Vehicle Information:Year/Make/Model: 2015 Honda Odyssey EX-L VIN: [XXX] Mileage: Approximately 45, 200 miles Component / Defect: Engine Oil Leak leading to Alternator Failure (Front Spool Valve / Valve Cover Gasket Leak onto Alternator) Summary of Incident and Service History: My car overheated and would not start on several occasions over the last year and a half starting in July 2025, leaving me stranded and requiring it to be towed. My car has been in service at the dealer multiple times for approximately 2 weeks each time. Oil Leak and Alternator Hazard: The vehicle experienced engine oil leaking directly onto/into the alternator. According to communications from Honda service personnel ([XXX]), oil leaking from the front spool valve and valve cover gaskets penetrates the alternator, causing component failure. The dealership explicitly warned that failure to complete these repairs would result in repeated alternator failure. XXX Service: The vehicle was brought in for diagnostic work and repairs at Brickell Honda Performed services included diagnostic procedures, engine oil & filter replacement, radiator fan motor replacement, A/C condenser fan motor replacement, and tire rotation. Total Paid: $2,443.60 Additional Repair Estimates Provided: Valve cover gaskets & spark plug replacement: $1,901.18 plus tax Front spool valve replacement: $1,641.70 plus tax Prior Related Invoices: An earlier invoice was issued on XXX for $2,096.29 by Brickell Honda for alternator replacement and other maintenance services. Safety Risk Statement: An unaddressed oil leak dripping onto the alternator poses a serious electrical component failure hazard while operating the vehicle. Sudden loss of electrical power while driving can cause engine stalling, loss of power steering, loss of headlights/lighting, and disablement in traffic, presenting a significant risk of collision and vehicle safety hazard. I am reporting suspected odometer tampering / odometer fraud. Vehicle: 2015 Honda Odyssey Touring Elite VIN: [XXX] Current odometer displayed: ∼96,xxx miles Actual mileage per history: 215,407 miles Seller Information: Name: "[XXX]" Phone: [XXX] Location: [XXX] Platform: OfferUp URL: [XXX] Evidence - CARFAX Report (attached) On XXX CARFAX shows two entries on the same day: XXX - 215,407 miles - Source: Auto Auction - Vehicle offered for sale / Vehicle sold XXX - 96,320 miles - Source: California Inspection Station - Passed emissions inspection - Flagged as MILEAGE INCONSISTENCY On XXX, same day, California DMV shows Title issued / New owner reported (Owner 2). CARFAX Additional History shows Owner 1 = Mileage Inconsistency, Owner 2 = No New Issues. The vehicle lost 119,087 miles between the auction sale and the smog check on the same day, before the new title was issued. This indicates the odometer was rolled back after purchase at auction and before obtaining a smog certificate to obtain a clean title as Owner 2. This appears to violate California Vehicle Code XXX (unlawful to disconnect, turn back, reset odometer with intent to alter mileage) and Federal Odometer Law 49 U.S.C. § 32703(2) and § 32705. I first experienced an engine misfire in February 2025. The vehicle had a P0301 misfire code, and at the time I was unaware of Honda’s extended warranty related to this issue. We had a trusted mechanic friend inspect the vehicle and replace the spark plugs and ignition coils. He noted that there was significant carbon buildup. After those repairs, the vehicle appeared to run normally, and we believed the issue had been resolved. Later, I received Honda’s extended warranty notice and realized that the issue I experienced may have been related to the condition covered by the warranty. I contacted my local Honda dealership in August 2025 and explained the prior misfire, repairs performed, and the warranty notice I had received. However, because the check engine light was not currently illuminated, I was told there was nothing that could be done unless the vehicle was actively displaying the issue. I also contacted Honda Corporate regarding this matter. I was advised that reimbursement for some of the repairs I had already completed might be possible, but that it would need to be reviewed. I chose not to submit those receipts at the time because I was concerned that doing so might impact my ability to use the extended warranty coverage if the problem returned. Unfortunately, the issue appears to be progressing. On June 2, 2026, the vehicle set a P3400 code, which is related to the Variable Cylinder Management (VCM) system. At that time, the oil level was low. After adding oil and completing several driving cycles, the check engine light turned off. However, the vehicle continues to consume oil at an alarming rate. I have had to add additional oil again in July. The problem is progressing - it is the issue that should be covered by warranty but holds refuses to do anything because the light isn’t currently on,

