Toyota IS recalling certain model year 2004-2010 passenger vehicles. the accelerator pedal can get stuck in the wide open position due to its being trapped by an unsecured or incompatible driver's floor mat.
A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.
Toyota filed an amended defect report on november 25, 2009, stating that dealers will modify the accelerator pedal and, on certain vehicles, alter the shape of the floor surface under the pedal. these changes address the risk of pedal entrapment due to interference with the floor mat. redesigned accelerator pedals will become available beginning in april 2010 and dealers will replace any modified pedal with the new pedal if desired. also, dealers will replace any genuine toyota or lexus all-weather floor mats with redesigned all-weather mats, or repurchase the previous mats from owners who do not want the new ones. additionally, software modifications will be installed on camry, avalon and lexus ES 350, IS 350 and IS 250 models that will ensure that the brake overrides the accelerator in the event both brake and accelerator pedals are applied. toyota will begin mailing letters to owners on december 31, 2009. owners may contact toyota at 1-800-331-4331, lexus at 1-800-255-3987.
Certain all weather floor mats (awfm) specifically sold as an optional accessory for use on the 2007 and 2008 model year lexus es350 and toyota camry vehicles. the awfm includes two grommet holes and retaining hooks (clips) to secure the mat to the vehicle's carpet. if the awfm IS not secured by itself or if it IS placed on top of an existing carpeting…
The accelerator pedal may temporarily become stuck in a partially depressed position when returning to the idle position. if this condition occurs, it may increase the possibility of a crash.
Toyota will notify owners of all 2007 and 2008 lexus es350 and toyota camry vehicles about the recall even though not all of these owners will have purchased the all weather mats. if the owner had not purchased the mats, they are not affected by the recall. this letter will advise owners when replacement mats will be available or how to return the all weather floor mat set to obtain a refund. a second letter will be sent after mats are available requesting owners to return their mats to a lexus or toyota dealer for exchange of the driver's side mat. the first letter IS expected to be sent during early october 2007 and the second in late november 2007. owners can contact the lexus customer assistance center at 1-800-255-3987 or the toyota customer experience center at 1-888-270-9371.

Sticky dashboard cannot be repaired due to manufacturer defect
Dashboard melts and becomes stick and shiny, causing excessive glare and significantly reducing the drivers ability to see out of windshield. this IS a well documented manufacturing defect. contacting lexus/toyota about this and they refused to provide any assistance to rectify this problem. the problem IS not bad in the winter time. But with ambient temperatures above 40 degrees f and with the sun shining in the car windshield the dashboard melts and becomes shiny and sticky causing driver vision issues. have contacted manufactures customer service and local dealers for past six months with no help from others. Auto IS low millage 65,000 and IS garaged when at home.located in midwest. this IS more than a cosmetic interior issue, as it causes the drive to have impaired operation of vehicle and should mandate a recall
While driving, front doors unlock themselves. Its been happening often over 3 years.the dealer was unable to repair. While parked, alarm sounds itself. Today it happened two times. This IS new.

Lexus ES 350 2007 dashboard IS melting and compromised. due to the sticky mess the airbags will not deploy properly and death will likely occur. very concerned about the toxicity of the dashboard as well. it's melting, changing color from black to white and cracking. this carcinogenic material IS now poisoning all who ride in the vehicle. included photo does not convey the glossy melting ,sticky reality of it all. the glossy melting reflection causes lots of glare and impairs visibility on sunny days. this issue must be resolved before a life IS taken needlessly. lexus refuses to replace the dashboard even though they admit that its a compromised part and have deceptively allocated this safety issue as a warranty problem that has expired. because my vehicle IS seldom driven and garaged (69,717k miles) problem did not surface until now.
Dashboard melted. while driving, cannot see through the windshield as a result of this unsafe condition.
TL* the contact owns a 2007 lexus ES 350. The contact stated that the dashboard was melting and created a blinding glare that could cause a crash. The vehicle was taken to prestige lexus of ramsey (1000 NJ-17, ramsey, nj 07446, (855) 484-6393), but was not diagnosed or repaired. The manufacturer was not made aware of the failure. The contact also received notification of nhtsa campaign number: 16v340000 (air bags). The failure mileage was 90,000.
