Tire description: firestone wilderness at, size p235/75r15, produced at the decatur, illinois tire plant and all firestone radial atx and radial atx II tires, size p235/75r15. operation of these tires at low inflation pressures, high speed, and in hot weather, can contribute to separation of the tire tread.
If the tread separates from the tire, the driver can lose control of the vehicle, possibly resulting in a crash causing injury or death.
The replacement/reimbursement program for this campaign expired on august 29, 2001. however, customers should contact firestone customer service at 1-800-465-1904 for possible assistance.
Equipment description: aftermarket brake rotors for use on certain ford and mazda trucks. cracked castings caused cracks to appear on the outer diameter of the rotor hat or around the stud hole.
Loss of braking can result.
Aimco will reimburse consumers/installers for the cost of replacement rotors and installation. consumers are requested to return to the location where the rotors were originally purchased/installed for replacement.
The studs which attach the brake master cylinder to the power brake vacuum booster assembly can develop stress corrosion cracking after being operated for an extended period of time in the southern california location.
If a crack occurs, it could result in one or both studs fracturing. The master cylinder could then separate from the vacuum booster, when the brakes are applied, preventing the service brakes from fully activating increasing the risk of a vehicle accident.
Dealers will replace the power brake vacuum booster assembly.
During extreme cold, northern climate winter conditions, ice can form in the throttle body of the engine causing the throttle plate to remain in the highway cruising position after the accelerator IS released or the speed control IS deactivated.
The vehicle stopping distances can be increased possibly resulting in an accident.
Dealers will replace the air intake duct and valve assembly with revised units.
The parking brake control self-adjust pawl does not line up properly with the self-adjust rachet causing the pawl to slip over one or more teeth in the rachet.
When the above occurs, it can result in parking brake system ineffectiveness. The parking brake will not hold the vehicle, causing the vehicle to roll freely, if the transmission IS not left in gear.
A wedge will be installed which will serve to lock the parking brake self-adjust mechanism pawl into engagement with the rachet. Also, owners will be advised to release and reapply the parking brake whenever tooth skip occurs and will be reminded to always turn off the engine and leave the vehicle in gear when the vehicle IS parked.
The front fuel lines have a section of flexible hose that IS susceptible to cracking which can allow fuel leakage.
Leaking fuel can result in a fire when exposed to a source of ignition.
Dealers will inspect and, if necessary, replace the front fuel line assembly.
TL* the contact owns a 1994 mazda b-series truck. While the vehicle was parked, it independently accelerated, crashed into a fence, and drove over the owner of the vehicle. The owner sustained fatal injuries. The vehicle was towed to an impound lot where the failure recurred, almost striking two police officers. A police report was filed. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The failure mileage was not available.
Tires purchased from sam's club in 12/09 have cracks between treads. I just noticed this and was never told when I had tire balanced and rotated. Its on all four tires and read it may separate. I had the road hazard and of course its an on going issue that sam's played it smart and only warrant road hazard for 4 years after I question them about concern.i have bf goodrich tires 225/70/14 on a mazda b4000 LE . I have goodrich tires on two other vehicles that I need to inspect. *tr
Approximately 4-5 months ago I began to experience engine surging while driving in town. unable to find any vacuum leaks, I inspected the fuel filler neck to find it badly deteriorated in the area just above the gas tank. I replaced it with a new oem quality neck. at the same time I replaced the throttle position sensor, the coolant temperature sensor, rebuilt the fuel injectors and replaced the intake manifold gasket. Afterward, I did not experience any surging until a few weeks ago when, approaching a stop sign, the engine began racing so badly that I quickly shifted into neutral and steered to the shoulder of the road, narrowly avoiding a rear end collision. since then I have driven the truck about 100 miles without incident. the truck has been driven 90,000 miles and well maintained. it may not be relevant to this problem, but this engine was built without an egr valve and egr vacuum solenoid. *tr
TL* the contact owns a 1994 mazda b4000. The contact stated that when refueling, fuel would spill back out from the filler neck. She also noticed a fuel leak when the vehicle was in motion. The vehicle was taken to an independent mechanic and they advised the contact that the fuel filler hose was fractured and needed to be replaced. The vehicle was repaired. The manufacturer was contacted and they offered no assistance. The failure mileage was approximately 202,000. The vin was unavailable.
TL* the contact owns a 1994 mazda b3000. The contact stated that he noticed fuel leaking from the vehicle. The contact inspected and noticed a fracture in the fuel line hose. The contact called both the dealer and the manufacturer and was told by both parties that the vehicle was not included in nhtsa campaign ID number: 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings). The vehicle was not repaired. The current and failure mileage was approximately 189,000.
TL* the contact owns a 1994 mazda b-3000 pick up. The fuel line has developed a crack. consequently during fueling gasoline leaks from the fuel system. fuel leakage in the presence of an ignition source can cause a fire. The current mileage was 323,000 miles.
