehicle is a Volkswagen ID.4 currently under lease. Prior to the incident, the vehicle had multiple wheel alignment services performed by an authorized Volkswagen dealership on May 24, 2025 and August 11, 2025. After the wheel detached on February 1st, 2026, the vehicle was towed and repaired by an independent repair shop. The vehicle and repaired wheel assembly may still be available for inspection if requested. The wheel detached while the vehicle was traveling on a public street, which caused an immediate loss of control and created a serious risk of collision and injury to myself and other road users. The vehicle could not be driven and required a tow. A police officer assisted at the scene in arranging a tow truck because the vehicle was immobilized.
My 2022 VW ID.4 Pro S has experienced a total failure of the GPS and telematics system (J949 module), causing a dangerous 'GPS Drift' where the vehicle reports its location miles away from its actual position. This creates two severe safety hazards: Automated System Malfunction: The Travel Assist and Dynamic Road Sign systems are receiving conflicting data. The camera correctly identifies a speed limit (e.g., 40 mph), but the faulty GPS overrides it with incorrect map data (e.g., 25 mph). This causes the vehicle to unexpectedly and dangerously decelerate while in active traffic. Emergency Service Failure: The eCall (Emergency Call) system is reporting the wrong location. In the event of a serious crash, emergency responders would be directed to a location miles away from the actual accident site. My dealer provided written documentation (attached) stating there is 'NO FIX' currently for 2021-2023 models, as they are hardware-locked to a failing Verizon-based data protocol that VW has not patched. The manufacturer has acknowledged the defect but offers no remedy for this safety-critical system.
Electrical gremlins present at time of purchase (3/14/2025). Entered service and repair on 3/17/2025 and remained for 30-45 days. Upon coming to retrieve vehicle electrical malfunctions still present upon entering vehicle in the service bay, alerted advisor. Car was checked back into service immediately and remained for 60-90 days. Between March and December I have had continuous electrical gremlins present from Dashboard and display malfunction, interior lights malfunction, trunk/doors/locks/windows malfunction to no end and no repair in sight. This specific vehicle is a lemon and riddled with irreparable electrical gremlins, it has cost me significant income loss and loss of faith in the reliability of Volkswagen brand that I love so much.
Subject: Request for Investigation and Recall – Door Handle Failure on Volkswagen Vehicle Dear National Highway Traffic Safety Administration, I am writing to formally request an investigation and potential recall concerning a serious safety issue with my 2022 ID.4 AWD Pro S Volkswagen vehicle. The problem involves the door handles failing to open from either the inside or outside of the vehicle. I can provide video evidence showing that, while the vehicle is fully unlocked, with no child safety mechanisms engaged and my foot on the brake at a complete stop, the doors cannot be opened from either the front or rear seats. This issue occurs repeatedly—approximately four to five times per week—and has become more frequent since the “919” recall was performed on the vehicle. Despite this, the dealership has stated that the issue is a “non-issue,” claiming that other vehicles of the same model exhibit similar behavior. The inability to open a vehicle door in normal operating conditions is a serious safety concern that could endanger occupants in an emergency situation. Volkswagen’s response minimizes a clear risk to consumer safety. If the software controlling door operation cannot be relied upon to perform such a critical function, it should be removed or replaced with a mechanical mechanism that guarantees the ability to open the door at all times. I respectfully request that the NHTSA investigate this matter and determine whether a recall or corrective action is warranted. Thank you for your attention to this important safety concern. Please let me know if additional documentation or video evidence would be helpful to your investigation. Sincerely, [XXX] [XXX] [XXX] [XXX] 2022 Volkswagen ID.4 Pro S AWD [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
On [XXX], my 2022 Volkswagen I.D. 4 burst into flames and exploded while it was parked in my apartment complex parking structure ([XXX] ). My car was completely engulfed in flames, nearby cars as well as the parking structure experienced heat damage. My life and others were put in danger because even though my car was electric, it was parked in a resident garage near other cars who are gas operated, which could have caused an even more catastrophic damage if the fire would've spread more. Dallas Fire Department was present, and a fire report was conducted (Incident number #[XXX]). As of today, the vehicle has only been inspected by the Dallas Fire department, as well as a virtual video meeting with my insurance agent from Allstate. Before the incident the car charged normally, was operating normally and there was no indication or warning signs to indicate that there were any issues that would have caused this. I understand Tesla started this trend of reducing horsepower based on the car's State Of Charge; however, there's times when having access to full horsepower can help collisions, especially rear end collisions. My car gets full horsepower when it's SOC is at 90-100%. Then is gets less as the SOC gets lower. It should get 100% horsepower regardless of SOC when stepping down on the pedal 100% to where I can fell a switch is being pressed. At freeway on ramps when I'm trying to merge, when my car's SOCis at 30%, it drives slower than at 80% and more obvious at 100%.
1. I still can't make sense of what exactly malfunctioned at my car accident on 9/26/2025 but I have reported to the Volkswagen Customer Service on 9/27/2025 and they couldn't give me a precise answer and told me they will have to research and call me back yesterday - 9/30/2025 but I haven't got a call as of this writing. It basically activated something that accelerated suddenly the car and it is like someone took control over the car causing me to crash with 2 cars in front of me. Yes, it is available for inspection. 2. I don't feel safe anymore with that car and also I am terrified that innocent people's lives will be put at risk. 3. I have seen some articles around the internet with the same experience that I had and it looks like Volkswagen has a law suit about it. 4. No, but I have reported to 911 and my insurance. 5. No, there's no any warning on the car dashboard before this incident happened.
