The brake pedal on my 2021 Ford Explorer intermittently sticks or fails to return to its upright position after being depressed. This mechanical binding and failure to release changes drastically with seasonal weather transitions and ambient temperature drops. When the vehicle or the outside air is cold, the pedal requires me to physically hook my foot underneath the pedal arm to pull it back up so that the brakes fully disengage. This creates a severe safety hazard, as the brakes remain partially applied or sluggish to respond, which causes unexpected vehicle deceleration, brake drag, and a delayed throttle response when trying to accelerate. The vehicle is unpredictable to drive in traffic. The vehicle has been inspected, and the issue is characteristic of an internal manufacturing defect or tight tolerances within the Electronic Brake Booster (EBB) assembly under temperature fluctuations, similar to the issues addressed in Ford’s prior Customer Satisfaction Program 20B51.
The contact owns a 2021 Ford Explorer. The contact received notification of NHTSA Campaign Number: 26V117000 (VISIBILITY); however, the part to do the recall repair was not yet available. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue.
On October 21, 2026, after an approximately one-hour round trip in our 2021 Ford Explorer, we arrived home and noticed smoke coming from under the hood. When we opened the hood, we found oil spread throughout the engine compartment. The smoke and oil leak raised concerns about a possible fire and the safety of continuing to drive the vehicle. The vehicle had recently received its second replacement engine, installed by Stuckey Ford of Bellefonte on September 2, 2026. We brought it back to that dealership on October 23. Service staff told us they believed the oil leak involved the valve cover gasket on the recently installed long block. This is a preliminary diagnosis; we have not received written confirmation of the cause. As of October 26, the vehicle remains at the dealership awaiting repair and is available there for inspection upon arrangement. Whether Ford has independently inspected this new issue is unknown. The vehicle has a history of repeated oil loss and low oil pressure warnings. The original engine was replaced under warranty in June 2024 at approximately 39,574 miles after the dealership documented oil in cylinder 2 and heavy cylinder wall scoring. In June 2026, we again reported almost no oil and a low oil warning. In July 2026, at 77,206 miles, the dealership documented excessive oil consumption and internal oil burning in cylinder 3, leading to the September replacement. Shortly after that replacement, the vehicle also displayed an overheating warning and required towing. The dealership found that a coolant hose had come off and repaired it in September. Whether warning lights or messages appeared before the October 21 oil leak and smoke incident is unknown. The repeated engine problems and this new incident have left us concerned about safely using the vehicle.
Fire started around center console when car was parked. HVAC/ info screen/ rear view mirror substained severe burn.
The contact owns a 2021 Ford Explorer. The contact stated that while driving 30 MPH, the vehicle suddenly shut off. The vehicle was pulled over safely and turned off; however, the vehicle failed to restart. The vehicle was towed to an independent mechanic and was diagnosed with low-pressure fuel pump failure. The contact associated the failure with NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline); however, the VIN was not included in the recall. The dealer was notified of the failure. The manufacturer was notified of the failure. The failure mileage was 78,209.
They have fixed zero recalls. I have no wipers at all. I’ve attempted to get a resolution multiple times with no care or concern nor resolution. This is ridiculous. A recall almost 8 months and people have zero wipers!!! And the engine recall. It’s going to blow up before it’s fixed!
My vehicle was just diagnosed with trouble code P2888 (Park Lock Actuator Circuit/Open). This prevents the vehicle from shifting safely into Park. SHIFT SYS FAILURE was the error displayed in the dash readout. This matches the exact model, year, and dangerous symptoms outlined in Ford Safety Recall 26S48. My local Ford dealership has recommended the exact repair specified by that recall—a full transmission valve body replacement. Despite having the identical component failure and safety risk as the recalled vehicles, I am being asked to pay out of pocket solely due to my VIN assembly date. Because this is a known, documented manufacturing flaw, I respectfully request that Ford provide goodwill coverage to resolve this repair. They denied it despite acknowledging the matching details.
