The contact owns a 2012 Lexus IS250. The contact stated that while driving at various speeds, the brake inadvertently activated without the brake pedal being depressed. The vehicle also experienced unintended acceleration without the accelerator pedal being depressed. The cause of the failure was not determined. The contact indicated that the failure occurred after the cell phone was hacked and was concern the vehicle security was also compromised and hacked. The manufacturer was notified of the failure. The local dealer was not yet contacted. The failure mileage was 137,000.
Takatabrantakata recall my vehicle was rear ended at 34 to 50 miles an hour. The vehicle behind me impacted me so hard that it pushed me into the vehicle in front of me and in turn she hit the vehicle in front of her. There was damaged to the front and side panels of my car as well. The air bags failed to deploy. I sustained and suffered injuries. This occurred on a busy city street while at a traffic light.
Door locks not working properly. both front door actuators need to be replaced which we were told IS a very common problem with this model. this has been unsafe for our teenage driver having to crawl through back doors to unlock her vehicle especially at night.
Dashboard coming apart clue seeping they dashboard sticky
Unintended acceleration while inching into parking space at very slow rate of speed. Engine raced unexpectedly and car jumped curb with all 4 tires, across a small median, and down another curb with 3 tires, striking a parked car, which stopped forward motion.
This IS the third lexus I have had that the automatic door locks don't work. All 4 actuators are bad after I even had one replaced. I have had this happen I all three lexus' that I have owned. I have talked to lexus about this and the service people say that they see more than they should go. Bad boy. Lexus/toyota does not think it's a problem. this becomes a safety hazard locking the doors manually and having to open one door with a key and reach around and open the others as needed. Specially with all the crime around us. I would like to see either toyota or lexus step up and admit they have a problem and fix them.

Parts distribution disconnect.
Takata recall- received a recall in march 2017 - still waiting for a fix (8/14/2017). Unable to drive for fear of injuries.
TL* takata recall. The contact owns a 2012 lexus IS 250. The contact received notification of nhtsa campaign number: 17v006000 (air bags); however, the part needed for the repair was not available. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The contact called an unknown lexus dealer on grant mill rd in birmingham, al who stated that the remedy part was not available for the repair. The manufacturer was not contacted or made aware of the issue. The contact had not experienced a failure. Vin tool confirms parts not available.
Takata recall. I received notification five months ago about this recall.i contacted the dealer ship about one month ago they told me they didn't have the part yet. I called the dealership today and they told me they wont have the part for at least two more months. In the meantime I am concerned in using my car with this problem. Sincerely [xxx] information redacted pursuant to the freedom of information act (foia), 5 u.s.c. 552(B)(6). *tr

Takata recall: I received a recall letter from lexus on march 7 for the airbag recall. no fix was provided and when I called they did not know if it would be 1 month or 6 months until a fix was available. their solution was " do not drive the car with a passenger in the front seat". this IS a stupid and unacceptable response. this takata airbag recall has been out for several years and someone in toyota/lexus has know about it? someone has been covering up the mess at lexus? this borders on fraud and as a minimum someone high in their organization has lied to keep this under cover for this long. Not driving the vehicle IS not an option. I have purchase 3 lexus in the past but this was my last.

TL* the contact owns a 2012 lexus IS 250. While driving approximately 2 mph, another vehicle ran the traffic light and crashed into the front end of the contact's vehicle. The air bags did not deploy. The vehicle was towed. A police report was filed. Injuries were sustained and medical attention was required. The damage to the vehicle had not been determined. The vin was unknown. The approximate failure mileage was 48,000.
I want to report that I experienced un-intended acceleration on my lexus IS 250C, 2012 model purchased in december 2011 as a result of which my vehicle crashed and went turtle.
TL* the contact owns a 2012 lexus IS 250. The contact stated when attempting to shift into reverse and engage the brakes, the vehicle failed to stop and the contact had to shift into park. The failure occurred more than once. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The approximate failure mileage was 17,000. the consumer stated when she stepped on the brake to start the vehicle and put the gear in reverse, the vehicle lurched backwards as she lifted her foot off the brake. She was unable to stop the vehicle with the brake. She threw the vehicle into park and turned it off. the vehicle was towed to the dealer, where they kept it overnight and ran performance checks. They did not find anything wrong and said it could have been the weather mat that was on top of the lexus carpet mat. the consumer paid for the repairs, but did not state what they were. The consumer stated it was the second time, she experienced sudden acceleration. Updated 09/17/15*jb
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
