Brake system sets a high tone pitch indicating failure accompanied by all Brake, ABS, PS, Traction Control Lights. This is an know failure for Hybrid systems. It can occur completely random, while driving without notice. Complete brake pressure is lost with exception of a hard feel to the pedal that allows some incomplete breaking to occur and can also cause locking of the brakes at slower speeds. We replaced The brake actuator as recommended with factory new in March of 2024 Started much earlier. Problem has returned Summer of 2025.
- Fuel/propulsion system
After adding gasoline to the tank, gasoline begins dripping from the left rear of the vehicle to the rear of the left rear wheel well. This happens even if the tank is only filled to 2/3 of a tank. I have stopped filling up since when the tank is filled up a LARGE amount of gasoline leaks out onto the ground. FIRE HAZARD while parked or driving. I have seen HUNDREDS of reports of this problem reported online.

From what I have been reading on that year and model, there seems to be major issues with the invertors. Like many I was driving on the highway and the vehicle just quits. This put me and many others in a very dangerous situation and I could have been hit at high rates of speed. If as many people are having these issues, then Lexus needs to step up and recall this part before someone gets killed

The power steering and hybrid system failed and vehicle completely stopped in middle of busy freeway with no prior warnings or issues before. The safety of my son and I, and others around me during the abrupt stop of the engine. I could not move out of way. The vehicle has been serviced for this exact issue during a recall campaign lasting from 2011 to 2014. Ten years later and it is still happening! There were NO warning lamps or messages or symptoms prior to this failure. It just passed vehicle inspections for the year

Dear manager: I have this 2006 LEXUS RX400H which has CHECK VSC SYSTEM and CHECK HYBRID SYSTEM problems. When I was driving on the highway, the cruise control button is unable to turn on, when at speed of 65 MPH it will suddenly stall in the middle of the road and the READY light will be off and the vehicle has to be stopped by the emergency lane which is REALLY DANGEROUS. I have searched online and found out there were many complains about this issue and LEXUS issued a recall for this type of car and the dealer can change the inverter and ICM free of charge. This recall link is as follows: https://static.nhtsa.gov/odi/rcl/2011/RCONL-11V342-6633.pdf So tonight I am sending this message to see if you can help me to solve this problem.

Hello, I had this issue on my lexus rx400h recently, and it was diagnosed and repaired by a third party auto repair shop at my expense. I want to know if I have any recourse to recover my cost approx $700. Thanks! vin recall consequence: the vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. It IS possible that the hybrid system will shut down while the vehicle IS being driven, causing the vehicle to stall unexpectedly, increasing the risk of a crash. Recall summary toyota IS recalling certain model year 2006-2007 highlander hybrid and lexus rx400h passenger cars manufactured from february 16, 2005, through august 30, 2006. A module inside the inverter module may contain inadequately soldered transistors that during high-load driving, may be damaged by heat caused by the large current. If this occurs, various warning lamps, including the malfunction indicator lamp, slip indicator light, brake system warning light, and master warning light, will be illuminated on the instrument panel. Recall solution toyota/lexus mailed an interim owner notification on july 19, 2011 to advise owners of this recall and the fact that they will receive a future notice when parts become available to complete repairs. Toyota dealers will inspect the hybrid inverter production number to determine whether the inverter contains suspect transistors and replace the module free of charge. Owners may contact toyota at 1-800-331-4331.

This vehicle seems to have a well-documented problem with water intrusion that damages the ECU and hybrid battery. The 2006 and 2007 RX400h has the HV battery located under the seat, which is the low spot in the rear section of the car. If water is spilled or the body has a water leak, the water will pool at the center of the battery. Lexus considered this, and added a drain under the battery, but the drain has a filter, presumably to keep insects out, and the filter is easily clogged. This, coupled with a pattern problem of split body seams near the rear hatch strut mount causing a water leak in the rear of the car, has created isolation faults in many of the RX400h SUVs. The condition presents as warnings on the instrument cluster and a no-start condition where the vehicle is inoperable. A search for "lexus ECU water intrusion" brings up my vehicle's exact model and year so this is well-documented.

My car exploded- huge fire. I was not driving- it had been parked. The fire marshal indicated that it was an electrical problem located near the battery. There we no indicators of any issues. I only had liability on the car- so i am out of luck. but the NYFD inspected. It happened on Wednesday 9/29/21. Was inspected on 9/30.