American Honda Motor Co., Inc. issued an official Warranty Extension in July 2025 covering engine misfires from piston ring deterioration (DTCs P0301-P0304) on 2013-15 Pilot and 2013-17 Odyssey vehicles. My 2015 Honda Odyssey (VIN: [XXX]) experienced this exact defect, resulting in severe engine misfires and secondary alternator failure caused directly by engine fluid saturation leaking onto it from the primary defect. I paid $5,275.80 for repairs prior to campaign awareness (Invoices XXX and XXX). I submitted complete reimbursement documentation on January 31, 2026. Honda has withheld my funds for 178 days (which is 122 days past their promised 8 week deadline) while corporate customer care agents falsely claim payment is “approved but pending final management sign- off.” I have called 11 times and corporate managers who are never available HAVE NEVER returned my calls like the representative with Honda I've spoken with each time assures. This deliberate delay subverts federal safety campaign integrity and artificially manages liability exposure. Request immediate enforcement action for an ongoing corporate warranty evasion pattern. I bought this car from a dealer in November of 2025 just over 95k miles. Two weeks after I bought it a PO303 code for a cylinder 3 misfire appeared. I changed the spark plug and the code went away. Then on Friday, April 10th, the code came back. I replaced the spark plug for a 2nd time. The code disappeared again. Then, on April 25th the code returned and I replaced the cylinder 3 spark plug for the 3rd time. After replacing the cylinder 3 plug for the 3rd time I took the car to an auto mechanic in my area. After their diagnosis I am looking at either repairing the current engine at a cost of 4500 dollars or replacing the engine at a cost of 7-8k. I believe my engine issue is the very same issue that 2011-2013 3.5l v6 Honda engines suffer from that resulted in a lawsuit and settlement.

However, the misfire has now returned with the check engine light on, again indicating cylinder misfire. The vehicle only has 71,000 miles on it, which makes this recurring issue particularly concerning. Based on my research, this appears to be a known defect related to Honda’s Variable Cylinder Management (VCM) system causing excessive oil consumption and spark plug fouling — an issue documented across 2013–2017 Odyssey vehicles and addressed under Honda Service Bulletin 25-061. I am requesting this complaint be on record and urge NHTSA to investigate this as a safety-related defect.”

I am submitting this urgent complaint regarding my 2015 Honda Odyssey, equipped with the 3.5L V6 engine referenced in (TSB) 20-023 which acknowledges that the front rocker arm oil control valve (spool valve) gasket may fail and leak engine oil directly onto the alternator located beneath it. This condition can cause alternator contamination, charging system failure, electrical malfunction, and potential fire risk due to oil exposure to electrical components. Despite knowledge of this issue, Honda did not notify vehicle owners and did not initiate a recall. Before the first breakdown occurred, I experienced a frightening and dangerous incident while in traffic. Without any human interference and without my pressing the accelerator, the vehicle suddenly accelerated and moved forward on its own, nearly striking the vehicle in front of me. I had to react immediately to prevent a collision. This unexpected surge was extremely disturbing and appeared to involve an electrical malfunction. Shortly thereafter, the vehicle broke down. After the first breakdown, I replaced the battery. Within approximately one hour of driving again, the vehicle failed again due to alternator failure. I was stranded at night, in darkness, alone, and exposed to active traffic. This created an immediate and foreseeable risk of collision, serious bodily injury, or death. Another car in that area around that time broke down on the highway I was expected to use and was hit by a semi on 95 and the driver of the stopped car was killed. That could have easily been me. A defect that can cause sudden electrical malfunction, unintended vehicle movement, loss of charging capability, vehicle disablement in traffic, and potential fire hazard constitutes a safety-related defect under federal motor vehicle safety standards. I respectfully request that you Initiate a formal investigation Evaluate whether a recall is warranted. Require reimbursement for repair costs Thank you very much.

