The brake booster assembly failed, as many of these models had before. A bunch of codes went off as I was driving and the brakes did not function properly. Toyota had an extended warranty for this which they discontinued last year and they would extend no good will towards the $4,000 repair

Emergency Brake, Traction and ABS warning came on and stayed on in addition to a five-second interval cycling on of a motor sound under the hood. O'Reilly Auto Parts and then Toyota dealership confirmed the same code that indicated brake booster assembly needs replacing (C1391) **This is a "known issue" but not yet a recall. Toyota did extend the part warranty to 10 years or 150,000 miles - my vehicle has only gone 85,500 and unfortunately is eight months into the 10th year and Toyota is refusing to cover any portion of the repair. I would like to be part of the effort to have this formally declared a recall because I got the quote today to replace the brake booster assembly and it's going to cost half the value of the vehicle! $4000 is the low estimate Bighorn Toyota gave me to do the repair/replacement. Thank you for your help!
- Service brakes
Brake light + ABS + traction light came on and the brake became harder to control. I was driving in the rain and briefly lost control. Fortunately, no accident.
- Service brakes
brake failure during operation due to bad brake booster pump and accumulator.
- Service brakes · Unknown or other
Brakes failing while driving in the rain. I was unable to use my brakes in the rain and headlights turned off while driving too. Took it to the dealer and they won't do anything unless lights are on on dashboard to read the code.

I started hearing the whirring sound from the brake booster that normally only comes on at startup almost constantly, about every 15 seconds. Along with this, a puffing sound and a pulsing from the brake pedal when stopped with the car on, such as at a traffic light. Also, when coming to a stop there would be a shudder from the brakes. After about a month I was driving when 3 separate warning lights lit up on the dashboard and the next time I braked the pedal felt very different than normal, very mushy, and the car seemed to not stop as well. I consulted a repair shop and they told me it’s a common problem with this model: brake booster/actuator failure resulting in a loss of the ABS system. They quoted me over $2000 to make the repair. Internet research confirmed that this is a common problem that Toyota is aware of but refuses to acknowledge as something to be recalled, despite the huge safety risk associated with it. This on a car with only 85,000 miles.

My Prius V has slow acceleration and the RPMs seem to go unusually high on acceleration. This makes it dangerous to pull out from a stop sign, etc. in certain situations that would be fine in another car of the same make and model. And recently it has started to throttle up when taking my foot off the gas and even when pressing on the brake. This happens intermittently and randomly. Fortunately no accidents have occurred yet and I'm trying to get the problems resolved with Toyota, but the dealer says it will be hard for them to diagnose when the problem is not occurring and there are no warning lights. Less than 60,000 miles on it.

Traveling at approximately 25 mph in a neighborhood, I had just turned into a diagonal parking space in a cul-de-sac at about 10AM and the car suddenly accelerated forward and hit a yellow iron post beyond the front of the parking space. it happened in a matter of 2-3 seconds and I couldn't stop the car by braking. the post stopped the car and caused extensive damage to the car. it was extremely perplexing and I still don't think it was caused by my actions due to the sudden acceleration. I no longer have photos of the damage. I contacted my insurance carrier and my rates increased significantly but I still feel strongly that I did not cause the accident. I had been driving at 20mph through the neighborhood prior to parking. repairs totaled about $2400.00.

September 18, 2020, driving on quiet residential street, instrument panel lit up with message "hybrid system failure. pull over immediately". before we could find a spot at the side of the road the engine shut off and wouldn't turn back on, blocking traffic. vehicle towed to local toyota dealer. engine turned on at dealership. dealer ran diagnostic tests and said nothing was wrong with car. sent us home saying car was fine. november 24, 2020 received notice from toyota about customer support program for failure of "intelligent power module (ipm) located inside the inverter assembly of the hybrid system". condition described as "hybrid system warning" showing on instrument panel and vehicle going into "fail-safe driving mode". called phone number on the notice and was told the problem we had with the car was the condition described in notice, but nothing could be done until it fails again and the diagnostic tests are positive. if they know this IS a safety problem, and it already happened once in our car, they should replace the faulty parts, not wait for us to get into a wreck on the freeway.

Issue started after 2 years, when wet and cold as I back up and put my foot on the brake pedal, a very loud noise heard coming from the back? It lasts as long as I put my foot on the pedal. Expressed my concerns at peterson toyota dealer locally, but the technician stated that "it's normal", could be the brake hydraulic booster system? The issues still continue as of today. I do record the noise when it happens
- Seat belts
The passenger side 2ND row seat belt IS stuck, it doesn't roll or release making it unusable for a passenger to use it. When I roll my fingers up where it comes out from, I feel there IS a kink, but I couldn't fix it. A seat belt shouldn't kink and get stuck. Now this seat belt IS useless for anyone sitting in that seat.

