Ford Motor Company (Ford) is recalling certain 2013-2018 Focus, 2013-2019 Escape, and 2015-2016 MKC vehicles equipped with a 2.0L engine. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

An electrical short circuit can increase the risk of a fire.
Remedy
Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded plug, and remove the block heater electrical cord. Interim letters, notifying owners of the safety risk, were mailed March 6, 2026. Additional letters will be sent once the final remedy is available. A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.
Remedy
Ford will notify owners, and dealers will inspect the latch date codes to confirm if the repair was done correctly, and replace the side door latches if necessary, free of charge. Owners will also have the option to inspect their own vehicle's door latches and enter the date codes into a website to determine if the latches are correct. The vehicle was rendered inoperable. Upon research, the contact associated the failure with Customer Satisfaction Program: 15N01 (EPAS Steering Efforts); however, the VIN was not included. The vehicle has a recurring fuel-level/distance-to-empty (DTE) malfunction that has caused it to run completely out of fuel and lose motive power while the instrument cluster indicates substantial remaining range. This has occurred four times. On the first three occasions, the vehicle ran out of fuel while displaying approximately 28-30 miles DTE. On two occasions, including the most recent, the engine shut off unexpectedly in the roadway and other motorists had to help push the disabled vehicle out of traffic. During the most recent incident, the vehicle displayed approximately 35 miles DTE when the engine shut off. There was no sputtering, warning message, warning lamp, or other indication that the vehicle was actually out of fuel. I previously brought the vehicle to an independent repair facility specifically for the inaccurate fuel/DTE indication and occasional stall-like behavior. Fuel trims were checked and no obvious cause was identified. I subsequently learned that Ford Safety Recall 18S32 had been performed on this vehicle in 2021, before I owned it. That recall addressed excessive fuel-tank vacuum/possible tank deformation and identified symptoms including inaccurate fuel gauge, inaccurate DTE, and possible stalling. I then brought the vehicle to an authorized Ford dealer and repeatedly identified the inaccurate fuel/DTE condition as my primary concern, including providing it in writing. The dealer performed a PCM update under another campaign, but the fuel-level/DTE malfunction was not diagnosed or repaired. After that dealer visit, the vehicle again ran out of fuel and shut off in the roadway, this time while displaying approximately 35 miles DTE. Roadside assistance was requested. The vehicle is available for inspection. Ford has been notified and an Automotive Safety case opened. The cause remains UNKNOWN. My concern is repeated unexpected loss of motive power while the vehicle indicates usable fuel remains.

The Transmission Control Module (TCM), within the PowerShift DPS6 Automatic Transmission, failed completely on a 2014 Ford Focus (72,000 miles). The failed component is no longer physically available for inspection as it was retained by the independent service facility as a core replacement, but full repair invoices and diagnostic documentation are available. The failure caused a sudden and unpredictable loss of forward motive power, severe gear hesitation, and failure to engage drive gears while operating the vehicle. The gears would slip and it made continuous forward acceleration at the speed limit nearly impossible. This created an immediate hazard of being rear-ended and/or stranded in ongoing traffic without acceleration capability. Shortly afterwards it also completely lost the ability to reverse. The problem was formally reproduced, diagnosed, and confirmed by an independent repair facility during service. The vehicle has not been inspected by the manufacturer, police, or insurance representatives. A Check Engine Light illuminated came on at approx 72,000 miles while I was running morning errands. The light came on as I started feeling some sudden rough driving after accelerating from a red light, and within 1-2 miles of driving was when the TCM started failing completely. While Ford acknowledged this systemic DPS6 defect under Customer Satisfaction Program 14M02, strict guidelines required an active diagnostic code (P090C) before approving a warranty replacement, preventing lower-mileage owners from receiving preventive service before the 10-year calendar cap expired. I was forced to pay $1,927.26 out-of-pocket for a replacement with an OEM part.