The right rear passenger door of my 2021 Ford Explorer ST repeatedly becomes unlatched and physically opens while the vehicle is in motion, including at highway speeds of approximately 72 mph. This occurs almost every time the vehicle is driven and creates a serious safety hazard. The door visibly separates from the vehicle’s body, which I can see in the passenger-side mirror. When this occurs, the dashboard displays a door-ajar warning, the interior lights illuminate, and wind can be heard entering through the opening. This is not simply a faulty warning indicator. The door is physically opening while the vehicle is moving. I am extremely concerned about the risk of the door opening farther and potentially causing a passenger, particularly a child seated in the rear, to be seriously injured or ejected from the vehicle. This defect was present before the vehicle was taken to Tipton Ford in April 2026. I also recall that that when I originally purchased the vehicle, the child safety lock was engaged on the right rear passenger door only. I found this unusual at the time, and given the ongoing malfunction involving that same door, I am concerned the issue may have existed before my purchase. Tipton retained the vehicle for approximately one month and completed repairs on May 8, 2026. However, the repair documentation indicates that the LEFT rear door latch assembly was replaced, while the RIGHT rear passenger door has consistently been the door physically opening. The dealership documented reproducing an intermittent door-ajar warning and replacing the left rear latch assembly. However, the original right rear door malfunction remains unresolved. The cause of the right rear door malfunction remains unknown. The vehicle is available for inspection. I am requesting an investigation into this potentially dangerous door-latching defect before it results in serious injury or death.
Never really used sun roof , but accidentally hit button to open … then closed….. not realizing that the cover didn’t close it SAG down behind my head… blocking rear view completely. It was dark out and had to pull over and try to fix so I could see out my rear window
The contact owns a 2021 Ford Explorer. The contact stated that while driving in rainy conditions, windshield visibility remained poor despite replacing the wiper blades multiple times. The contact received notification for NHTSA Campaign Number: 26V117000 (Visibility); however, the part required to perform the recall repair was unavailable. The dealer was contacted. The contact stated that the manufacturer exceeded a reasonable amount of time to provide the recall remedy. The failure mileage was 83,000.
Vehicle: 2021 Ford Explorer XLT - 2.3L EcoBoost I-4 VIN: [XXX] Odometer: 127,824 Date of failure: [XXX] Complaint Description: I am reporting a complete engine failure due to coolant intrusion on my 2021 Ford Explorer 2.3L EcoBoost. This is the known defect affecting 2016-2024 Explorer 2.3L engines. Independent shop diagnosis from attached receipt states: "Engine failure due to Head problem, which cylinder 3 + 4 and head in engine known for coolant intrusion. Cylinder Head failure, carbon build up plus Turbocharger also failure." Vehicle stored DTCs P0300, P0302, P0304, P0316 - random misfire and start-up misfire. Symptoms included rough idle on cold start, white smoke from exhaust, loss of power, check engine light. Mechanic confirmed coolant in cylinders 3 and 4 due to cylinder head failure. This matches Ford's own TSB process for coolant intrusion where if coolant is found, the long block must be replaced. Safety Risk: Loss of drive power, stalling, overheating. This occurred at 127k miles with regular maintenance. This is not wear and tear - this is the documented design defect where machining grooves in the head allow coolant to pool and leak into cylinders. I have requested Ford Customer Relationship Center (1-866-436-7332) open a case for goodwill and check VIN for open recalls including 23S05 (PCM/10R60) and the June 2026 2.3L long block recall for roller finger failure. Ford has not yet issued a formal recall for coolant intrusion despite class action litigation. Request: NHTSA investigate and expand recall to include 2021 Explorer 2.3L EcoBoost coolant intrusion / cylinder head failures. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
I am extremely disappointed with the safety and reliability of this vehicle. While I was backing up with my five children in the vehicle, the right-side control line suddenly snapped in half. There was absolutely no warning or indication that anything was about to happen. The failure left us stranded in the middle of a busy road, putting my children and me in a very dangerous situation. I had to call the police for assistance, and the responding officer actually identified the issue as the vehicle’s control line. A major vehicle component failing suddenly and without warning, especially with five children in the car, is extremely concerning. I expected this vehicle to be safe and reliable, and this experience has seriously shaken my confidence in it. I strongly believe this issue needs to be investigated to determine whether there is a defect or safety concern with the control line, particularly because of how suddenly it failed. I would not want another family to experience what we went through.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The dealer was contacted and confirmed that parts were not yet available.
The passenger window and the driver window will not respond. The windows will not go up on either side, exposing the driver and passengers to the elements of rain. In addition, the car is vulnerable to theft and break-ins. The windows are part of a larger electrical problem that the car has, including key fob delays and screen shut off during driving. The problem has been confirmed by the service center. No warning signs are available for these problems.
The contact owns a 2021 Ford Explorer. The contact stated that while driving 65 MPH, the vehicle ran over an object and a moment later, the vehicle suddenly jolted very hard and transmission was upshifting and downshifting erratically. The contact was able to pull over safely. The contact became aware of a foul burning odor inside the vehicle cabin of the vehicle coming from the transmission. The manufacturer was notified of the failure. The vehicle was taken to an authorized dealer where technicians confirmed a front axle leak and discovered that the internal E-clutch inside the transmission had come apart, causing the transmission fluid to burn. The contact referenced Ford Special Service Message SSM: 49507 and Ford Technical Service Bulletin: 23-2250; as possible causes of the failure. The failure mileage was 69,000.
