NHSTA SAFETY COMPLAINT TEXT Safety defect and Incident Chronology: I am reporting a catastrophic, recurring sudden unintended acceleration (SUA) defect originating strictly from a forward idle state, culminating in a severe bidirectional runaway and property damage crash. This vehicle utilizes an Electronic Throttle System-intelligent (ETCS) that failed to trigger a Malfunction Indicator Lamp (MIL) , Check Engine Light or fail safe limp mode during all three events. INCIDENT 1 (MAY 15, 2026): While maneuvering at low-speed idle into my residential garage, the engine un commandingly surged . I was tapping the brake to go cautiously. Simultaneously, the brake pedal became rigid and physically impossible to depress. The vehicle crashed forward into my tool benches and drywall. INCIDENT 2 ( on or about May 22,2026): Under the identical conditions, the vehicle without command surged forward with total brake rigidity. Fortunately, this time it caused no damage. INCIDENT 3 (June 10, 2026) During a slow speed idle parking maneuver , the vehicle violently surged forward with total brake rigidity and struck shelving. To cut propulsion I immediately shifted out of Drive. Instead of neutralizing the event the car the car underwent a severe bidirectional runaway, violently accelerating in REVERSE for approximately 35 feet across my driveway crashing into my woodpile . I ended up moving forward and stopped somehow. TECHNICAL EVALUATION for INVESTIGATORS: The recurring rock hard brake pedal serves as empirical pneumatic proof that the throttle body valve uncommandedly opened wide while my foot was actively modulating the brakes, instantly destroying the engine manifold vacuum reserve. Furthermore, the vehicle accelerating forward and backwards proves system failure. The vehicle was transported via flatbed to PAPPAS Toyota. Toyota Case # TA 261680161. I request NHSTA safety Egrs secure and audit this transient ETCS-I failure mode.
Copper wiring on driver seat caught fire causing damage to driver door driver harness underneath the dash and electric seat mechanism also caused staining to the fabric and interior

On march 26,2020 I went to drive my car. The dashboard was cracked, sticky, and melted throughout. the next day I called priority toyota and was told (john service manager) that they would not fix it due to the warranty enhancement notification-ze6 which ended may 31, 2017. Also, I had owned the car for more than 10 years. He would replace it at my cost of approx. $2200 but he would provide an exact amount when I decided to fix it. then I explored the internet to see the scope of the problem. it can lead to visibility problems (glare) and problems with the dashboard airbags. Yesterday, april 7, 2020 I called the toyota customer assistance center 1-888-2709371 and spoke to randall who repeated what john had said, but added that my car was old and not worth much and I had gotten my monies worth out of it. I asked who I could call next and he said he was the end of the line. I was assigned a case #[xxx] but can't see to read it for myself. the car has remained in my garage since I saw the problem on march 26. it IS getting worse. It was last driven on march 5 with no problems. I have a toyota rav 2017 from priority toyota, hampton, va 757 838-5000. Please help me if you can. *tr *ln*dt information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(B)(6).'*jb*jb
My dash IS cracking and falling apart. what kind of engineering IS this that the dash falls apart in such a short time in a car that IS supposed to out last its competitors.
Toyota issued an extended warranty for this very common sticky dashboard issue. It creates both a safety issue - the dashboard becomes so reflective that you cant see the road - and IS disgusting. I was told by the dealer that I was on a waiting list... To date I never was told that they ere able to fix it and now they are saying the term IS over for getting it repaired. They are clearly at fault. What do I do to get it repaired now?
Fails to accelerate when the pedal IS pressed or revs to quickly when the pedal IS pressed-see rk. found electronic throttle control fault. There IS no throttle movement until the accelerator pedal IS pressed approximately 1/3 to 1/2 way to the floor. Then the vehicle lurches due to rapid throttle opening to 1/3-1/2. there are no stored diagnostic codes. the vehicle IS not going into fail safe mode as it should. With any throttle or accelerator pedal malfunction. the vehicle never entered fail safe mode even after repeated malfunctions. the vehicle IS unsafe to drive.
The dash board IS cracking and falling apart
Complaint regarding sticky, shiny dashboard. I just purchased a 2006 toyota solara. Complaint IS about the dashboard. (1) the stickyness of the dashboard - holds dust and grime and can not be cleaned, and (2) the sticky surface results in a shiny/reflective creating a safety hazard as it reflects glare both day and night on the windshield. This IS a health and safety issue. I bought in oct 2017 and immediately took it to the toyota dealership near me. I was told due to the car manufacture date, I was not able to take advantage of the recall of the dashboard from toyota. I am told the dashboard part IS $1,800 - wow! This IS a health & safety issue and I demand my dashboard be replaced at no charge to me.
