Fuel pump control module failed at under 60k miles. No parts in stock and was told by dealer, no parts anytime soon. This is unacceptable, especially since this is a known problem. A rental car should be provided as expected wait time is now going on two weeks.
Driving on the highway when all lights lit up on the dashboard. Car slowed down immediately following alert notifications. Towed to dealer. Was told I needed Camshaft and both banks of lifters all head bolts and head gaskets (W) [Other]: Camshaft actuator and bolt and chain tensioner
While driving down interstate at approx. 55-65mph, vehicle "sputtered" and shut off while driving. Vehicle would crank but not start. After sitting approx. 10 mins, was able to restart and drive approx. one mile or less before "sputter" and vehicle died, followed by another crank no start. No check engine lights came on. Vehicle was in roadway in very dangerous location with blind curve requiring Police to be called in fear of my safety, since vehicle was in roadway, I was afraid for my children's safety as there were in car seats in the vehicle at time of failure. I was also concerned about my safety as well as other motorists safety. I was scared that I was going to get hit by another vehicle since my vehicle died in the roadway in a curve. There was an increased probability of injury or death from the faulty part on this vehicle. Police were on scene to assist with traffic, however no report was made, vehicle was disabled. Vehicle had to be towed to residence. CEL came on after approx 3rd start attempt. Believe issue to be Electronic Fuel Control Module which is common with this vehicle year/make/model. However in contacting the dealer in which I purchased Tahoe, they state that this particular VIN has no open recalls, however it matches description on recall N252516900 Fuel Pump Power Control Module. Vehicle is not yet repaired, it has not been inspected, and faulty part is still on the vehicle, vehicle and part are available for inspection.
The contact owns a 2021 Chevrolet Tahoe. While the contact’s wife was driving 55 MPH, the vehicle unexpectedly started losing automotive power before the vehicle stalled. The driver was able to safely guide the vehicle to the shoulder of the highway. The vehicle was towed to an independent mechanic, where it was diagnosed with a fuel pump power control module failure and the vehicle was repaired. The contact associated the failure with NHTSA Campaign Number: 21V739000 (Fuel System, Gasoline); however, the VIN was not included in the recall. The dealer was contacted and stated that there was no recall associated with the VIN. The manufacturer was contacted by phone and made aware of the failure. The failure mileage was approximately 96,202.
While traveling at highway speeds, the vehicle experienced an instantaneous, simultaneous, and total failure of both the electronic eBoost braking system and the electronic power steering assist. The dashboard flashed multiple critical system malfunction notifications with zero prior warning. When attempting to slow down, the brake pedal sank completely to the floor with zero mechanical resistance and provided absolutely no braking response. Primary control of the heavy SUV was lost, forcing a dangerous maneuver across three highway lanes to avoid a high-speed collision. The vehicle is currently at a dealership due to a Fuel Pump Power Control Module Recall. Because this module interfaces with the high-speed electronic data network (CAN bus), its failure mode appears linked directly to the catastrophic communication drop that disabled the eBoost brakes and steering. This failure aligns with the mechanical faults documented under federal safety recalls 21V-455 and 21V-579, as well as the active NHTSA Engineering Analysis EA26006 investigating GM eBoost system failures. The defect completely stripped the vehicle of its primary safety functions without warning.
There are thousands of people across social media and internet websites with the same exact problem they are having with 2021 chevy tahoe. There has also been multiple recalls for the same problem but seems it was not issued for all the vehicles that has this default. The Brake service light first came on the dash along with service esc, max speed limited to 43, service brake monitor system. It was taken to multiple shops for a diagnostic including Jim Hudson Chevy and all of them said how this is an ongoing issue with 2021 tahoes. No one knows the exact problem or cause. The dealership said it’s probably the brake control module, because that’s what the issue was all of the other times they had this occur.
