  Risk and remedy

  Risk

  If the frontal air bags, seat belt pretensioners, and side air bags are disabled, there is an increased risk of injury to the vehicle occupants in the event of a vehicle crash that necessitates deployment of these safety systems.

  Remedy

  Chrysler will notify owners, and dealers will replace the OCR, free of charge. Interim letters informing owners that parts are not available yet were mailed on October 26, 2016. The recall began on August 15, 2017. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S61.

  My vehicle has an open safety recall related to defective headrests. The dealership never contacted me to perform the recall repair, and the headrests were never replaced. Mopar issued an extended warranty for this defect, but the dealership is now refusing coverage claiming it expired. The defect is now occurring, creating a safety risk, and the recall remedy was never completed through no fault of my own.

  The contact owns a 2014 Chrysler 200. The contact stated that while driving 50 MPH on the highway, the vehicle stalled, and the vehicle turned off independently. The contact was able to coast over to the right side of the road, and attempted to restart the vehicle, and the vehicle started as intended. No warning light was illuminated. The vehicle was taken to AAA where it was diagnosed and determined that the totally integrated power module (TIPM) needed to be replaced. The vehicle was repaired. The contact stated that the horn was connected to the module. The manufacturer was notified of the failure and offered no assistance. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The approximate failure mileage was 106,000.

  the automobile will shut down when driving uncommanded, when it does this you lose power steering and braking and sometimes locks the steering wheel. the horn sounds uncommanded randomly. the windshield wipers will intermittently not work. have researched the problem and have found this issue is common, with this year and make of vehicle. i have contacted the manufacturer and they have been no help. they state they know there is an issue but they do not have parts to fix the issue. when it shuts down you do not get any lights or notice on the dashboard. this puts the driver and passengers at risk and other drivers. the manufacturer stated on the phone its basically my problem and since they do not have parts to repair it i'm on my own, on finding the part, most likely refurbished

The car would start back up, but the same issues would happen, making it extremely dangerous to drive. This happened on several different occasions. One of my issues with this problem is that Chrysler had the same issue with the 2015 Chrysler 200 and the 2015 was recalled. If you look at sites other than Chrysler, customers had reported the same problem with the 2014 and it looks like Chrysler just swept it under the rug. The dealership is saying that the TIPM system needs to be replaced on my car, for a cost of $3000.00 for a know problem.

  Both the driver and passaged (front seat) head rests have deployed. When looking inside them (parts are exposed since they have pooped and stayed open in a projected position) broken pieces are visible. Therefore, the headrests cannot be popped back into their proper position. This is extremely dangerous because the headrests could have deployed while we were driving. Also, the headrests no longer serve their purpose of providing safety to people riding in the front seat. There was no car accident or hit from the back. The headrests deployed (popped open like an air bag and cannot be closed) on their own. Many Chrysler 200 owners have complained about this safety issue. Chrysler should do something to help us consumers. I’m disappointed.

  A couple of years ago my passenger head restraint deployed while parked and would not reset. I was charged $652.00 to re place. Chrysler later reimbursed me for this expense and extended my warranty for this for one year. Two days ago the driver side activated while the car was parked. That means the warrant extension had passed. Did some research and this is a common problem caused by the weakening of the plastic that is under pressure of the retaining springs. This problem not only affects the Chrysler 200, but 300s and several keep and Dodge vehicles manufactured between 2010 and 2015. It is just by the grace of God no one has been injured. Mileage was 78000 the first time and 106500 the second time

  Horn didn’t work and car stalled in the middle of highway. Part (clock spring)inside horn was replaced but horn still went off by itself or didn’t work. I noticed this morning leaving my house heading to work as I was driving down my street a rattling noise and looked over to see the head rest had ejected! Thankfully I didn't have any passenger's in my car! I have no warning lamps other than low tire notification that refuses to go away after taking my car several times to make sure the tires are properly inflated and a check engine light that ofcourse came on today. I'm home and searched how to fix it and came across an article stating that I need to report this. I don't have any files. Just a pic of the headrest, current milage and notification lights. I picked the vehicle up today and it seemed to run smoothly, then it stalled out driving thru an intersection without warning.

  My wife and daughters almost died in a car accident due to the apparent failure of this vehicle. We were driving on an HWY at night and the car just died out of nowhere. Dash started to flash and just died. We initially thought that it was the mechanic that had just fixed the car but after an investigation concluded that it was not anything they could have done. My family sustained life-threatening injuries and almost did not make it out alive as thee were rear-ended by a truck. Then on top of it when they were hit no airbags deployed which is what caused the injuries to be so much worse. After reading multiple complaints on NHTSA about the same issue for the same year and make I am baffled that this issue has not been addressed. I almost lost my family.

Appears to be a major safety concern which could lead to an accident. I will be attempting to address this issue quickly as it represents a major safety problem. It is available for inspection for now. Had this happened while driving I believe an accident would have resulted. I will be contacting the dealer and an independent shop on Monday. At this time it has not been inspected by anyone official. No warning signs were noted.

  I received recall alert for code V51 (re-program powertrain control module) over a year ago (dec 2019) and the dealership has been giving the run around. I've called numerous times to inquire about getting it fixed and every time I'm told the manager will start making calls to owners for the recall but no such thing has occurred.