The dasboard IS melting severely which causes a bright glare day and night, causing visibility and safety concerns. There IS also a very obnoxious smell to it. I bought this car used and didn't notice the issue at the time. Lexus should just own up to it and replace the dash no matter how old the car IS since it IS a safety hazard to me and others on the road. I personally try not to drive during the daytime when its the worst but thats not always feasible.
TL* the contact owns a 2007 lexus es350. When the contact took the vehicle to be serviced at an independent mechanic, they diagnosed the vehicle and stated that the engine had a fuel leak. The vehicle was not repaired. The manufacturer and dealer were not notified of the failure. The failure mileage was approximately 114,000.
The issue that I am having with my 2007 lexus ES 350 IS while im in motion on a highway or street both of my headlights will shut off leaving the street completely dark putting me and my family at risk for an accident. I have to manually turn the headlight switch off then on again and thats when they will come back on, however this can happens several times while in motion during one extend driving period. Sometimes the left headlight will turn off and the right stays on and vice versa for the right or both. I contacted the dealership and they said I needed the headlamp replaced but once I did some research on this issue with the make and model of my vehicle I realized there are numerous of individuals having the same dangerous issue with having the headlamps replace the issue still remains the headlamps shut off completely inhibiting the view of the highway at night.
During the month of june, 2018 my dashboard top melted and released an obnoxious smell. Now when driving facing the sun there IS a glare on the windshield that prohibits a clear view while driving creating various safety problems. To overcome this hazard the driver has to put on sun glasses and if driving after sunset the glare from street lights, traffic signals, etc., creates many problems by distorting the view thru the windshield. And now,the defective dashboard cannot be repaired other than replacing it with a new one.
Dashboard IS melting. I was told I should have gotten a recall letter. I did not receive a letter pertaining to the replacement of the dash. it IS between 96 to 100% for months here and the dash gets so hot even to touch it. . I put a sunscreen over windshield and punctures it. I am not asking for a replacement but just repairing it.. I love my car and intend to get new one next year but I plan on keeping it for another year. I went to sterling mccall lexus and was told mine offer expired may, 2017. had I received a letter I would replace it then. please help me with this matter. I would be forever grateful.
Steering: linkages: tie rod assembly. While driving the vehicle the front right passenger side shakes as if something's loose while driving
TL* the contact owned a 2007 lexus es350. While driving at an unknown speed, another vehicle crashed into the front passenger side of the contact's vehicle. The air bags failed to deploy. A police report was filed. The contact stated that the front seat passenger sustained a neck injury that did not require medical attention. The vehicle was towed to an unknown towing garage and deemed destroyed. The contact called hoffman lexus at 860-525-3987; however, they could not determine the cause of the failure. The manufacturer was not notified of the failure. The approximate failure mileage was 91,000.
TL* the contact owns a 2007 lexus es350. While driving 70 mph, the vehicle jerked and decelerated. The vehicle was taken to a repair shop where failure codes were found for the transmission computerized system and transmission drive train. The vehicle was not taken to a dealer for diagnostic testing. The vehicle was not repaired. The manufacturer was contacted. The failure mileage was 220,000.
While driving at a slow speed, my front passenger window fell out of the car when I went to raise it. The window didn't break, and I made it home and reinstalled it and held it in with tape. This happened last winter. I have done a lot of research on this, especially in the lexus forums, and this has been happening to other owners as well. In my case, the window clips did not break, but rather there was insufficient adhesive applied to the clips, or the adhesive simply failed. In my opinion, this IS a safety issue for which lexus should take responsibility. Other, later years were also affected, and the cause IS usually 1 or both broken clips. Other owners have reported as many as 3 windows being replaced the lexus, at great cost since the entire window must be replaced. In my case I found an auto glass service that will do it for $250, compared to the dealer cost of $700. But the issue IS safety! If this happened at highway speed and either the driver's window fell out or any other window fell out, it might cause loss of control or possible injuries to others or a passenger. Attached IS a tsb released by lexus.
We have complained about our air bag on the driver side for years before the recall. We have yet to get this corrected even after service claim to have repaired or replace airbag. I am asking for help on this situation it has been to long dealing with. Now dealership asking me to pay additional monies and they are not sure what the problem IS? We have not had any accident just airbag lights stays on.