Cracked dashboard not sure if air bags will work
Abnormal operation between accelerator and throttle. Tech tested throttle body and tests passed. Tech tested accelerator pedal and values out of range. Tech would think this could cause a dtc to set if not functioning properly but there were no codes. At random moments, the car will not accelerate. Cannot get answers. Occurs most often after idling at a light then starting to accelerate it won't move. This has occurred over the years so we have had the throttle and filter cleaned twice in the past but problem IS persistent and becoming more frequent.
I was sitting in my car stopped behind school bus with my seatbelt on .i was rear ended my shoulderbelt did not lock and my visor dropped down and severed my forehead and I hit the top of my head.i needed 50 stitches in my forehead and needed plastic surgery. The visor, which IS all steel , sliced my forehead open.i do not think a steel visor should be in front of a drivers head.
Dashboard has become sticky and shiny. This has produced a glare making visibility an issue. Cleaning doesn't help.
My 2006 solara convertible has deep cracks all along the dash end to end which may affect the operability of the airbag. it's shiny, goofy, sticky which definitely affects visibility. I have no idea what else it affects. I thought it was caused by heat until I read the some 75 exactly identical complaints. toyota needs to recall the dash rather than ask we consumers to pay $2,400 to fix it. this was a $32,000 vehicle. bad enough that the adhesive around the back window dissolved, now this. it's undrivable, unsaleable and I only have 27000 miles on it
Yesterday, july 26, 2015, I received in the mail a second letter from toyota motor sales customer experience department regarding the warranty enhancement program - ZE6 concerning the replacement of the sticky dashboard of some make and model. mine IS a 2006 toyota solara. it's ridiculous why the warranty enhancement IS only for the sticky dashboard when the steering wheel and inside door panels are made of the exact same material. these parts must be covered as well especially we touch these parts every single time we use the vehicle. on mine, I noticed the disintegration of the material since last year when I was wipe cleaning the interior of my solara. this warranty enhancement must be extended to fix the material on the steering wheel and door panels. I know I'm not the only one experiencing this because I read blogs and there are others experiencing the same issue even on lexus 2006. you should look into this because we pay a lot in purchasing our vehicles and we deserve our money's worth.
My dashboard IS sticky and IS cracked from driver to passenger sides.also IS cracked where airbags are. Dashboard IS also shiney and suns reflections makes it dangerous driving. I am afraid airbags will inflate.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
The petitioner owns a my 2006 toyota camry with a 4-cylinder engine that was purchased new in january 2006. The petitioner also previously owned a my 2005 camry. He alleges that both vehicles exhibited vehicle engine surging, which he described as a short duration (1 to 2 second) increase in engine speed occurring while the accelerator pedal IS not depressed. In an initial interview, the petitioner estimated that 6 to 8 surge incidents, of varying severity, occurred in the my 2006 vehicle over the course of 10,000 miles and 7 months of ownership. The petitioner reports that the brake system IS effective at overcoming the surge. However, he IS concerned about reports filed with nhtsa alleging uncontrolledsurging in my 2002 to 2006 camry vehicles bringing those vehicles to a high rate of speed (in some cases, purportedly, with the brakes applied).after review and analysis of the available information, odi has not identified a vehicle-based defect that would have produced the alleged engine surge in the petitioner?s vehicle, nor was it able to witness such an event when road testing the petitioner?s vehicle.evaluation of a suspect throttle actuator removed from the petitioner?s vehicle did not reveal a component problem. Warranty and parts sales of the actuator are unremarkable.these data do not support the existence of a wide-spread defect or ongoing concern. The fault detection and reaction strategy described in toyota?s technical documents indicates that a loss of throttle control due to a component or system failure would be detected within a one second periodafter which engine power would be limited. The petitioner?s my 2006 vehicle brake system overcomes full engine power at easily achievable brake pedal forces. Based on the analysis conducted, it IS unlikely that nhtsa would issue an order for the notification and remedy of a safety related defect at the conclusion of the investigation requested in the petition.consequently, in view of the need to allocate and prioritize nhtsa?s limited resources to best accomplish the agency?s safety mission, the petition IS denied.please see the march 9, 2007 federal register notice for further details.