Fpcm went out about 2 hours from home vehicle only has a little over 12000 miles there's a recall for this problem just not under our vin how does a part go bad that is recalled the same year but isn't covered
Vehicle is a 2021 Chevrolet Tahoe Premier with under 50,000 miles. The vehicle experienced a sudden and complete loss of reverse gear, accompanied by violent lurching, sputtering, and dangerous hesitation/stuttering while accelerating between gears at speed. A certified dealership inspection physically diagnosed the vehicle with multiple stuck valves inside the transmission control valve body, requiring a complete valve body replacement. This exact mechanical failure completely mirrors the known safety defects outlined in GM Special Coverage Program N242454441, which details transmission pressure losses causing gear engagement conflicts and potential rear-wheel lock-up hazards. The replacement transmission component installed by the certified dealer failed catastrophically within four miles of pickup, again causing severe transmission sputtering and shifting failures that render the vehicle entirely unroadworthy and a safety hazard on public roads. The dealership has been forced to order an entirely new replacement valve body assembly under warranty due to a consecutive failure. General Motors Corporate Customer Assistance acknowledged that the vehicle is physically experiencing this transmission valve defect, but refused a voluntary warranty override solely because the vehicle's automated database VIN block falls just outside their pre-programmed list for Special Coverage N242454441. The corporate case representative explicitly instructed the consumer to file this federal safety report with the NHTSA due to the systemic component failure.
My car on the dash board said do not accelerate and was shaking and the gear would not go into reverse. When I tried the car would not move. I towed the car to local Chevy dealer and when I told them about N242454441 or related safety campaigns like N242454440 nothing came up for my VIN but was informed GM had this program specifically for my code ( check engine light on) reading P0776 and P0700. This was not covered.
The contact owns a 2021 Chevrolet Tahoe. The contact stated that while driving approximately 40 MPH, the vehicle lost automotive power, stalled, and failed to return to normal operation. The contact researched and was made aware of NHTSA Campaign Number: 21V739000 (Fuel System, Gasoline); however, the VIN was not included. The vehicle was towed back to the residence. A dealer was contacted by phone and made aware of the failure. The manufacturer was notified of the failure by phone, and the manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 50,000.
I am writing to report a serious safety defect regarding the Driver's Side LED headlight assembly on my 2021 Chevrolet Tahoe (VIN: [XXX]. While driving, the headlight suddenly dropped completely out of alignment and is now pointing directly at the ground, casting light only a few feet in front of the bumper. Upon inspection, it was determined that the internal plastic headlight adjuster mechanism completely snapped/failed. This failure has created an immediate and severe night-driving hazard. Because the LED headlight now illuminates only the ground immediately in front of the vehicle, forward visibility on the driver side is effectively reduced to zero at highway speeds. This drastically reduces my reaction time to avoid pedestrians, debris, or oncoming obstacles, making the vehicle unsafe to operate after dark. The internal plastic adjuster mechanism cannot be purchased or repaired separately. General Motors requires the replacement of the entire sealed headlight assembly, which costs upwards of $1,500. This is not a wear-and-tear item; it is a premature structural failure of a critical safety component. I request that this failure be investigated as a safety defect to protect other motorists from sudden loss of nighttime visibility. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
The vehicle suffered complete internal engine failure due to a snapped timing chain on startup. The vehicle turned over via the starter motor but failed to start. A professional diagnosis confirmed the broken timing chain caused extensive valve-to-piston damage, requiring a full engine replacement. This failure appears to be a direct consequence of the known 6.2L L87 V8 engine crankshaft and connecting rod manufacturing defects. Severe mechanical resistance or violent rotational harmonics at the crankshaft level place un-engineered stress on the timing drive system, causing the chain to snap under startup torque. All broken components are preserved and available for inspection. The defect creates a severe risk of sudden, total loss of propulsion while driving. Had the compromised timing chain snapped at highway speeds rather than on startup, it would cause immediate engine seizure, a sudden loss of power steering, and a loss of power brake vacuum assist. This leaves the driver unable to accelerate or safely steer out of active traffic, creating an extreme risk of a high-speed crash. The broken timing chain and total engine destruction were officially diagnosed and confirmed by professional automotive repair technicians. The damaged components have been inspected by independent repair representatives. The vehicle has not yet been inspected by General Motors because this specific VIN is excluded from the current recall scope. There were no warning lamps, check engine lights, or diagnostic trouble codes (DTCs) prior to parking the vehicle, and no outward symptoms of severe engine stress were present until the complete no-start failure.