Dashboard IS very sticky and shiny. A voluntary warranty service recall was issued. The condition produces a glare on the windshield which IS very dangerous. Warranty extension reimbursement offer expired in may 2017. My car has been garaged for 2 1/2 years. Received service note and just set it aside as car was garaged. Recently relicensed vehicle and immediately on first bright day was reminded of the issue. Lexus says offer expired, period. I can certainly pay for the replacement ($1500). I contend this IS an ongoing safety issue and should have been a recall. I contend lexus/toyota avoided a safety recall by preemptively issuing a service/warranty notice. how many es350's are out there with dangerous glare on the windshield? this IS not a cosmetic issue. It's a safety issue. number pol14-03 replacement bulletin number: 12-17 nhtsa item number 10091863 date 2015-12-17 component structure date added 2017-02-15 summary wpb: background: lexus has received reports where some vehicles may exhibit cracked and/ or sticky/ melting dashboards as a result of heat or humidity. In mid-december, 2014 lexus announced the reimbursement phase of this warranty enhancement
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
The office of defects investigation (odi) opened dp09-001 on april 8, 2009, to evaluate a defect petition requesting additional investigation of potential throttle control system defects unrelated to floor mat interference with accelerator pedals in model year (my) 2007 lexus ES 350 vehicles.the petitioner referenced an earlier odi investigation concerning floor mat interference with accelerator pedal return (pe07-016/ea07-010), which he stated was too narrow in scope as it did not include all incidents of unwanted acceleration and consider all potential causes of vehicle speed control concerns.the petition also requested an investigation of my 2002-2003 lexus es300 for those 'longer duration incidents involving uncontrollable acceleration where brake pedal application allegedly had no effect' that were determined not to be within the scope of investigation pe04-021. As background, the petitioner owns a my 2007 lexus es350 that allegedly experienced an unwanted and uncontrolled acceleration event (odi complaint number 10261660).toyota concluded that the incident was caused by an unsecured floor mat. To assess the petitioner's request, odi interviewed the petitioner, inspected his vehicle, reviewed information submitted by toyota, reviewed owner complaints alleging incidents of unwanted acceleration in the subject vehicles and material related to the investigations cited by the petitioner.odi identified 64 complaints alleging incidents of unwanted acceleration in my 2007 lexus vehicles, resulting in 8 crashes and 15 injuries.odi's analysis of these complaints determined that 50 (78%) involved incidents of floor mat interference, including 7 (88%) of the crashes and all 15 injuries.therefore, odi's analysis found that the only defect trend related to vehicle speed control in the subject vehicles involved the potential for accelerator pedals to become trapped near the floor by out-of-position or inappropriate floor mat installations. On october 5, 2009, toyota initiated a safety recall (recall 09v-388) to address concerns with potential accelerator pedal entrapment by floor mats in approximately 3.8 million vehicles, including the subject vehicles.except insofar as the petitioner's contentions relate to that recall, the factual bases of the petitioner's contentions that any further investigation IS necessary are unsupported. In our view, additional investigation IS unlikely to result in a finding that a defect related to motor vehicle safety exists or a nhtsa order for the notification and remedy of a safety-related defect, as alleged by the petitioner, at the conclusion of the requested investigation.therefore, in view of the need to allocate and prioritize nhtsa's limited resources to best accomplish the agency's safety mission, the petition IS denied.this action does not constitute a finding by nhtsa that a safety-related defect does not exist.the agency will take further action if warranted by future circumstances.please see thefederal register notice for further details.
