Remedy
Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is 52D. The Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov on June 11, 2026.
Chrysler (FCA US, LLC) is recalling certain 2022-2026 Pacifica and Voyager vehicles. The right and left side curtain air bags may have insufficient pressure retention. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 226, "Ejection Mitigation."

Air bags with insufficient pressure retention increase the risk of ejection and injury during a crash.
Remedy
Dealers will replace the side curtain air bags, as necessary, free of charge. Owner notification letters were mailed May 14, 2026. Owners may contact Chrysler customer service at 800-853-1403. Chrysler's numbers for this recall are 06D, 10D, and 11D. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning April 2, 2026. This recall expands and replaces previous NHTSA recall numbers 25V302 and 25V573. Vehicles previously repaired under a prior recall will need to have the new remedy completed.

Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.

Remedy
Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. The vehicle experienced a concurrent failure of the high-voltage traction battery pack and the High-Voltage Battery Coolant Heater (Diagnostic Trouble Code P0E15). The electric battery coolant heater is a critical component of the high-voltage battery thermal management loop and vehicle cabin heating/defrosting system. When this heater fails, the high-voltage battery cannot regulate cell temperatures in colder ambient conditions, which disables electric propulsion mode, triggers powertrain limp-mode/reduced propulsion warnings, and leaves the vehicle unable to clear or defrost the windshield in freezing temperatures. A sudden loss of propulsion power in traffic or loss of windshield visibility during cold weather presents severe safety hazards. The manufacturer approved the traction battery pack under warranty but denied statutory coverage for the concurrently failed high-voltage coolant heater, leaving the vehicle disassembled and inoperable.

The vehicle was left in parking on a Saturday afternoon in Costco. When we came back and we put everything in the trunk, the vehicle turn on automatically, not by remote start, on its own. When we saw the dashboard it said: "Put on in parking to prevent rolling", but the vehicle didn't take any gears and it was not able to be turn off. We try to turn it off several times but it came back on, on its own with the same message: "Put on in parking to prevent rolling", after towing we dropped in the dealer and the dealer couldn't find anything not even a computer code. This could have happened on a semaphore waiting for a light and maybe the car would have rolled on its own? that's something that it is concerning.

We purchased 2022 hybrid Chrysler Pacifica from Mountain View CDJR in June 2026 and around July 4th the car went into critical red turtle limp mode on the highway slowing from 65 to under 15 mph with all our children in the car. Thankfully my husband was driving and able to get us to the local CDJR who helped inspect the vehicle and discovered contaminated coolant issue and showed us the different colors that shouldn’t be there from a car purchased from a CDJR dealership. The issue was brought to Mountain View who initially attempted to charge us over $3,000 for an issue stemming from their own negligence. After agreeing to pay around $600 for new radiator and more back and forth I believed the car would be returned to us well serviced. There was a recall they were supposed to handle and also they had the vehicle during a prescheduled oil change and then failed to perform the oil change. After then vehicle had been returned to us the hybrid capabilities have failed, we cannot charge the vehicle, and gas mileage has continued decline. When brought to service manager, he asked me “well is it still going into limp mode?” As if the car not stalling in the highway was successfully servicing the vehicle. Car went into limp mode several times before Mountain View’s service, car has since continued to decline in gas mileage and does not charge at all, and though it has not gone into red turtle or limp mode I feel there are still underlying safety issues as coolant flush was never performed. Mountain View has lied and been very combative.

Rear passenger side center section captains chair had an uncommanded back of seat drop downward followed by the entire seat rolling forward as I was slowing down the vehicle to make sure the passenger was not injured. The chair flops back and forth. Approximately 12500 miles on the 2022 vehicle. The drivers side of this seat ALSO failed but nobody was sitting in it at the time! The passenger was not injured but could have been severely injured if I had reacted with harsher breaking, if it occurred during a situation that the vehicle could not have been stopped or if the individual was unhealthy! The seat was basically freewheeling on the skids with the entire back of the seat flat! Sliding forward with breaking, sliding backwards with an increase in speed, seatbelt NO LONGER RESTRAINED individuals upper body!