Honda issued a warranty extension covering piston ring deterioration that causes engine misfires (DTCs P0301–P0304) due to oil fouling. I experienced this exact condition on 3/11/25 prior to issuance of the warranty extension, resulting in an engine misfire and loss of drivability. To restore basic operation of the vehicle, an independent repair facility replaced a fouled spark plug, documenting the misfire and oil fouling condition (including the P0301 code). I elected the minimum necessary repair to restore drivability, as the warranty extension had not yet been issued and the vehicle could not be operated safely without immediate service. Honda has acknowledged that this repair was temporary and consistent with the covered condition. Honda has denied reimbursement for this repair because the piston ring or short block replacement identified in the campaign was not completed. Honda has also stated that it will not perform the covered repair unless the misfire code is currently active or stored and the check engine light is illuminated. Honda representatives have acknowledged that this condition commonly recurs after a temporary spark plug repair, but have advised that no repair can be performed unless the issue resumes. This creates a situation in which the vehicle has a known covered defect, but access to repair is delayed until failure reoccurs, potentially outside the warranty extension mileage or time limits. I am submitting this complaint due to concerns that these requirements may prevent timely access to emissions-related repairs for a known defect and may discourage reasonable interim repairs necessary to maintain safe vehicle operation. Honda’s requirement that I continue operating the vehicle until the misfire reoccurs raises safety concerns. The prior failure resulted in loss of drivability, and I am uncomfortable transporting my family while knowingly operating a vehicle with a documented defect that Honda has acknowledged is likely to recur.

On December 31 at approximately 300 p.m., our 2015 Honda Odyssey experienced a sudden and complete loss of motive power while traveling eastbound on the Sam Houston Parkway South just after crossing the Buffalo Bayou Toll Bridge at highway speed. While driving normally, the Check ABS warning light illuminated. Immediately afterward, multiple dashboard warning lights began flashing and the engine abruptly shut down with no warning. The vehicle lost all ability to accelerate and became disabled in active traffic lanes. Steering and braking were severely affected due to the sudden loss of power. Hazard lights were activated while the driver attempted to move the vehicle from the middle lane to the shoulder. We narrowly avoided a collision and were able to stop near the Pine Avenue on-ramp. Once stopped, the vehicle completely shut off and would not restart. All vehicle systems appeared inoperative except for the hazard lights, indicating a possible electrical system failure. Emergency services were required due to the dangerous location. The vehicle was occupied by five people, including three children. The sudden loss of motive power at highway speed created an extreme safety hazard and could have resulted in serious injury or fatality.

The car sporadically does not shift from 1st 2 second and then abruptly shuts cause a loud jerk of engine and car. It is startling and scary when it happens. I’m afraid the car will break down while in the road. Every time we take the car to dealership they “cannot replicate” issue therefore there is nothing to fix on an issue that is all over the internet with Honda vans and suvs.