The contact owns a 2021 Chevrolet Tahoe. The contact stated that while driving at various speeds uphill, the vehicle made a loud clunking sound while downshifting. While driving 70 MPH on one occasion, and while driving 20 MPH on another occasion, the vehicle made a loud clunking sound while downshifting and lost motive power, prompting the driver to pull over to the side of the road. The message "Reduced Acceleration - Drive with Care" was displayed, and the check engine warning light illuminated following the failure. The vehicle was restarted but the warning light remained illuminated. The vehicle was taken to a local dealer, where it was diagnosed and determined that the transmission control valve body and a separator plate or spacer needed to be replaced. The contact associated the failure with TSB: 22-NA-258; however, the VIN was not included. The vehicle was being repaired. The manufacturer was notified of the failure by phone and offered partial financial assistance. The failure mileage was approximately 76,000.
On [XXX], while driving near Mammoth Mountain, California (Highway [XXX]), the vehicle suffered a sudden, severe engine malfunction. The engine began misfiring violently, accompanied by a loud, mechanical clacking noise, and experienced an immediate loss of propulsion power. This mechanical failure left my family—including young children—completely stranded on the side of a mountain highway in a secluded wilderness area with dropping temperatures (39°F) and active local wildlife Because the vehicle could not safely maintain highway speeds or generate sufficient power, it created an immediate and severe traffic safety hazard, forcing us to secure emergency transport for the vehicle. Which took 4hours for someone to come out to the secluded hwy on a holiday weekend. Dealership Diagnostic Findings A formal inspection conducted by a certified General Motors dealership (Michael Hohl Motor Company) confirmed the following: Symptoms: Gas engine diagnostic confirmed "Misfire, and clacking noises coming from passenger side of the motor." Diagnosis: Cylinder Head / Valve / Lifter / Lash Adjuster failure. The dealership documented a collapsed lifter and recommended a complete engine tear-down to inspect lifters and pushrods for wear or bending. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
On September 19, 2026, while traveling approximately 30 mph, the vehicle experienced a sudden, hard shift. The check-engine light then illuminated. I pulled over as soon as it was safe to do so. After stopping, the vehicle would not shift into reverse and entered limp mode. The transmission malfunction created a safety risk because the vehicle’s operation and available gears became restricted unexpectedly while it was being driven. The loss of reverse also prevented the vehicle from being maneuvered normally. The vehicle was not driven after the incident and was towed from the location. The vehicle and affected components are available for inspection upon request. At this time, the vehicle is awaiting inspection and diagnosis to determine the exact cause of the transmission malfunction.
2021 Chevrolet Tahoe – Unexpected Shutdown/Restart While Stopped On two separate occasions, my 2021 Chevrolet Tahoe has unexpectedly appeared to shut down while I was stopped with my foot on the brake. The vehicle is equipped with Auto Stop/Start, and initially the event seems like a normal Auto Stop. However, instead of a normal engine Auto Stop/Start cycle, the vehicle appears to shut down or reset more completely. The event is jarring, and the power door locks lock/unlock as the vehicle shuts down and restarts. The vehicle subsequently restarts and I have been able to continue driving. This has now occurred twice while stopped. I am concerned that the vehicle may be experiencing an unintended stall, loss of electrical power, or another malfunction rather than normal Auto Stop/Start operation. I checked my VIN through NHTSA and it does not currently show an open recall. I am aware that certain 2021 Chevrolet Tahoes have previously been recalled for a condition that could result in an engine stall, although my VIN does not appear to be included in that recall population.
The number 7 DFM lifter failed and is suspected of failing due to the Active Fuel System. The vehicle is currently at the dealership where it was diagnosed and is awaiting replacement parts that are on GMC back order for a month or longer. The vehicle started shaking on 8/25/26 when idle and the following warning lights were visible: AVS, traction control, and ECU.
I received an automated message from the vehicle stating to service the safety restraint system. I took the car to the local dealer and they ran a diagnostic review. There is a TSB for the vehicle and they say that the problem is coming from the overhead console part number 86575239. This part is not available anywhere. I contacted General Motors and requested assistance. All they have done is escalate it to the parts delay team. When I called today, all they say is they do not have any information available. They can’t even say when the part will be in stock. The dealership was unable to say that the airbag system would work as required or not. The car is unusable due to the passenger air bag system fault.