This vehicle is experiencing a severe, recurring premature failure of the power sliding door lock actuators on both sides. When I took it to the dealer for inspection I didn’t know the clicking sound was a problem. But was told it needed to be replaced because it was a “safety issue.” • September 5, 2024: The Right-Side sliding door lock actuator failed prematurely at approximately 15,000 miles and had to be replaced. The dealership replaced under warranty. • Earlier This Year (2026), I don’t know exactly when, both door lock actuators are showing signs of failure by making that same clicking noise while closing. This marks the second premature failure of the right-side actuator and the initial failure of the left-side actuator. I declined to have the vehicle inspected by a dealer for this new failure because there would be a $225 diagnostic charge when I already know what the problem is. It manifest as a rapid clicking noise during the final latching phase of the sliding door shutting.The right side door regularly sticks closed and won’t open. This is a critical safety defect that puts passengers at risk. I’m concerned that as these actuators fail, the power sliding doors will not lock securely while the vehicle is in motion, creating a severe risk of the doors opening inadvertently on the highway—a major hazard for children in the rear seats. Conversely, the mechanism can stick or jam shut, potentially trapping rear occupants inside the vehicle during an emergency evacuation or crash scenario. The fact that a replacement factory part (Mopar 68185976AD) failed for a second time within a short window proves that this is not standard wear-and-tear. It is a fundamental engineering defect in the internal gears of the actuator that Chrysler has failed to permanently resolve for the 2017–2023 platform. The manufacturer needs to investigate these recurring component failures and issue a permanent remedy.

The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving up to a four-way intersection at approximately 5 MPH, a second vehicle driving approximately 50 MPH crashed into the front driver’s side quarter panel of the vehicle. During the crash, no air bags inside the vehicle deployed. The driver sustained a brain contusion and was transported to the hospital. The second-row driver’s side passenger sustained lower back and neck strain, and the third-row driver’s side passenger sustained a head contusion. Both were later driven to the hospital for medical attention. The driver of the second vehicle also sustained unknown injuries and was transported to the hospital. The vehicle was destroyed and towed to a nearby storage facility. A Police report was filed at the scene but not provided. The failure mileage was 135,000.

I have contacted Chrysler Corporate (Case # XXX) and multiple local dealerships, including Carolina Chrysler in [XXX].The manufacturer and its dealer network are actively refusing to diagnose or inspect this documented safety defect unless I pay a $206 diagnostic fee upfront. I am on a strict zero-dollar budget and cannot afford this fee. The manufacturer is using a corporate loophole to block the inspection of a life-threatening, premature component failure on a 2022 vehicle. This file serves as notice that a safety inspection is being denied by Stellantis/Chrysler over an upfront diagnostic fee. The dealership replaced the display, which failed to remedy the defect, and during service, the radio module failed completely. Vehicle: 2022 Chrysler Pacifica Hybrid. Approximately 25,300 miles On Saturday 9/12/26 while driving at highway speeds the following message displayed "Stop Safely, Vehicle Will Shut Off Soon". When the message displayed the check engine light illuminated as well as the limp mode light. In limp mode the vehicle reduced speed rapidly and would not accelerate above 20 MPH. This happened at highway speeds on a multilane highway. We were able to get to the shoulder, but it was very dangerous changing lanes in limp mode. Turning the vehicle on and off several times cleared errors except the check engine light. We were able to get off the highway and get home without the message or limp mode triggering again. The check engine light stayed illuminated for 24 hours and went off when I refueled the vehicle the next day. For the next week I drove at local speeds with no issues. On Saturday 9/19/26 the vehicle would not start. This is a plugin hybrid and was fully charged. The electric motor would not start, and the check engine light illuminated again. I turned the vehicle off/on 3 times to get it to start. I did my errand and returned home. When I returned home, I turned the vehicle off then on again and the check engine light cleared. On Tuesday 9/21/26 I dropped the vehicle off for service, getting there without issue. The dealership stated the transmission will need to be rebuilt. Problem code is P0A43-00. This is the SECOND transmission failure since ownership. I've had the vehicle serviced only at the dealer where purchased. The dealership confirmed the FIRST incident with the same problem code was diagnosed on 1/30/24. The transmission was replaced at this time. The van had 12,134 miles. The 2024 incident also was triggered at highway speeds. If my memory is correct, I believe I reported the first incident to the nthsb at the time.