[XXX] [XXX] September 14, 2025 National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation 1200 New Jersey Avenue, SE Washington, DC 20590 Subject: Formal Complaint – Safety Defect in 2022 Volkswagen ID.4 (Power Steering and Brake Failure) Dear Sir or Madam, I am submitting this formal complaint regarding a serious safety defect in my 2022 Volkswagen ID.4, which resulted in a crash and a minor injury to my wife. This defect poses a substantial risk of death or serious injury to the driving public. On [XXX], my wife experienced sudden unintended acceleration, followed immediately by complete loss of braking and steering control. She was unable to avoid leaving the roadway and entered a ditch, sustaining a minor injury. The vehicle suffered fender damage. The vehicle was towed to our authorized Volkswagen dealership. The dealership’s technician documented that the power steering module failed. However, Volkswagen of America subsequently dispatched an engineer, who instructed the dealer not to communicate further with us. Volkswagen headquarters later claimed the power steering module failed only because the vehicle entered the ditch—an explanation directly at odds with the technician’s report and a witness account. Volkswagen has refused to accept responsibility, declined to cover medical or repair costs, and denied replacement of the failed power steering module. Our insurance provider also declined coverage, determining that the accident was caused by a mechanical failure. Legal and Regulatory References 1.49 U.S.C. § 30118 – Motor Vehicle Safety Act Requires manufacturers to notify NHTSA and vehicle owners when they determine a motor vehicle contains a defect related to motor vehicle safety. The failure of the power steering and braking systems clearly qualifies as a “safety-related defect” because it directly compromises steering and stopping ability. 2.49 C.F.R. The airbag sensor turns on and off, "airbag error", the charging door plate fell of the car while driving in the freeway
The automatic cruise control failed and disengage with a sudden braking. After that the following warnings appear: Front assist has limited availability, rear traffic is currently not available, proximity sensors malfunctioning. I took the vehicle to service and diagnostic suggested the replacement of brake booster. Service department recommended a replacement a cost of $2,800 as the vehicle is out of warranty (more than 50K miles), I paid $295 for diagnostic. I currently don't have the funds for repair. Failure of braking system is a serious matter. I am not sure if I should continue driving this car, unfortunately I need it to go to work.
After the 3.5 software update OTAthe airbag error showed up and I am reading on forums that multiple folks have this issue but no recall
My Volkswagen ID 4 Dashboard display constantly gets rebooted in his black while I’m driving. I had the same issue last year, but in that issue, even though speedometer was not visible, Fortunately, I’m not Seeing a black screen Behind steering wheel But the main console gets constantly rebooted and all black, CarPlay does not work and it’s a big distraction when when I’m driving and a big safety risk, My calls get constantly dropped, and The entire center dashboard just keeps getting rebooted every two seconds and flickers
When my car starts in ECO mode, meaning ECO mode was the last setting on my last drive, the car appears to drive in ECO mode given the pedal's delay in power; however, on freeways, it can easily go pass 80mph, which it isn't supposed to. Then when I change my car mode to any other mode after starting my car in ECO mode, so Comfort, Sport, etc, and then switching back to ECO mode, I noticed the pedal now has more resistance then it did before. It feels like now, it's 100% ECO mode because I get way better efficiency now. For example, yesterday, when I drove to work and my car started up on ECO mode, it took 6% to get to work. Today, after starting up my car when it is already in ECO mode from the last drive, then switching to comfort or sport mode, then back to ECO mode, it took 4% to get to work. Both drives are on the same road, same speed limit and AC set at 70F for the entire drive for both days.
Thei vehicle was purchased in 2022 and so for either the dealership or the VW did not address the issue. I am not able to lease my vehicle under turo or any other online car renting programs. I was told the parts were back ordered but all the resale vehicles at dealership are already fixed for this recall issue.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
On May 16, 2024, the Office of Defects Investigations (ODI) opened Recall Query (RQ) RQ24010 to investigate the remedy and scope of recalls 23V-213 and 23V-312. The affected vehicles by these recalls included model years (MY) 2021 through 2023 Volkswagen ID.4 vehicles. ODI received reports of the vehicle’s doors opening while the vehicle was in motion post recall remedy. ODI, also, received similar allegations on vehicles that were outside the scope of the recalls. Volkswagen issued recall 23V-213 on March 29, 2023, and recall 23V-213 on May 03, 2023. These recalls addressed the defect of water ingress into the circuit board assembly of the door handle. Water ingress can cause communication issues between the door handle and the door control until. This malfunction resulted in the vehicle door inadvertently opening while the vehicle was in motion. Over the course of the investigation, ODI received 17 unique VIN VOQ complaints of the vehicle doors opening while the vehicle was in motion occurring either post recall remedy or on a vehicle that was outside the recall scope. Volkswagen submitted 190 unique VIN complaints reporting the same circumstances. On September 4, 2024, Volkswagen submitted recall 24V-651 to address the issue of inadvertent opening of the door while the vehicle is in motion. The scope of this recall expands the affected vehicles to include all MY2021-2024 Volkswagen ID.4 models, including those remedied under the subject recalls. The recall remedy consists of replacement of the door handle assembly on all four doors as well as a software update. The software update is performed to update the door handle parameters. The new door handle will have the membrane relocated to the back of the handle to avoid standing water within the handle. Also, a protective coating will be on the front and back of the circuit board to avoid direct moisture formation. In view of the action being taken by Volkswagen, Recall 24V-651, ODI is closing this Recall Query. The Agency reserves the right to take additional action if warranted by future circumstances. To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