DFM (Dynamic Fuel Management) system failure causing collapsed lifters, oil consumption, and complete engine failure. To repair our vehicle it will require a complete engine replacement. This has been a known issue with GM and yet there has been no recall to fix the issue. Now our engine must be replaced.
Headlight adjustment broke and headlights point straight down. Can not see anything when driving at night.
Total engine failure. No warning lights shown and vehicle had been checked numerous times and reported as just fine.

To date, ODI has received 499 complaints alleging post-remedy engine failure in vehicles subject to Recall 25V-274, 473 of which involved an oil viscosity change and 26 a complete engine replacement. GM reported receiving 6,953 complaints of post-remedy engine failure. ODI will close this RQ and continue to investigate complaints of post-remedy engine failure for Recall 25V-274 as part of an Engineering Analysis (EA26005). The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001). GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274. This presents a potential safety risk that warrants further investigation. ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues of vehicles built outside the recall scope. On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024. GM cited the root cause as a supplier manufacturing and quality issues. GM’s recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures which fall outside of the scope of recall 25V-274. Based on GM’s recall, ODI will close this PE. NHTSA will continue to investigate complaints of engine failure outside the scope of recall 25V-274 as part ofEngineering Analysis (EA25007). On April 9, 2021, the Office of Defects Investigations (ODI) opened PE21-010 to investigate instances of air bag malfunction indicator light (MIL) illumination and potential for frontal air bag non-deployment in several vehicles manufactured by General Motors (GM). The vehicles included model year (MY) 2020 through 2021 CT4 and CT5 passenger cars, Sierra and Silverado trucks (1500/2500/3500 models), XT4 and MY 2021 Escalade, Suburban, Tahoe, Yukon SUVs.Technical Service Bulletin (TSB) 21-NA-005, issued in March 2021, addresses air bag MIL illumination with diagnostic trouble codes (DTCs) B0001-1B or B0012-0D. It cites rust particles in the connection terminal interface of the driver air bag inflator housing as the cause of the air bag MIL illumination. Illumination of the air bag MIL under these circumstances may result in a non-deployment of the frontal driver air bag during a frontal collision and increased risk of injury to the driver.GM stated a correlation exists between production delays due to the pandemic shutdown and premature corrosion initiation on the surface of the air bag inflator housing, which, in some cases, caused rust development during a seven (7) month period of vehicle production. In the rare instance when a rust particle dislodges from the driver air bag inflator housing and enters the air bag connector prior to assembly, an intermittent resistance spike at the air bag connector may result. The condition generates excessive resistance in the air bag electrical system when the steering wheel horn cover is manipulated or the steering wheel is turned. This excessive resistance is momentary. A crash commanding air bag deployment would need to occur exactly when the momentary excessive resistance occurs. Thus, there is a very low likelihood the hazard will present a risk during a crash of sufficient severity to warrant a commanded deployment. This failure mechanism occurs early in life and results in an air bag MIL illumination that corresponds to a DTC warning the driver that the vehicle requires service. The vehicle's air bag remains functional while the MIL is illuminated, and the DTC is present.Once the connection of the driver air bag module to the clockspring is made at the plant, or at the dealership for service, there is no longer an opportunity for rust migration into the connection point. The connector is sealed and has gold terminals, eliminating the expectation of fretting corrosion. All subject vehicles passed standard corrosion validation testing for extensive corrosion with no issue of air bag non-deployment.There is a low rate of occurrence: 1.41 incidents per thousand vehicles (IPTV) for all related occurrences, and 0.04 IPTV for occurrences with a mention of visible rust and/or corrosion on the air bag inflator housing. The average occurrence mileage for the subject vehicles is approximately 2,100. Every occurrence related to the alleged defect experienced an air bag MIL illumination and there are no verified allegations of air bag non-deployment within all data sources. To date, there have been no reported fires, crashes, injuries, or fatalities related to this issue. In view of the low rate, and the nature of early-life failure, the high detectability of failures, and low potential hazard to drivers, this Preliminary Evaluation (PE) is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take additional action if warranted by new circumstances.Review the ODI reports cited above at nhtsa.gov under the following identification numbers: 11537927, 11528888, 11525020, 11521401, 11506057, 11493107, 11486767, 11441525, 11433146, 11425658, 11423440, 11385267, 11376429, 11373466, 11366995.