When I bought it, there was a suspension noise that appeared to be due to strut mounts. Due to this noise, dealer lowered the sale price. Since the time of purchase, I have replaced both struts, inner & outer tie rods, lower control arms, sway bar links and both cv axles. And it is still making the same noise, only it has gotten much worse. Based on the last mechanic's inspection, he states that the noise is from a bad steering rack. I reviewed this online and found that NHTSA has an open investigation for the steering rack and would like to see if there is anyway that you can help me with replacing it as it poses a severe safety hazard and is very expensive to replace. I was driving my Pacifica with my special needs children to thier therapy appointments. The power steering light came on all of a sudden going down the highway at 50-55mph. Steering became extremely difficult. I was barely able to take the the turn to pull into the parking lot. This is even more dangerous as their is a drop-off on the other side if i had not managed to turn aggressively enough. It scared both of my children and myself. The dealership wanted $206 diagnostic fee which we cannot afford. The light will go off and we regain steering once it has been off for a few minutes then light kicks on and we lose steering again.

I am filing a safety complaint regarding Chrysler Corporate's handling of recall campaign 06D (NHTSA 26V-189) for my 2022 Chrysler Pacifica.I received an official corporate notification via mail explicitly stating that parts were now available to remedy this active safety recall. However, when I immediately contacted my authorized Chrysler dealership to schedule the critical safety repair, I was turned away and informed that the parts are completely unavailable and backordered with no estimated release date.Sending out official notices claiming an open safety risk can be fixed when the necessary components are entirely missing from dealer inventory is a highly deceptive and unsafe practice. It causes vehicle owners to waste time coordinating service under false pretenses while forcing us to continue operating family vehicles with known, unaddressed safety hazards. Chrysler must be held accountable for coordinating its corporate communications with actual supply chain availability so owners are not driving compromised vehicles under the false impression that a fix is ready. It is alarming that the post card they just sent me had big letters "URGENT SAFETY RECALL" headline and the recall site says the parts are in and the dealership says they only gets parts to fix 2 a week and it will still be close to 6 months before the parts are available.

Vehicle suffered sudden loss of motive power due to Diagnostic Trouble Code P0A43 (Drive Motor A Position Sensor Circuit Intermittent). The failure is caused by thermal and chemical degradation of the internal transmission wiring harness, resulting in an internal electrical short submerged in transmission fluid. The vehicle is able to be powered on again and driving can be continued however certain speeds trigger the issue. The check engine light appears and remains for a short period. The vehicle was purchased through CarMax of Naperville, IL [XXX]. The vehicle began to experience the defined issue the last week of May 2026. The vehicle was brought to CarMax [XXX]. CarMax transferred the repair of the vehicle to Castle Chrysler of Naperville/Stellantis IL, and they conducted the initial repair. Stellantis attempted to mask this hardware failure via software recalibrations under NHTSA Recall 03A (23V-010) rather than replacing physical parts. The vehicle was returned and appeared to be repaired. [XXX] the same issue occurred while on a cross country road trip. The vehicle was able to be driven but under specific speed conditions as not to activate the transmission failure. [XXX] the vehicle was brought to an independent technician at Stephens Automotive in Palatine, IL and they were able to verify the issue and documented it. The vehicle was then brought to Castle Chrysler on [XXX], and the vehicle has been there since. Stellantis later acknowledged the true physical failure via Warranty Extension XE1 and TSB 21-014-26, requiring Electric Rebuild Kit #68729719AB. The vehicle has been continuously grounded at Castle Chrysler since [XXX] (>30 cumulative days) because Part #68729719AB is on indefinite backorder. Stellantis's failure to supply hardware repairs leaves the vehicle inoperable and creates a severe safety hazard on public roads. Stellantis Case #[XXX]. This is abysmal customer performance and and unacceptable turnaround time for what amounts to a serious safety risk. Chrysler has not and continues not to provide an estimated timelines for resolution. I am renting a vehicle in the meantime given the safety risk (both while driving and while parked), paying for the rental cost out of pocket and taking a risk that Chrysler will reimburse. In short, the manufacturer has failed or is unable to remedy this safety recall for this vehicle in a timely manner.

