Following the execution of mandatory Emissions Recall 25E01 (PCM Reprogramming) by a Ford dealership, my vehicle suffered a severe network software corruption causing unintended engine acceleration and a sticking throttle. The engine holds its RPMs and continues holding until I release my foot off the gas pedal and when the car is placed in reverse the car puts/jerks. Additionally, the electronic trunk latch fires spontaneously , causing the trunk lid to fly open while in motion and/or while the car is off and locked along with other electrical issues. When the trunk opens after I’ve locked and secured my car, it has caused my car battery to drain overnight and required multiple jumps before operating again, I have since received a new battery as a resort of this electrical disturbance. The dealership has confirmed drivetrain issues but refuses an immediate remedy. This is forcing me to operate a criminally non-compliant and unsafe vehicle that I notified the dealership immediately and same day in June 2026. More problems continuously presents itself.

Sudden Loss of Power Steering Assistance and Simultaneous ABS/AdvanceTrac System Faults I am submitting this complaint to document a serious vehicle safety incident involving my 2014 Ford Fusion SE. The vehicle experienced a loss of communication with the Power Steering Control Module (PSCM), accompanied by faults affecting the ABS and traction/stability control systems. During the incident, the dashboard illuminated multiple warning lights, and the steering suddenly became extremely difficult to operate. The simultaneous loss of power steering assistance and warnings involving critical vehicle safety systems created a serious potential crash hazard, particularly at highway speeds or during an emergency maneuver. Diagnostic scanning with FORScan revealed the following communication-related diagnostic trouble codes: • U0131 — Lost Communication with Power Steering Control Module. • U0121 — Lost Communication with Anti-Lock Brake System (ABS) Module. • U0420 / U0415 — Invalid or implausible data received, as indicated by the recorded codes. I am concerned that an electrical, electronic, or component-level failure involving the power steering system may be associated with the simultaneous faults in other vehicle systems. I request that NHTSA review the incident and determine whether similar complaints involving the 2014 Ford Fusion SE indicate a broader safety concern. One concern is that the failure is intermittent. The steering and warning lights appear to recover after the vehicle is shut off, allowed to cool, or restarted. Such intermittent behavior make the problem difficult to reproduce and diagnose, even though the underlying safety concern remain.

Ford Motor Company, a massive automotive manufacturer, sold a vehicle with a known, systemic defect that can cause a driver to suddenly lose power steering at highway speeds, while simultaneously disabling critical safety systems ABS and AdvanceTrac stability control. The vehicle's dashboard lights up like a Christmas tree, steering suddenly becomes incredibly heavy, and the safety nets designed to prevent a rollover or spin-out turn off at the exact same moment. Lives are absolutely being put at risk. Because Ford has chosen to handle this through expensive, individual part replacements rather than a massive federal recall, the burden of safety has been unfairly shifted onto the consumer. When the PSCM (Power Steering Control Module) lost communication, it simultaneously disabled my ABS and Traction Control systems. This caused a sudden, unexpected loss of critical safety and handling systems while driving, creating an immediate crash hazard. I have already fixed the vehicle, so I must log the failure. Ford’s network architecture turns a single component failure into a massive, vehicle-wide electrical blackout. I've analyzed the underlying mechanics, and it became clear why this specific cycle is happening and why it causes the entire dashboard to go haywire. The Ford does not use simple independent wires for different parts; it relies on a High-Speed Controller Area Network (HS-CAN). Think of this like a high-speed internet data rail that loops through the car, where the Power Steering Control Module (PSCM), the Anti-Lock Brake System (ABS), the Powertrain Control Module (PCM), and the Body Control Module (BCM) all "talk" to each other in real time. The steering gear houses the internal Steering Angle Sensor. The ABS module needs to know my exact steering angle dynamically to calculate how to apply the brakes or handle stability control (AdvanceTrac).