This IS an occasional and intermittent issue. at 35 to 45 mph, traveling straight, on city avenues and boulevards. when braking in an urgent fshion (not panick and not easy) the brake peddal IS soft and travels significantly further than usual before the cars breaking system takes effect. this was on dry roads and above freezing temperatures. we have not experienced any accidents due to this but there have been moments of near panick prior to the brakes working as expected. as a separate brake issue, on two occasions when panick braking from apeeds over 50 mph, traveling straight, on freeways the brakes have locked and the car has skidded. this was on dry roads during daylight hours, dry roads and above freezing temperatures. this vehicle has antilock braking system. the local toyota dealer was advised of this locking issue and reported they found no issue. we have our brakes inspected by a local toyota dealre at manufacturer recommended intervals and have been given a 'green' rating for pad thickness. the above events started late in the first year of ownership and we are the first owners of this vehicle. I ama retired engineer with a fair background with performance driving on closed circuits. I have a solid understanding of proper brake performance. I fond the above experiences disconcerting and of concern. the below date of occurrence IS for the latest slow brake response, not the locking up.

2015 toyota prius v: this car has a brake system accumulator that activates after the car has sat for a sufficiently long time (overnight, during the workday, etc). when you open the driver side door, the accumulator activates and pressurizes the brake system. however, when using the brakes while driving for the first time after the accumulator activates, the brakes are way too soft/spongy - so soft that it feels like the car will not stop and the stopping distance IS lengthened, sometimes more than doubled. my local dealer claims they have to be able to reproduce the problem to fix it. I have had other model priuses where they have said the same thing, yet the problem turned out to be a systemic issue in a certain prius model. I fear that this IS another pervasive brake issue that toyota IS trying to hide.

Prius V3 lunging backwards 1/21/2016 on thursday 1/21/2016, I was parked in a big lot and after my appointment, got in the car and turned it on. But rather than pull backwards out of the spot, I decided to first make a call using bluetooth. The call took about 10 minutes. I then hung up, released the emergency brake, and moved the stick shift into reverse with my foot, of course, on the regular brake. As I released that brake, the car lunged hard about 10 feet backward across the parking lot "drive", into where other cars could have been parked but luckily weren't. I automatically and immediately put my foot back on the brake which stopped the car. Needless to say, I was bewildered and scared by what had happened, counting myself very lucky there was no other car parked behind me. I don't know what caused the engine's lunge, I just know that I have driven toyota prius cars for the last 15 years, love them, and never had any driving issues. In researching the problem online, I saw that it might have been the car's power train, since similar lunges were described in cars a couple of years ago.

The radio/navigation/telephone system on the toyota prius v 2015 started freezing a few weeks after I purchased the vehicle. The gps, radio, telephone etc. Are all controlled by a central touch screen panel on the dashboard of the vehicle. The gps becomes un-responsive to touch and can suddenly no longer be used! This has happened repeatedly but randomly on my vehicle, and many owners are seeing the same issue on their prius v 2015 too. After a few miles of driving, the screen becomes unresponsive, and remains un-responsive until the car IS turned off and started again, especially on hot days. The only way certain owners have been able to get a permanent fix for the issue IS by a replacement of the navigation system. It appears there IS a service bulletin from toyota t-tt-0336-15 that mentions the issue. However, I have already been to my toyota service center 3 times for this issue, and they keep saying that they cannot "reproduce" the issue, and need to analyze further. I have taken multiple videos of the issue between mar 25 and aug 8 and shared with the service center. But they refuse to acknowledge and take action. if it IS a known issue, why IS toyota not recalling and replacing the unit, customers like me are wasting time, waiting endlessly and waiting for the "analysis" when toyota knows the problem, and must fix the issue proactively! please note: I have talked to the service center as well as with toyota customer care, trying to resolve the issue in good faith, but they asked me to contact nhtsa if I have any complaint, or keep going through their analysis process - they are not ready to own up to the issue and fix it proactively, not just for me, but for all owners of the affected vehicles.

- 11007445The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
- 11007444To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
- 11007443Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: Minor damage: requires only buffing to repair. Moderate damage: usually requires neutralizing, color sanding, and buffing. Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.

- 11005153Toyota vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination.
- 11005149Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid.

While the quality and durability of paint on Toyota vehicles is continuously being improved, it remains the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. To read, While the quality and durability of paint on Toyota vehicles is continuously being improved, it remains the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.
- 11003594The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. While Toyota paint quality, durability, and resistance to environmental factors is continuously being improved, it remains the responsibility of the dealer to protect and maintain the vehicles paint finish from the time of arrival at the dealership until retail delivery to the customer. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
- 11003592To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible. SUPERSESSION NOTICE The information contained in this bulletin supersedes Service Bulletin No. PD017-04. The entire bulletin has been updated.
- 11003589Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere.