I own a 2014 Ford Focus Titanium with approximately 75,000 miles and the DPS6 automatic transmission. The transmission has developed a serious and recurring problem. The vehicle began violently jerking/shuddering while moving forward and suddenly would not go into reverse. A transmission warning light came on. After the battery was recharged, reverse temporarily returned, but the same transmission problems came back while driving. The vehicle was diagnosed with trouble code P090C:68, related to the clutch actuator system. Ford has known about recurring DPS6 transmission problems in these vehicles for years and has issued multiple technical service bulletins and customer programs addressing transmission clutch shudder, shifting problems, loss of engagement, and related failures. Ford’s own documentation has acknowledged conditions that can result in loss of transmission engagement, loss of power, and other driveability problems. My Ford dealer told me Ford has known about this problem for a long time but considers it not to be a safety issue and therefore will not pay for the repair. The dealer estimated the repair at $6,077.31, and I was charged $159 for the diagnosis. I believe this is a serious safety concern. Sudden jerking, unpredictable transmission engagement, inability to reverse, or unexpected loss of propulsion can create a dangerous situation when entering traffic, crossing an intersection, turning, accelerating from a stop, or attempting to move out of the path of another vehicle. I am concerned that Ford has known about these recurring transmission failures for years, yet affected vehicles can still experience potentially dangerous transmission failures without a permanent safety remedy. I request that NHTSA investigate the DPS6 transmission in 2014 Ford Focus vehicles and determine whether these known defects constitute a safety-related defect and whether a safety recall or other corrective action is warranted.

Purchased 2014 Ford Focus from a private dealer. The vehicle had 75k miles on it and needed a reliable vehicle for my daughter who is in college. The same day as the purchase, the engine light came up as well as the transmission light. Had to tow it to ford and was advised the TCM needed replacement. Apparently this is a well known issue but the vehicle wasn’t covered under a recall for the issue. Paid $1960 to get that repaired as well as $135 for a tow. Ridiculous and Ford will not do anything about it.

A few days later and still smelling gasoline, I took it to tire place so they can lift it up to check the bottom where the fuel tank is located and the immediately saw fuel/gasoline leaking from the tank. I took it back to the Ford dealership and complained how they missed it the first time. They checked it and told me the fuel tank was completely ruined from excessive vacuum effect of the previous canister purge valve issue and that I needed a new fuel tank. They put in a new fuel tank and I told them this issue had a recall on the same make and model of car in 2018 and from that recall, this issue had been fixed then but it just happened again. I paid $2,088 for the new fuel tank and power train control module reset. So the total I have paid to fix this issue over $2,500 and about two days later, Ford motor company issued a new recall 26S40 for this specific issue on this specific 2012-2018 make and model car. So I hope my car will be added and searchable for the recall on both Ford’s and government’s websites when they initiate it on July 6, 2026. I have all the repair receipts and will file for a refund as soon as I receive the recall letter. Thank you.

The contact stated that while driving at an undisclosed speed, the transmission failed to shift into 2nd gear, causing the transmission to slip, and the vehicle shuddered heavily while accelerating forward. Additionally, the contact stated that the backup camera failed to function since the vehicle was purchased in 2022. The vehicle possessed a long history of identical transmission failures. The vehicle was taken to a dealer on several occasions for transmission failures. The vehicle was first taken to the dealer at 56,838 miles, where an electrical failure covered under Customer Satisfaction Program: 14M02 was addressed, and the dealer replaced the TCM assembly, performed adaptive learning, and installed a replacement clutch assembly. The vehicle was taken to the dealer for a second time at 69,000 miles, where the dealer replaced the clutch assembly due to the recurring failure. The vehicle was taken to the dealer for a third time at 90,000 miles, where it was determined that the clutch assembly had failed once more and required a third replacement. The dealer was notified of the current failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 90,000.