On XXX, while my 2014 Ford Fusion was moving, the left-front door latch failed and the door became unsecured. As a result, I partially fell from the moving vehicle. This was an actual occupant safety event and created a significant risk of serious injury. After the incident, I took the vehicle to Richmond Ford Lincoln for inspection and repair. On XXX, the dealership performed a door-latch recall repair. The dealership service order, SO #XXX, identifies the repair request as “DOOR LATCH REPLACEMENT 20R01 - L/F DOOR WILL NOT CLOSE.” The service order identifies the cause as “recall” and documents the correction as “performed recall replaced latches.” The parts listed include a door latch and cable assembly, and the invoice total was $0.00. The dealership's documentation therefore confirms that a door-latch recall repair was required on this vehicle. The recall repair occurred eight days after the XXX incident. I am requesting that NHTSA review this incident in connection with Ford Fusion door-latch recalls and related programs and determine whether the XXX failure represents a failure of a recalled component, a prior recall remedy, or another safety defect. I request that NHTSA review the recall and campaign history associated with VIN [XXX], the specific door-latch program applicable to this vehicle, whether the failed latch had previously been repaired or replaced, and whether additional investigation or corrective action is warranted. The vehicle was repaired on XXX. I have retained the dealership documentation and can provide photographs, medical documentation, repair records, and other evidence concerning the incident. The important safety issue is that the door-latch failure occurred while the vehicle was moving and resulted in an occupant partially falling from the vehicle. I took my vehicle in to Athens ford 6/8/26 to have this fixed then on Friday my car lost power while driving, would not go l, then when I cut it off & back on it was like it went in to failsafe or limp mode. I called Athens ford 5 times on Friday & got no answer, 3 on Monday with no answer then found the number of the lady I dealt with in June. If I paid to have it towed there they could run diagnostics on it she said bc the mobile technicians couldn’t come out and do it. So I log in to my ford account and it showed this recall on the recall I had done it June. I screenshot it and sent it to her and she said it didn’t show it I needed to contact customer care. They told me the same thing that it didn’t show it. So I done a screen recording today of me logging in to my ford account and this is what it showed. Sent that to her and all they can respond is contact customer care. So I’m reaching out with this proof of what it shows and that being exactly what my car done and ford refusing to help me. I ruptured my acl and had surgery and have drs appt and physical therapy twice a week and this is the “courtesy” I get from ford Joni Spikes 7067654293 I have the screen recording but I can’t attach it for some reason

A brake flush for brake lights staying on didn’t make sense. We felt we were being upsold for something we didn’t need and they had no answere for us! We declined the second repair and picked up car on 8/07/2026 and still the brake lights are staying on while driving the car. We woke up to looking out our window and noticed lights were on . Mind you the car had not been driven for hrs and the lights turned themselves on while car was parked. Skip to Sunday driving home from in laws and dash lights up. It says hillside assist and yellow wrench pops up. I fear my daughters safety and feel it is unsafe now. Called Ford to make a complaint. But meanwhile I have a car that we need but feel unsafe to drive. Our car is having more issues than when we took it in to the dealer. I need a resolution but what I really need is my money back so that I can take the car elsewhere! I feel car was misdiagnosed and I should not have to pay for their mistake. No options were given just to pay for more repairs. We asked for past invoices to be printed so I can keep a copy. Was told they cannot print past invoices. I felt we were taken advantage of we should get our money back for a diagnosis that was not repaired.

The backup camera is faulty and will sporadically stop working. Sometimes it will stop working in the middle of backing up. When it doesn't work the screen will either go black, flicker on and off, show a reversed image, or show an image with blue lines through it. This has put my safety at risk multiple times as I've been backing up and suddenly the image will disappear. We've taken the car into an independent service shop. They were unable to solve the issue and their only solution was to replace the system for $600. There was no warning before this started happening, however it does seem to be getting worse as time goes on. It only works about 10% of the time now.

SUBJECT: URGENT COMPLAINT: Deceptive Practices/Misrepresentation & Services Not Rendered (Woodhouse Ford of Omaha) I. Actionable Safety Issue Summary This complaint concerns fraudulent billing and dangerous misrepresentation by Woodhouse Ford of Omaha, which claimed a vehicle's 3rd rack and pinion system was "safe" on 2/19/2026 after a known safety issue, following a previous failure. They further charged $219.22 for unrendered/improper ABS programing repairs that was not successful because the electric rack and pinion is defective on 3/5/2026 and fabricated invoice records to shift liability. II. Parties Involved •Consumer: [XXX] •Dealer/Service Provider: Woodhouse Ford of Omaha, Service Manager Jeremy Sullivan, Technician Anthony Rubio. •Financial Institution: First National Bank of Omaha (Regulated by OCC). III. Nature of the Dispute 1.Safety Hazard/Misrepresentation (2/19/2026): Service Manager Jeremy Sullivan and Tech Anthony Rubio stated the vehicle’s rack and pinion was "safe" despite a prior failure. This misrepresentation has left me with a vehicle that has had the rack and pinion fail or require replacement three times in under a year. 2.Services Not Rendered (3/5/2026): Charged $219.22 for "programming the ABS module" which was not rendered or not performed correctly. 3.Fabricated Documentation: Woodhouse Ford created fraudulent service invoices stating I diagnosed the ABS fault and declined further diagnosis. This is an attempt to evade liability for ongoing, dangerous defects. IV. Regulatory Violations & Requested Action The described actions violate consumer protection laws, including: •Dodd-Frank UDAAP (12 USC § 5536): Unfair, Deceptive, or Abusive Acts or Practices. •Fair Credit Billing Act (15 USC § 1666): Billing error/services not received. •Regulation E (12 CFR 1005.11): Procedures for resolving errors. Requested Action: Initiate an investigation into Woodhouse Ford of Omaha’s service practices and safety diagnostics. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA),