Parking brake non-functional. fuel leak, not able to locate. Repair shop claims probable no under warranty. brake vacuum booster IS failing. Rotors have been turned, pads replaced. drivers seat back broken. Repaired. Broken again. *Tr
The filler tube going to the gas tank ruptured. When I filled the tank, gas spilled on the ground. Fumes are always around the truck. I paid to have this tube replaced. The truck had 53 k miles. Now it has done the same thing at 74 k miles. I think that this IS a very dangerous condition and IS costly to keep repairing. *Tr
Tl*the contact owns a 1994 mazda B400. the contact stated that fuel leaked from the vehicle and onto the ground while he was pumping fuel into the tank. his vehicle IS not included in recall # 94v111002 (fuel system, gasoline:delivery:hoses, lines/piping, and fittings). the current mileage IS 98,000 and failure mileage was 92,000.
TL* - the contact stated that the front fuel line in his 1994 mazda b3000 was cracked and leaking fuel. he call mazda and gave them his vin but they stated that his vin does not fall under the recall. the contact would like the recall extended to include his vehicle. *nm
On long trips, often in cold, wet or snowy weather, the engine will occasionally rev out of control. this IS not an issue of the throttle cable icing up because turning the engine off and restarting it (which IS the last thing you want to do while still moving on icy roads) temporarily resolves the issue. therefore, it seems to be most likely an electrical problem. no diagnostic trouble codes (dtcs) are registered in the computer's memory, therefore no information exists by which to diagnose the problem. I have been to dealers and they have said they cannot help me unless they can replicate the problem in the shop. as I said, it has only happened after many hours of driving on long trips, which makes it impossible to replicate in the shop. *nm
Dt: the contact stated the fuel filler hose cracked which resulted in a leak. when attempting to fill the fuel tank there was a large amount of gasoline that leaked around the filler neck and formed a large puddle underneath the vehicle. the filler hose was previously replaced by an independent repair shop, but the hose cracked again. no further repairs have been made at this time. *ak updated 12/27/2005 - *nm
My 1994 mazda b3000 pickup (i.e. Ford ranger) leaks gasoline when being filled at the filling station due to a faulty fuel tank filler neck. fuel spills on the ground of filling station (approx 1/2 gal) and IS a fire hazard. cost to replace fuel filler neck IS $280 from ford dealership, not including labor - this IS prohibitively expensive for a rubber hose. many other ranger owners experience this same problem, thus indicating that there's a defect in the design/construction of the filler neck. *nm
Dt: the gas tank filler hose on the mazda pick up truck IS faulty and IS leaking gasoline when the tank IS full, and fumes when the tank IS half full or less. In the consumer's research he found that this IS a common problem. Madza IS charging $400.00 for the replacement part. consumer has been in contact with mazda, with no help. consumer has placed on order for the replacement part. Consumer had remedy for this issue and it 4 years. However, problem recurred. *ak
Gas spill of approx. 1/4 galllon at end of every fill up.
While the vehicle was parked consumer noticed fluid leaking from underneath the vehicle. it was taken to the dealer for inspection, and mechanic determined that the front fuel line assembly needed to be replaced. *ak
I have a 1994 mazda b3000 with 75000 miles and after ten years the tank has developed a crack in an area that IS protected from road debris and has no strees or flex . I beleive that this tank while it IS ten years old IS defective and should be replaced by the manufacturer. The result could be fire from the leaking fuel . The crack IS not at a seam . The vehicle IS not used in rough terrain causing stress. *jb
Request for assistance-check attached paid bills to determine if any repairs qualify for recall. *bf the drivability problem was constant and was present during normal road conditions and the egr was found rusted and allowed incorrect egr flow. *sc
During a trip from home to store, the dash lights for the ABS and brake system came on without any prior warning. Another driver made a u-turn in front of me prior to my arrival at the store causing me to have to apply the brakes in an evasive manuever. The brakes l,ocked up and caused me to lose control of the vehicle. Fortunately, I was able to avoid the collision. I took the vehicle to a local repair shop and they informed me that the ABS hydraulic control valve needed to be replace. They indicated that there had been 3 revisions of the original ABS system valve, indicating that mazda was aware that there had been a problem with this valve. We have never received any indication of a recall or problem.*ak
The paint on the hood and roof has bubbled and flaked off. I purchased the truck with 8,000 miles on it. I consistently washed and waxed the truck.(waxed 5-7 times yearly). The body shop I took the truck to said this was poor quality work and they were suprised to here this was from the factory and not repair work. *la
I have replaced the fuel assembly twice on my 1994 mazda b-3000 due to leaking fuel and paid $400.00 each occurence since I purchased the truck from the dealer. I took note of recall number 94v111002 and mazda states my vin number does not match. My truck specifications match as much as I can tell and I would appreciate a detailed explanation. Thank you. *Ak
When starting the vehicle brake pedal went to the floor. dealership was notified, but did not resolve the problem. *ak
My truck a 1994 mazda b-4000 4X4 lost brakes at 40 to 45 mph. I eneded up ditching the truck to avoid hitting a schoolbus that was letting off childeren a stop. my mechanic told me that this IS a common problem with ford and mazda. the problem was where the rubber brake line and steal brakeline come together blew apart. The result IS a complete loss of brakes
While driving 65 mph the gas pedal descended to the floorboard and the vehicle reached a speed of 90 mph. The consumer tired to gain control of the truck by applying the brakes but they failed. the consume had to turn the engine off to coast to the side of the road. Both the manufacturer and the dealership were contacted. Please provide any additional information. *nm
While traveling about 60mph on highway and without prior warning vehicle will not shift into gear. there was a loud banging noise. *Ak