The driver side floor mat will not interfere with the throttle pedal if properly secured using the retaining hooks provided by toyota.however, if the all weather mat IS unsecured and moves forward from its intended position it can entrap the throttle pedal at the fully open position after it has been depressed.this can happen regardless of whether or not another mat (carpet) IS underneath.when this occurs, some operators react by applying the brake pedal multiple times, depleting the braking system's (vacuum based) power assist.stopping the vehicle with unassisted braking while the throttle IS fully open requires significant pedal force, which some operators did not, or were unable to, apply for the required duration.continued driving in this condition results in overheated brakes, which further diminishes the braking effectiveness.some operators attempted to turn the vehicle off by depressing the engine control button, however they were unaware the button had to be depressed for three seconds to stop the engine when the vehicle IS in motion; this functionality was not explained adequately in the owner's manual.in odi interviews, owner's reported they were unaware the all weather mat had the potential to interfere with the throttle pedal, advising that this was never properly explained given the significant hazard it represented.owners reported several explanations for why the mat was unsecured, including that the vehicle was delivered in this condition (most common reason), or that they had (unwittingly) installed the mat themselves, or that another party, such as a car wash attendant, had disturbed the mat without the owner's knowledge.one consumer reported that their vehicle was delivered without the retaining devices installed.in a september 26, 2007 letter to nhtsa, toyota indicated that they would conduct a safety recall to replace the all weather mat with a redesigned mat.according to toyota, the new mat design will reduce the potential for mat interference with the throttle pedal.the population noted above represents the number of subject matsthat have been sold in the united states.since toyota cannot identify which vehicles may have the subject mats, they will send a letter to all registered owners of the subject vehicles (estimated 750K) advising them of the concern and remedy.a copy of that letter will be posted to nhtsa's web site under recall 07e-082; it contains a description of the condition and the actions an operator should take in the event throttle entrapment occurs while driving.the fatality incident noted above occurred in july 2007 and was investigated by nhtsa's special crash investigations office.it has not been reported on a vehicle owner questionnaire.the operator reportedly travelled at speeds in excess of 100 mph for an estimated eight miles on an interstate in california before it struck two other vehicles.one of the struck vehicles and the subject vehicle caught fire.the occupant of the struck vehicle did not evaculate and died at the scene.the subject vehicle driver suffered a broken bone.the california highway patrol IS investigating the incident also.odi did not issue an information request letter during this investigation.the toyota reports noted above were reported during preliminary evaluation pe07-016 and are current through april 2007 for my 2007 lexus es350 only.odi does not have field experience data from toyota for camry vehicles.throttle entrapment due to improperly installed floor mats could be a concern in all vehicles.therefore, drivers should always ensure their floor mats are properly and safely installed.this incl
Complainants interviewed by odi stated that they applied the throttle pedal to accelerate the vehicle then experienced unwanted acceleration after release.subsequent (and sometimes repeated) applications of the brake pedal reduced acceleration but did not stop the vehicle.in some incidents drivers traveled significant distances (miles) at high vehicle speeds (greater than 90 mph) before the vehicle stopped (odi notes that multiple brake applications with the throttle in an open position can deplete the brake system's power (vacuum) assist reserve resulting in diminished braking).the subject vehicles have an engine control button instead of a traditional ignition key. Some drivers reported that their attempts to turn the engine off by pressing the control button were unsuccessful.in these cases it appears the drivers were unaware that the button's functionality changes when the vehicle IS in motion requiring that it be depressed for 3 seconds (instead of momentarily) to stop the engine.odi has observed that an unsecured lexus accessory all weather floor mat can trap the throttle pedal in an open position resulting in significant unwanted acceleration.unsecured floor mats have been found in the majority of incident vehicles.in addition, odi has inspected two incident vehicles documenting brake damage due to overheating. In the five odi crash incidents the operators allege they experienced unwanted acceleration and were unable to stop the vehicle to avoid the collision.four of the five crashes are multi-vehicle (one involving 7 vehicles) and two resulted in total loss damages (one involving a fire).one injury, which resulted from a roll-over crash, was significant and the injured person continues to show symptoms.one injury incident IS not crash related (complainant alleges a knee injury due to overexertion during brake application). In their response to odi's april 5, 2007 information request letter, toyota acknowledges that some of the alleged incidents are likely related to improper installation of the driver side all weather floor mat resulting in interference with accelerator pedal movement.toyota describes various changes to the labeling of the all weather mat (including a sticker for the bottom of the mat and enlarging the statement on the visible surface) and the packaging used for sales of the mats, and has notified odi of an owner mailing to subject vehicle owners warning of the dangers of improper mat installation.toyota believes the subject vehicles and the all weather mat do not contain a safety related defect and that the actions they have taken are sufficient to address any future concerns. This investigation has been upgraded to an engineering analysis to further investigate this issue and to assess the actions taken by toyota.