On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash. On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. No failures of brake hoses on hybrid vehicles have been reported to ODI or Ford.Ford's actions address ODIs concerns at this time. Hybrid vehicles will be monitored for related failures. The agency reserves the right to take further action if warranted by new circumstances. The 103 ODI numbers cited above can be viewed at NHTSA.gov under the following ID numbers:11511014, 11510867, 11510318, 11509030, 11505993, 11504989, 11504400, 11502147, 11502082, 11501820, 11501283, 11498439, 11498296, 11496842, 11496335, 11494931, 11494851, 11494252, 11493001, 11492084, 11492070, 11491444, 11491274, 11488858, 11488027, 11487627, 11487416, 11487233, 11486150, 11484924, 11484526, 11484080, 11483948, 11483926, 11483739, 11483723, 11482773, 11482690, 11481077, 11480788, 11480294, 11480052, 11479338, 11479334, 11479252, 11479247, 11479187, 11479094, 11479048, 11478985, 11478953, 11478729, 11478216, 11476892, 11476282, 11470558, 11468994, 11467109, 11466294, 11462746, 11461217, 11456065, 11455367, 11444848, 11436419, 11436376, 11430685, 11428329, 11425083, 11421026, 11420738, 11420735, 11420298, 11418197, 11403926, 11397549, 11366991, 11366757, 11361016, 11359968, 11354569, 11352165, 11350344, 11348799, 11343929, 11343205, 11331947, 11329020, 11319596, 11310529, 11292271, 11288455, 11254787, 11253479, 11253244, 11234622, 11230669, 11230050, 11228240, 11221111, 11143630, 11129392 and 11120228.
- Desiccated Air Bag Inflator Rupture
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
- Door Latch Failures
In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs…
Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below. On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge. On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property. The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. ODI will take further action if warranted by new information. The reference (OD

On March 12th, 2018, Ford Motor Company submitted a Defect Information Report to NHTSA describing a defect in the steering wheel fastening system of approximately 1,301,986 model year (MY) Ford Fusion and Lincoln MKZ. On March 1, 2018, Ford discovered a design change that affected the MY 2014-2018 subject vehicles. The affected population are MY 2014-2016 Fusion and MKZ with manual and power adjust steering columns and MY 2017-2018 Fusion and MKZ equipped with manual adjust steering columns only. The affected populations have steering shafts with a reduced amount of threads for the steering wheel bolt to engage. The thread length was decreased by 5mm in the affected models. Ford stated that another contributing factor was the placement of a nylon locking patch torque retention on the fastening bolt.As a result, the steering wheel bolt may not maintain torque allowing the bolt to loosen and eventually separate from the steering shaft, resulting in a loose or detached steering wheel. The two crashes and one injury noted in Ford?s Defect Information Report are from two legal claims. The first accident is a claim where the driver reported the steering wheel twisted freely on the shaft which caused the driver to lose control. An injury was alleged but no details were provided. The second claim resulted in minor damage to the driver side of the vehicle including a punctured tire. No injury was reported.Ford?s recall remedy procedure will use a bolt that is 17mm longer, providing a more robust steering column thread and torque retention patch engagement. The torque retention feature will be 13mm longer to ensure proper engagement. This investigation is closed based on Ford's recall.

- 10235153For 2000-2024 Ford and Lincoln vehicles, swapping a module from a vehicle for diagnosis purposes is likely to cause errors and is not recommended. It is also not recommended to order a replacement module using a vehicle identification number (VIN) from a different vehicle. Most modules on these affected vehicles are VIN/vehicle specific and hardware variations between modules do exist. Swapping a module from a vehicle or ordering a module using a different vehicle/VIN can cause ineffective repairs and additional vehicle down time. Make sure all appropriate Workshop Manual (WSM) procedures are followed when diagnosing the condition prior to all module replacements and only order modules using the correct VIN.