  The Office of Defects Investigation (ODI) opened PE19014 on September 9, 2019, to investigate 128 consumer complaints alleging that the Active Head Rest (AHR) inadvertently deployed without the occurrence of a crash or other rear impact event in model year (MY) 2014 Jeep Grand Cherokee and Dodge Durango vehicles.When ODI opened the investigation, ODI was aware of thirteen alleged injuries.A small number of Early Warning Reports identified driver and/or passenger front seat AHR inadvertent deployments without a rear impact event.Reported safety consequences resulting from the AHR deployments include concussions, headaches, soft tissue injuries, disorientation, nausea, vomiting and/or loss of vehicle control as a result of being surprised and distracted by being struck in the head by the inadvertent AHR deployment. On September 24, 2019, ODI sent an Information Request (IR) letter to Fiat Chrysler Automobiles (FCA).ODI requested certain detailed information from FCA related to the following subject vehicles: MY 2010-2014 Dodge Avenger 200, MY 2010-2014 Dodge Calber, MY 2010-2014 Chrysler Sebring, MY 2013-2015 Ram Tradesman, MY 2012 Ram Van, MY 2010-2019 Dodge Grand Caravan, MY 2010-2016 Chrysler Town and Country, MY 2010-2017 Jeep Compass, MY 2011-2020 Dodge Durango, MY 2011-2020 Jeep Grand Cherokee, MY 2010-2019 Dodge Journey, MY 2010-2012 Jeep Liberty, MY 2010-2011 Jeep Nitro, and MY 2010-2011 Jeep Patriot vehicles.On December 2, 2019, ODI received a response from FCA that included vehicle production data, consumer complaints, field reports, warranty claims, manufacturer actions, production changes, and other data related to the alleged defect from the start of production (2010) through September 24, 2019.On January 27, 2020, ODI received an update to the December 2, 2019, IR response from FCA.On September 22, 2020, ODI requested additional subject vehicle AHR information from FCA.ODI received FCA's response on October 21, 2020.ODI subsequently received an update to FCA's October 21, 2020, response on February 16, 2021.During the investigation, ODI requested updates to certain IR questions on two occasions.FCA provided updates to question #2 on March 5, 2021, and questions #2 and #5 on May 26, 2023. FCA’s IR response data identified approximately 7.5 million subject vehicles (with approximately 15 million AHR assemblies).Due to the large number of vehicles and failure reports, ODI focused its analysis on the FCA vehicle model with the highest failure rate to represent the worst-case scenario.For this purpose, ODI focused on MY 2012-2015 Jeep Grand Cherokee vehicles for its analysis.This population includes 710,890 vehicles that FCA manufactured between August 19, 2011, and August 8, 2015.ODI identified 2,839 alleged AHR failures within this limited population.ODI identified 263 alleged injuries that were caused by AHR failures in MY 2012-2015 Grand Cherokees vehicles.Of these alleged injuries, ODI categorized them as eighty-two (82) soft tissue (neck/whiplash, bruise) injuries, four (4) concussions, fifty-nine (59) headaches, and one hundred eighteen (118) without a specific injury description.Although complainants have alleged injuries, ODI has been unable to obtain evidence to substantiate or validate any injuries where medical treatment was required.Consumer outreach was conducted by ODI, where sixty-three (63) FCA complainants with alleged injuries were contacted.Twelve of the sixty-three FCA consumers responded and were interviewed by ODI.ODI requested medical records, however, only one response was provided. Attorneys representing the responding complainant provided a synopsis that indicated they had experienced prior neck-related injuries dating to the mid-1990s.ODI also contacted one hundred (100) consumers who submitted Vehicle Owner Questionnaires (VOQ) in an attempt to validate alleged injuries. Despite the efforts of ODI, the office has been unsuccessful at obtaining substantive documentation/evidence supporting complaints that allege serious injuries (where pre-existing medical conditions did not already exist). In addition to the low severity of alleged injuries caused by AHR deployments, there have been eleven alleged crashes and two alleged lane departures within the entire subject vehicle population that has been in the field up to sixteen years (with over 8,500 AHR inadvertent deployments).ODI was not able to validate that the alleged crashes were the result of inadvertent head rest deployments, and the crashes were minor in nature. ODI has conducted a thorough investigation of the alleged safety defect and with up to sixteen years of exposure for some vehicles and over 8,500 inadvertent AHR deployments, no serious crashes or injuries could be validated in the sub-population of focus.The lack of serious injuries validated by ODI with such large period of field exposure is consistent with thework done by NHTSA’s Human Injury Research Division and the Biodynamic Research Corporation, which support that an inadvertent AHR deployment is not likely to cause serious injury.In addition to the low risk of injury, FCA released an extended warranty program that covers inadvertent AHR deployment for ten years / unlimited mileage from the vehicle's in-service date. PE19014 is closed with the FCA extended warranty program.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the Agency reserves the right to take additional action if warranted by new circumstances. Installing a R8453637A$ or R8599680A$ in a 2014-18 VF will result in a 2014-18 MY core being returned. Mopar can no longer convert 2014-18 VF 62TE core to the 2019-21 version. Continued mis-usage will result in a core shortage. Mopar may deny warranty claims where the incorrect version is used in 2014-18 VF. 3. The engine to transmission dowel pins are critical to the longevity of several components. Verify the dowel pins are in the rear face of engine block before installing a replacement transmission. Mopar will add loose dowel pins with remanufactured 62TE. 4. All 62TE equipped vehicles. If vehicle is jerking/slipping with MIL code(s) P0740 or P2764, check TCC Solenoid operation. If TCC Solenoid is ok, the codes are indicative of a torque converter failure. Replace the torque converter. 5. There may be incorrect fluid measurement on the dipstick tool when checking 62TE fluid level after filling the ATF as per service library specification. Please change with new valve body pan P/N:68145886A$ as there may be incorrect valve body pan is used in Transmission.