I was driving down the road and the vehicle started to cut out like it was running out of fuel. Maybe 1 minute later it died in the middle of heavy traffic and would not re-start. I had to have it towed and I called Ford and they told me to wait until the repair remedy was available to have it fixed. I have been without this vehicle for 6 weeks now and still no remedy.
Ladder being shut off, I exited the vehicle. I shut the driver side door. The car locked itself after shutting . Locking the keys and entrapped my 1.5 year old for 30 minutes while I waited for police to come and assist.
While driving the vehicle, in any mode, the car will stutter or "cough" causing it to slow down. I have to pull the vehicle to the slow lane until it eventually corrects itself. Also, on multiple occasions when I hit the gas the car stalls until I let off the gas pedal and let the car "think" for a moment and then it will drive. This happens a lot at stop signs or when the light changes. I will hit the gas to go and the car does nothing. It feels like the engine stalls even though the car is still on. On a few occasions I will be driving at high speeds on the interstate and I will punch the gas to go around a car and it stalls. On EVERY occasion when this happens, whether I am pulling out at a busy intersection or am driving with the flow of traffic on the interstate, I am around multiple cars and cars are having to hit their brakes or swerve to avoid my vehicle. When this happens it only takes about 10 seconds for the car to go again but that is still long enough to get hit. I am scared to drive my car with my children in it because of this. A have contacted the dealership but they say to bring it in when it is acting up but it does not do it everyday so there is no way for me to do that. They also said they could keep it and drive it but they do no offer a loaner vehicle so I cannot be without my only vehicle for multiple days at a time. On other occasions the vehicle seems like it looses momentum and is very sluggish. Sometimes the car jerks into gear instead of a smooth transition.
Within a month of Purchasing the Vehicle, around end of June, the image on the back up camera would flip upside down when trying to reverse. This only happens occasionally but the times that it has happened I have backed into a ditch and ran over a child's bicycle. There has been no damage to my vehicle as of yet. Because this does not happen regularly the dealership has not been able to look at it and diagnosis it. They tell me to bring it when it is doing it but that is impossible unless I drive in reverse the entire way to them.
Since buying the vehicle the front windshield wipers will intermittently stop working during a rainstorm causing me to have pull over and either wait out the rain or getting out in the rain to wipe the windshield. Sometimes if I turn the vehicle off for a few minutes the windshield wipers will start working again. If I had not pulled over during these times I could have been in a severe accident. Yes the vehicle has been inspected and they said there was no remedy for it as of now. No warning lights have came on.
About 3 months ago, every time I started the vehicle, I heard a whining noise coming from the 2nd row passenger seat that my [XXX] sits in with her car seat that We could not figure it out for awhile. I then took her car seat out of the car 2 weeks ago to clean underneath it and the seat automatically folded up with force. I jammed it back into the lock position and it would hold, sometimes for an hour or so and sometimes for a couple days before it started whining again and folding back up. I switched the car seat to the other seat and put my [XXX] in the seat that kept folding to see if it would fold on her and It folded up on her while I was driving down the interstate causing a big bruise on her arm and causing her to severely stub her toe on the bottom of the seat in front of her due to the seat sliding forward and folding with such force. I pulled over on the interstate and put her in the 3rd row. I reached out to FORD customer relations on September 29th and told them all of this and this was their reply. " There is not a remedy for that until maybe end of October so I would just stop driving the vehicle with your kids in it" We argued back and forth about how that was not possible because this is the vehicle that I bought to haul my kids around and eventually I asked for a supervisor to call me back. He called me back and left a voicemail and this is what he said " We cant fix something that we don't have parts for so call the dealership and get put on a call back list so they will call you when the new parts come in" I will be contacting the dealership on Monday as soon as it opens to get my car in asap or to ask for a loaner until I can get it fixed since it is not safe for my children or any other adult to ride in it. As of now nothing has been inspected . There is no warning lights that have came on the vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. In view of the recall action being taken by Ford, ODI is closing this Engineering Analysis. On July 17, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 2 consumer complaints alleging loss of vehicle forward power and/or loss of transmission torque of the rear wheels in model year (MY) 2020-2022 Ford Explorer vehicles (subject vehicles). The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. Recall 23V-199 addressed 674 of the subject vehicles produced from February 2, 2019 to April 4, 2022, which received an incorrect remedy under Recall 22V-255. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque. According to Ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by Ford, ODI is closing this RQ. NHTSA will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary. The agency reserves the right to take additional action if warranted by future circumstances.