The repair is estimated over $300 which is ridiculous! This should be a covered repair under a recall as it seems to happen frequently.

I am unable to drive it and it officially needs its entire transmission rebuilt. My wife was driving back from work and the car shutdown in the middle of the highway and she had to risk her life to get it to an emergency zone. The turtle symbol came on the dash and the dealer confirmed the transmission needs to be rebuilt because of a manufacturer defect.

This causes the seats to move forward and back sliding when the car is in motion as they will not stay locked in place. They cannot be used for transportation due to it being unsafe for the passenger as the seat does not remain upright. The seat is available for inspection. The seats have not been inspected by the dealer/manufacturer as they are wanting to charge a large service fee and do not have appointments available in a timely fashion. There were no warnings, the seats each failed suddenly upon regular use.

On July 19, 2026, I was driving my 2022 Chrysler Pacifica Hybrid on E-470 at approximately 75 mph. Without warning, the dashboard displayed "Stop Safely. Vehicle Will Shut Off Soon," and the amber check engine light illuminated. At the same time, the vehicle immediately lost all motor power, began slowing, and did not respond to accelerator input. There were six occupants in the vehicle, including four young children. I was traveling on a busy six-lane highway with little to no shoulder. I had to maneuver across traffic without motor power to reach the side of the road, placing all six occupants at immediate risk. After turning the vehicle off for about five minutes, I restarted it. Only the check engine light remained illuminated, and the vehicle appeared to have normal power. We continued driving at 55–65 mph to reach a safer location. About 10 minutes later, the same warning appeared and the vehicle again lost all motor power. This time we were on a steep, busy exit ramp and had to immediately pull off as the vehicle rapidly lost speed, again placing all six occupants at risk. After waiting about five minutes, we restarted the vehicle, drove it to a safe location, and had it towed to a dealership. The dealership was closed, so the cause of the failure has not yet been determined. This incident created an extremely dangerous situation, and we narrowly avoided a collision on the interstate. A sudden loss of propulsion at highway speeds without warning is a serious safety hazard that warrants investigation and consideration for a recall.

Vehicle repeatedly entered limp mode / lost power at highway speed a few days after the dealer performing the remedy for NHTSA Recall 23V- 010 (FCA Recall 03A, transmission internal wiring connector short). The recall remedy did not correct the defect. Prior to the recall work, the van entered limp mode on 2 other occasions. Timeline: On 7/10/26, Park Chrysler Jeep in Burnsville, MN performed the open recall remedy (Recall 03A / NHTSA 23V-010) on our vehicle. We informed the dealer we were departing on a multi-week interstate family road trip, and the dealer released the vehicle as repaired. On [XXX], about 40 minutes into our trip, the vehicle entered limp mode with a severe loss of motive power while driving at highway speed on [XXX] between Northfield, MN and [XXX]. We were able to reach the shoulder safely. After multiple restart attempts, the vehicle resumed operation. On [XXX], the vehicle entered limp mode 2 additional times under the same conditions on [XXX] in IL, and on [XXX] in IN, again at highway speed and with heavy traffic present. In one case there was barely enough shoulder to pull over safely. Each event created a sudden, significant speed differential with surrounding traffic. On 7/16/26 the vehicle was taken to Bob Chandler Chrysler Dodge Jeep Ram in Columbus, OH. The dealer's diagnosis is that the transmission must be replaced. The repair order ID is [XXX]. The vehicle is at that dealership awaiting parts. Safety concern: The vehicle lost motive power at highway speeds 3 times in two days after the recall remedy was performed and the vehicle was returned to us as repaired. Occupants at the time included 2 adults and 3 children. No crash, injury, or fire occurred, but each event presented a serious collision risk from sudden deceleration in traffic. This experience is consistent with the concern in NHTSA's reopened investigation that the software-only recall remedy does not address the underlying hardware defect. On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances.