Ford's own warranty history documents six prior DPS6 repairs between 2016 and 2023, including two full clutch replacements, three TCM/PCM reprogramming sessions, and a full TCM replacement under program 14M02 — all at Ford dealerships. The current failure occurs just 35,000 miles after Ford's most recent transmission repair. Prior repair orders document dangerous safety events including sudden loss of drive while in motion, no-start conditions, and complete loss of throttle control on the highway — consistent with the known DPS6 failure pattern. Ford issued six Customer Satisfaction Programs covering this transmission over eleven years — 14M01, 14M02, 15B22, 19N07, 19N08, and 14M02 Supplement 7 — acknowledging a persistent design defect. Despite this documented repeat failure history on this specific vehicle, Ford has denied goodwill assistance. I am requesting NHTSA investigate Ford's continued denial of repair assistance on vehicles with documented repeat DPS6 failures and the ongoing safety implications of this transmission's known design defects.

The RQ focused on the effectiveness of the remedies of Recall 18V-735 and 19V-515. In October 2018, Ford issued Recall 18V-735, which updated Powertrain Control Module (PCM) software in MY 2012-2018 Ford Focus vehicles equipped with 2.0L GDI engines built prior to April 13, 2017, and those with 2.0L GTDI engines built prior to February 2, 2018. The recall addressed vehicles experiencing a malfunctioning Canister Purge Valve (CPV) that can stick open and PCM software that does not adequately detect a stuck-open CPV condition. A CPV that is stuck open can cause excessive vacuum in the fuel vapor management system, potentially deforming the fuel tank, collapsing on itself. This deformation of the fuel tank can cause a Malfunction Indicator Light (MIL) to illuminate, an inaccurate or erratic fuel gauge indication, drivability issues, and/or loss of motive power. In July 2019, Ford issued Recall 19V-515, which addressed certain MY 2012 and MY 2017 Ford Focus vehicles equipped with 2.0L GDI and MY 2013-2014 Ford Focus vehicles equipped with 2.0L GTDI engines whose PCM software had been reprogrammed as a part of recall 18V-735. The updated software calibrations for these subject vehicles were not properly installed by Ford at the time of production. Approximately one year after this RQ was opened, on September 25, 2024, Ford initiated a customer satisfaction program (24N07) for MY 2012-2018 Ford Focus vehicles equipped with the 2.0L GDI and GTDI engines to—as ODI learned during this investigation—address a different issue with the CPV from the primary focus of the safety recalls (18V-735 and 19V-515). Ford stated the CPV may not operate as intended in these vehicles. A failed CPV on its own merit, may cause drivability concerns such as engine hesitation, running rough, or stalling at low speed, which will be accompanied by an illuminated MIL. This program provides a one-time replacement of the CPV for vehicles with 15 years of service or less (with no mileage restriction). This coverage will automatically transfer to subsequent owners. It is to be noted that the customer satisfaction program being taken by Ford to address this RQ investigation results in a much different and significantly less severe problem than the safety action taken to remedy 18V-735 and 19V-515. ODI found no further issues with the combined scope and severity related to 18V-735 and 19V-515 therefore, ODI is closing this Recall Query. In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property.The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. In January, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation based on consumers' allegations of doors failing to latch and/or inadvertently opening on model year (MY) 2012-2013 Ford Focus vehicles (subject vehicles).ODI has received 464 unique reports related to the alleged problem, with over 200 claiming that the door(s) opened inadvertently while the vehicle was in motion.One consumer alleged an injury that occurred while driving when a door inadvertently opened and then closed on a child's arm.Another alleged that a door that inadvertently opened struck a parked vehicle.In response to ODI's information request, Ford Motor Company (Ford) provided 500 unique reports related to the alleged problem on the subject vehicles.Ford reported no injury or accident claims.Ford also submitted 1,513 warranty claims related to door latch failures.Many customers indicated that the affected door could not be latched at all and some of them decided to restrain the door by other means in order to operate the vehicle.Some customers also reported door latch failures occurring on multiple doors and at different times.Additionally, Ford stated that MY 2014-2015 Ford Focus, 2013-2015 Ford Escape, 2013-2015 Ford C-Max, 2014-2016 Ford Transit Connect, 2015 Ford Mustang, and 2015 Lincoln MKC, all use the same door latch as the 2012-2013 Ford Focus.On September 6, 2016, Ford notified ODI that it would conduct a safety recall (16V-643) to remedy the door latches in the MY 2012-2015 Ford Focus and other Ford models mentioned above (total of approximately 2 million vehicles).According to Ford, a component within the door latch may break preventing the door from latching.Ford dealers will replace all four door latches in the recalled vehicles with a new more robust latch design that is currently in production.The recall action initiated by the manufacturer appears to address the safety risks.Accordingly, this investigation is closed.The ODI reports cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the identification (ODI) numbers found in the attached list.

