An explosion of an inflator within the driver frontal air bag module may result in sharp metal fragments striking the driver, front seat passenger or other occupants resulting in serious injury or death.
Remedy
Honda will notify owners, and dealers will replace the driver's air bag inflator with an alternate inflator, free of charge. The recall began April 10, 2019. Honda owners may contact customer service at 1-888-234-2138. Honda's number for this recall is O41. Acura owners may contact customer service at 1-888-234-2138. Acura's number for this recall is U40.

Honda will notify owners, and dealers will replace the passenger air bag inflator, free of charge. The recall began on July 1, 2015. Owners may contact Honda customer service at 1-888-234-2138. Honda's numbers for this recall are JU4, JU5, JU6, and JU7. Note: Vehicles that have already received a replacement passenger air bag inflator as part of the recall remedy for an earlier campaign such as 14V353 or 14V700 (Honda recall numbers S95, JH6, JH7, JH8, JH9, JJ0, JJ1, JJ2, JJ6, JM5 and JM6) are not included in this recall. However, this recall does supersede 13V132 and 14V349. Note: Vehicles that have had their driver frontal air bag replaced previously as part of a recall remedy prior to September 12, 2014 need to have their air bag replaced under this recall as well. Honda is recalling certain model year 2003 and 2004 Pilot and Odyssey and 2003 through 2006 Acura MDX passenger vehicles manufactured from November 26, 2001, through August 30, 2006. The interlock lever of the ignition switch may deform, which can allow the interlock function of a vehicle with an automatic transmission to be defeated.

Removal of the ignition key when the gear selector of a vehicle with an automatic transmission has not been shifted to the park position can allow the vehicle to roll away, increasing the risk of a crash.
Remedy
Honda will notify owners and instruct them to take their vehicle to a Honda or Acura dealer. The dealer will install an updated shift interlock lever and, if necessary, replace any necessary part of the ignition switch, free of charge. The recall began on February 22, 2013. If the terminal ends melt within the wiring harness connector, the low beam headlights may become inoperative, reducing driver visibility and increasing the risk of a crash.

Honda will notify owners, and dealers will repair the low beam terminal harness or replace the lighting switch as necessary, free of charge. the original recall began in april 2012. owners of vehicles involved in the september 2012 expansion will be notified in september 2012. owners may contact honda customer service at 1-800-999-1009.
Honda IS recalling 2,032 hitch style bike racks sold as aftermarket for use on my 2003-2007 and some my 2008 honda pilot, ridgeline, and acura MDX vehicles. a pinch bolt exists between the bike arm bracket and the upright assembly of the "hitch" style bicycle rack.

When moving the bike arm bracket to its down position, this pinch bolt has the potential of causing severe laceration or amputation to the user's fingertips or hand.

On some MINI vans, sport utility and passenger vehicles, certain operating conditions can result in heat build-up between the countershaft and secondary shaft second gears in the automatic transmission, eventually leading to gear tooth chipping or gear breakage.

Gear failure could result in transmission lockup, which could result in a crash.
Remedy
On vehicles with 15,000 miles or less, the dealer will update the transmission with a simple revision to the oil cooler return line to increase lubrication to the second gear. on vehicles with more than 15,000 miles, the dealer will inspect the transmission to identify gears that have already experienced discoloration due to overheating. if discoloration exists, the transmission will be replaced if discoloration IS not present, the dealer will perform the revision to the oil cooler return line. the recall began on april 21, 2004, for pilot, odyssey, and MDX owners. owners of the accord vehicles will start receiving letters on june 28, 2004, and on june 29, 2004, for owners of the TL and cl vehicles. owners should contact honda at 1-800-999-1009 or acura at 1-800-382-2238.
On certain minivans, sedans, coupes, and sport utility vehicles equipped with V6 engines, a timing belt tensioner pulley on the water pump IS misaligned and could cause the timing belt to contact a bolt on the cylinder head. eventually the belt could be damaged and fail.

If the timing belt breaks, the engine will stall, increasing the risk of a crash.
Remedy
Dealers will inspect the water pump and if it IS one of the defective pumps, the water pump and timing belt will be replaced. owner notification began august 30, 2002. owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact honda at 1-800-999-1009 or acura at 1-800-382-2238.

I am the original owner of my 2003 Honda pilot and had to replace the front driver side door actuator years ago. Now all the remaining 3 doors need to have the door actuators replaced. If you're outside the car you cannot open the door. Using the key remote or putting the key into the front door keyhole will NOT unlock or lock the door. You have to be inside the car and lift or depress the lock button to get them to open or to lock it. This is a very dangerous situation for if an emergency should occur and it's necessary to either exit the vehicle or enter you won't be able to. I know the later year Honda's has had recalls on door actuators.

Reporting an important safety issue I experienced due to random engine stalling on my 2003 Honda Pilot, typically after driving for a few minutes. The engine would crank for several seconds or sometimes start immediately after stalling, but no diagnostic trouble codes were ever stored. The vehicle had stable fuel trims and idle, and multiple components including the EGR valve and valve clearance were serviced without resolving the issue. Ultimately, the root cause was traced to a failing PGM-FI (fuel pump) relay located under the driver’s dash. After replacing the relay close to two months ago, the stalling stopped completely. This issue could pose a safety risk if the engine stalls in traffic, especially in a hazardous area where there’s no breakdown lane (construction zone, etc.) , and I believe this factor should consider this a known failure point in aging vehicles of this model. Attached are two images of the new relay zip tied under the driver’s footwell (could not remove the bad relay since it was too difficult to remove from its bracket).