The Office of Defects Investigation (ODI) has completed its review of a Petition dated May 30, 2026. The Petition requested an investigation into inadvertent air bag deployment in model year (MY) 2011-2017 Honda Odyssey vehicles. The Petition contended that air bags can deploy absent a sufficiently severe event to trigger a deployment, such as an impact, rollover event, or G-force threshold breach. The Petition further alleged that the Diagnostic Trouble Codes (DTCs) and Supplemental Restraint System (SRS) conflict and inaccurate data is being recorded for the air bag system. An air bag is a supplemental vehicle occupant restraint system that deploys a cushion designed to inflate in milliseconds during a collision. The cushion prevents vehicle occupants from striking hard interior surfaces and, in some cases, exterior surfaces. Air bags are designed to work in conjunction with seat belts, the primary occupant restraint system. Seat belts restrain occupants keeping them in place for the air bag to provide proper protection and to mitigate injury from air bag deployment. An air bag system consists of an air bag cushion, an inflator module, an air bag Electronic Control Unit (ECU), and impact sensors. When a crash begins, impact sensors located on the vehicle exterior begin to measure impact severity. If a crash is severe enough, the sensors send a signal to the system’s ECU, which then signals the inflator to fill the air bag. ODI reviewed 9 Vehicle Owner Questionnaires (VOQs), one Early Warning Reporting (EWR) Death & Injury (D&I) report, and several TREAD (Transportation Recall Enhancement, Accountability, and Documentation) Field Reports alleging inadvertent air bag deployment in the Subject Vehicles. Several complaints allege that the inadvertent deployment of air bags occurred while traveling over potholes, road debris, or during minor curb strikes. All the complaints allege that there was no warning prior to the inadvertent deployment. In general, different types of air bags are designed for different types of impacts. For instance, frontal air bags are generally designed to deploy during frontal impacts; side curtain air bags are generally designed to deploy both during rollovers and impacts to the sides of the vehicle; and side thorax air bags are generally designed to deploy during impacts to the sides of the vehicle. While these are broad categories of crash scenarios, the specific conditions under which deployment is intended are multifaceted and dependent on complex algorithms. Differences in particular variables can make a deployment an intended part of the design in one scenario and not in another similar scenario. Although the reports described above all contain allegations of inadvertent deployment, the conditions under which those reported deployments occurred differ widely. The reports ranged different types of air bags, different combinations of air bags, and different driving scenarios preceding the deployment. Based on the data available to ODI, there is no clear pattern or factor to prompt the alleged inadvertent deployments. Thus, ODI’s review did not identify a trend of a vehicle defect regarding any type of air bag in the Subject Vehicles. Moreover, ODI did not identify any reports of inadvertent air bag deployment that resulted in a loss of vehicle control. ODI is likewise not aware of any crashes or severe injuries involving the Subject Vehicles that resulted from inadvertent deployment of the air bags. ODI recognizes that inadvertent air bag deployments can create dangerous safety hazards. For instance, an inadvertent deployment can surprise a driver, leading to a loss of vehicle control; or if an inadvertent deployment occurs when an occupant is out of position, the force of the deployment may injure the occupant. Inadvertent deployments have been the basis for past NHTSA recalls and investigations. Here, the Petition cites one such recall: Recall 26V-227, which concerns inadvertent deployment of the side curtain and thoracic air bags in MY 2018 to 2022 Honda Odyssey vehicles (the “Recalled Vehicles”) due to the air bag ECU containing incorrect deployment parameters. Although this recall involved Honda Odyssey vehicles in adjacent model years to the Odyssey vehicles that are the subject of this Petition, ODI understands certain key differences to exist between the two groups of Odyssey models. Honda introduced a new generation of the Odyssey line in 2018. This new generation involved a large-scale redesign and a new vehicle platform compared to the previous model years. As a result, the vehicles covered by the Petition differ from the recalled vehicles in many characteristics, including physical dimensions, engines, transmissions, and air bag ECU algorithms. Given these differences, ODI’s review has not identified evidence that the reports of inadvertent deployment in the Odyssey models covered by the Petition correlate to Recall 26V-227. After thoroughly assessing the material submitted by the Petitioner, consumer complaint information in NHTSA’s databases, and other relevant information already in possession of the Agency, NHTSA has not found sufficient evidence to indicate that a trend of inadvertent air bag deployments as alleged by the Petitioner is likely to present a safety related defect that would warrant a formal investigation. After full consideration of the available information the Petition is denied. A denial report will be posted in the Federal Register. The closing of the Petition does not constitute a finding by NHTSA that a safety related defect does not exist, and the agency reserves the right to take additional action if warranted by future circumstances. The Petition can be viewed at NHTSA.gov under ODI Number 11744118.