Hybrid propulsion system - inverter failure how do we get our vin on the recall list? our repair should be covered. our issue IS exactly the same problem encountered by the vehicles covered under the hybrid propulsion recall (2005-2008) and our 2006 vehicle IS a year which IS covered my wife was in a dangerous situation for 4 hours because of this faulty system which has a history of failing and has a known recall a vehicle which had no issues, within a few minutes, turned into a dangerous vehicle losing power on the turnpike, breaking down in the middle of the night, and IS now inoperable and worthless, IS unacceptable if toyota and lexus are reputable companies and stand behind their product, I would hope they stand up and accept responsibility for an obvious faulty hybrid system

I bought a lexus rx400h back in 2013-2014 and recently my car experience some serious issues. While driving on the highway to take my son to his football game my ABS system completely went out on me almost causing me to crash. I recently went to lexus to find out the problem and was told it was my actuator. I ask lexus about a recall I was informed of back in 2016-2017 for actuator and they stated it never was a recalled. Since I never was notified and only heard about it from another lexus mechanic I ask him for documents and he tried to throw away the paper that showed my car was recalled for the actuator, melting dash board and economic loss class action lawsuit. I need help figuring out what I can do to get lexus to honor the safety recalls on my car.

On February 16, 2010, NHTSA's Office of Defects Investigation (ODI) opened RQ10-003 to determine whether the scope of prior Toyota recalls relating to potential unintended acceleration were sufficiently broad, including, among others, Toyota recalls 07E-082, 09V-388, 10V-017, and 10V-023.NHTSA also requested information regarding potential electronic causes of unintended acceleration in Toyota and Lexus vehicles.I. Pedal Interference from Floor mats, Carpet Covers and Plastic Pads in Carpets.During the RQ10-003 investigation, ODI reviewed a large volume of documents to assess whether additional vehicles should be recalled.Following the agency's analysis, NHTSA requested that Toyota recall additional vehicles.Toyota complied with the agency's request. The details of these recalls are set forth more fully in Toyota's reports to NHTSA pursuant to 49 CFR Part 573.These recalls are as follows:Recall 11V-112: (1) model year (MY) 2004-2006 Toyota Highlander and Highlander Hybrid; and, (2) MY 2004-2007 Lexus RX330,RX350, and RX400h (hybrid model). The total estimated population under this recall is 769,379 vehicles. This recall remedies potential accelerator pedal entrapment caused by a loose floor carpet cover (trim panel). Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles. This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles. This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration. After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles. That study has concluded. Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.

- 11003650To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
- 11003648The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. While Lexus paint quality, durability, and resistance to environmental factors is continuously being improved, it remains the responsibility of the dealer to protect and maintain the vehicles paint finish from the time of arrival at the dealership until retail delivery to the customer. In areas known for high frequency and/or concentration of acid rain, frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting. SUPERSESSION NOTICE The information contained in this bulletin supersedes Service Bulletin No. BO029-91. The entire bulletin has been updated. Service Bulletin No. BO029-91 is obsolete
- 11003643Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: Minor damage: requires only buffing to repair. Moderate damage: usually requires neutralizing, color sanding, and buffing. Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. While the quality and durability of paint on Lexus vehicles is continuously being improved, it remains the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.
- 11003642Lexus vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination. Follow the Removal Procedure in this bulletin to remove the RapgardTM protective film within 90 days from initial application. SUPERSESSION NOTICE The information contained in this bulletin supersedes Service Bulletin No. L-PG004-94. The entire bulletin has been updated.
- 11003641Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid. Follow the Repair Procedure in this bulletin to clean vehicles that may have been subjected to contamination by airborne iron particles such as rail dust during rail transportation or extended storage near industrial areas.

- 10177780TSB: SUPERSESSION NOTICE The information contained in this bulletin supersedes Service Bulletin Nos. ST004-01, SU001-08, and L-SB-0174-08. The aforementioned bulletins are obsolete, and any printed versions should be discarded. Be sure to review the entire content of this service bulletin before proceeding. This Service Bulletin provides best practice procedures for vehicle pulling complaint, diagnosis, and repair for 2002 ? 2021 model year Lexus vehicles. This information supplements Repair Manual procedures when the symptoms are: ?Vehicle Pulling: The vehicle moves to the right or left when the driver holds the steering wheel while driving straight ahead without exerting steering effort. ?Steering Wheel Off Center: The vehicle travels straight, but the steering wheel is not pointed straight ahead. The vehicle is not pulling. Before repairing a vehicle pulling to one side, it is necessary to clearly identify the cause of the pulling condition. Frequently, pulling to one side is diagnosed as a wheel alignment concern. However, the actual cause may be related to the lateral force generated by the tires, vehicle damage, road crown or worn components. Performing a wheel alignment when tire force is the cause could result in the wheel alignment being set at a value outside of specifications. This could then cause other problems such as uneven tire wear, etc. A customer may notice a pull to the left when in the left lanes of travel and notice a pull to the right or no pull when traveling in the right lanes of travel on the very same section of roadway. This road crown effect is environmental and cannot be corrected with suspension adjustments or any other wheel or tire modification discussed in this bulletin.