It is also not recommended to order a replacement module using a vehicle identification number (VIN) from a different vehicle. Swapping a module from a vehicle or ordering a module using a different vehicle/VIN can cause ineffective repairs and additional vehicle down time. This is caused by hydrofluoric acid which is used in some car wash solutions as a cleaning agent. Hydrofluoric acid can attack the glass and cause visual distortion and/or discoloration on the outer surface which cannot be removed by further cleaning. If this type of distortion is found, inspect the area of the glass that is covered by the cowl and/or molding. The portion of glass that is covered by the cowl or molding will not be affected by the acid. Hydrofluoric acid damage is not a defect in material or workmanship and is not a warrantable condition.

Positive battery cable clamp BT4Z-14450-AA and negative battery cable clamp BT4Z-14450-BA are available for this type of repair. Do not cut or shorten the battery cable, instead carefully remove the affected battery cable clamp and clean the cable end as necessary to preserve wire length. Some rework may be necessary for some vehicle environments. Bend the new positive battery cable clamp as needed and crimp it to the existing battery cable. Make sure to use heat shrink tubing over the battery cable to battery cable clamp connection. Obtain the heat shrink tube locally.

- 10160931On vehicles equipped with an automatic transmission it is imperative that the converter pilot hub be greased with Motorcraft Multi-Purpose Grease Spray XL-5-A even if grease was not applied from the factory. Also verify that the engine/transmission alignment dowel pins are present as well as the engine crankshaft dowel pin, if equipped, when installing the transmission. Failure to adequately lubricate the converter hub greatly increases the likelihood of the flexplate cracking in the future. Missing dowels may cause a misalignment issue also resulting in a premature failure. Refer to Workshop Manual (WSM), Section 307-01.

When specified in the WSM, the turbocharger oil supply filter must be replaced whenever the turbocharger or the turbocharger oil supply tube are replaced. Turbocharger bearing failure may occur if oil starvation occurs due to a restricted filter.
- 10159489Some 2013-2019 vehicles equipped with a EVDC air conditioning (A/C) compressor may exhibit poor A/C performance/improper climate function and/or a diagnostic trouble code (DTC) P06A0 in the powertrain control module (PCM). To support the proper diagnosis of this compressor, the EVDC control valve tester has been developed and is available from Rotunda (339-EVDC100). A tester has been provided to each U.S. dealership free of charge with shipments completed 19-Apr-2019 (reference EFC07013 on FMCDealer.com). This tester will help reduce the potential for misdiagnosis and unnecessary compressor replacement. Make sure that this tester is used for warranty repairs and document the test results in the Technician Comments field. Refer to the Rotunda catalog for a video and user manual which provide detailed operating instructions.

Inspect the BCM for the presence of fuse F88. If fuse 88 is missing, install a 25 amp fuse and then re-evaluate the concern. If fuse 88 is installed in a vehicle with an auto start/stop system, the vehicle may exhibit concerns where the radio is turning off during an auto start/stop event and the instrument panel cluster (IPC) back lighting slowly goes dim during auto start/stop events.