Water leaked into front floors of vehicle soaking the carpet and the fuse panels in the floor. Yes it is available. Fuse boxes had standing water building up behind them slowly dumping water into them causing shorts in electrical system which could have caused the car to catch on fire. Yes the problem has been confirmed. No the vehicle has not been inspected by anyone other than mechanic. The only signs of the problem I noticed was the carpet was soaked and couldn't understand why. The battery died randomly one night after locking the doors. No other signs were noted.

severe erosion of my undecarriage, metal just falls off from under the car. My fuel tank leaks and gas pours out. It has been deemed unsafe to drive by my Honda dealer, but has a good engine and other parts. It has been severely impacted by rust and erosion. The previous owner was a resident of Chicago, where salt is used, which I believe influenced this defect.

My 2003 Honda pilot rear subframe suspension is rusting and causing control issues. This is a common problem on the 2003-2008 Honda Pilots. In Honda TSB 21-019 Honda states " The subframe mounts may pull away from the body of the vehicle (see images below), but extensive testing has concluded that vehicle stability will be maintained." This is NOT true. Other Honda's have experienced similar issues and Honda has issued a safety recall. There should be a safety recall for the 2003-2008 Honda pilots for this subframe rust issue. I’ve left numerous messages and still no response I gave a 1 year old daughter, we almost got hit by a car and if it had hit us something could’ve happened! Because I had to swerve off road my suspension and power steering are messed up because of pot holes,Just so I wouldn’t hit and impact the passenger side of me my car where airbag is bad and nothing would’ve happened to us, not that it was our fault the driver was driving almost right into us!I blame Honda for this disaster would could have been very bad! Of course the other driver drove away like nothing happened! So now I am stuck at home not able to go to work and bring my daughter to doctors appointments and invade of in emergency my vehicle is not safe enough to get us anywhere! I am asking god to bless us in this case and count all the blessings he has given us! I don’t have a lot of money to pay for repairs and this is the only car I’ve ever purchased and owned. Both rear axel mounts rusted away from the subframe while being driven by my teenage driver. This caused the tires to lean inward and caused mounts to rest on the muffler system. The vehicle is available for inspection, but has not been inspected since it is not mobile or safe to drive. The safety of my child and others were at risk, as this failure could have caused an accident. There were not warning lamps or messages, but rust was noticeable underneath the vehicle.

Attempting to put transmission into reverse caused similar slippage. Returning on Hwy 321 toward Dallas, north carolina, the transmission slipped and the car would not reach highway speed. Turning onto Hwy 279 to go home, the Honda would not go into gear at first, then slippage so that other cars almost struck our car which was unreliably stumbling. From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. This investigation was prompted by Preliminary Evaluation (PE) 11-017, and was based on reports from consumers alleging that both low beam headlights stopped illuminating simultaneously.Some complainants also reported that the combination switch (switch) that controls the headlights and turn signals, and/or its electrical wiring harness connector were damaged by overheating.ODI's PE information request letter asked Honda for certain data on model years (MY) 2002-2006 CR-V, 2003-2008 Pilot, 2003-2008 Element, 2001-2005 Civic, and 2000-2001 S 2000 vehicles.Analysis of complaint and Honda data show that a complete low beam headlight circuit failure occurs in models with 2-bulb headlight systems (i.e., a single bulb on each side for high and low beam).Models with 2-bulb systems include MY 2002-2004 CR-V, 2003-2005 Pilot, 2001-2003 Civic, and 2003-2008 Element.Models with 4-bulb headlight systems (i.e., two bulbs on each side, one for high and one for low beam) have different electrical circuits for the headlights and do not experience the same failure mechanism as the 2-bulb system.MY 2000-2001 S 2000 models with 2-bulb high intensity discharge headlights also have different electrical circuits for the headlights and do not experience the same failure mechanism.Honda introduced the 4-bulb headlight system in CR-V in MY 2005, in Pilot in MY 2006, and in Civic in MY 2004.On March 29, 2012 Honda submitted a defect information report (DIR) to recall 554,428 vehicles, including MY 2002-2004 CR-V and MY 2003 Pilot.In the DIR, Honda stated that tension from the combination switch wiring harness to the electrical terminal for the low beam headlight circuit could cause a small amount of motion as the combination switch was operated.This motion, over time, can wear the surface of the terminal creating copper oxide, increasing electrical resistance, which may cause the terminal to melt within the harness connector.ODI continued its investigation, and on August 31, 2012 Honda submitted another DIR to recall an additional 820,789 vehicles.This report included the MY 2002 Civic vehicles which were not subject to a prior safety recall (04V-086) for headlights, all of MY 2003 Civic, and MY 2004-2005 Pilot.Honda's DIR advised that operation of the turn signal switch lever at high speed and/or with heavy force can cause movement between the combination switch body and the sub-harness wiring connector.This can cause wear and continued movement can create copper oxide, increasing electrical resistance, and potentially causing the terminal to melt within the harness connector. Please see both DIRs at http://www-odi.nhtsa.dot.gov/recalls/ under safety recall 12V-136 for complete details.Honda also provided additional information on the Element model, stating that it was not recalled due to it's very low failure rate.Honda sold 301,497 Element vehicles in the United States and received 17 reports, while ODI received one report (see VOQ 10395219).Honda's technical explanation for the low rate, which was submitted under request for confidentiality, is that the headlight switch, as installed in the Element, reacts differently to the forces applied to it as compared to the recalled models.See attachment for additional information.
