Vehicle description: MINI vans equipped with 2-spoke luxury wheels manufactured july 1992 through june 1993; or equipped with 4-spoke sport wheels manufactured from july 1992 through september 1993. the steering wheel armature stamping can crack and separate from the center hub attachment to the steering column.
If the steering wheel separates from the center hub attachment to the steering column, loss of control can occur increasing the risk of an accident.
Dealers will inspect the steering wheel for cracks and any wheels with cracks will be replaced. all other vehicles will have a reinforcement plate installed to prevent steering wheel separation in the event cracking occurs.
Vehicle description: MINI vans. the bolts used to attach the strut to the body and the rear liftgate can accumulate fatigue damage if three factors combine. if the bolt IS loose, the gas strut IS over-pressurized, and the frequency of opening and closing cycles IS higher than for the average customer, the bolt can fail.
Personal injury could occur if the bolts break allowing the liftgate to fall suddenly.
Dealers will inspect for loose fasteners, if the fasteners are loose, the strut (and integral bolts) will be replaced. if the bolt shows evidence of torque, the bolt will be removed, larger washers will replace the existing washer and the bolt will be reinstalled and hand tightened to the target torqued.
Vehicle description: MINI vans. the wiring that initiates the driver and/or passenger air bag could electrically short circuit to ground. a short circuit to ground that exists immediately after turning the ignition key to the "on" or "start" position can cause the air bag(s) to inadvertently deploy.
Inadvertent air bag deployment can injure a front seat occupant.
Dealers will perform an electronic diagnostic check. any short circuits identified in either the driver's or passenger side air bag initiator wire circuit will be repaired.
Vehicle description: light duty passenger minivans without remote release. this IS not a safety recall. there has been no determination that a safety defect exists with the minivan latches. chrysler IS conducting a service campaign to replace the liftgate latch assembly of the affected minivans. in a collision, the rear liftgate latch assembly could…
If the liftgate latch opens, occupants can be ejected resulting in significantly increased risk of injury or death.
Chrysler dealers will replace the liftgate latch with a new stronger latch assembly without charge. chrysler began notifying owners of these vehicles beginning september 1995.

I bought my van on 03/02/2012. It was inspected on 03/03/2012. On 03/08/2012 I was driving and it was all over the road and very hard to control. I stopped at monroe muffler and had the tires checked, they were fine. I made an appointment to have the van totally checked on 03/12/2012. I was told I had a bad tie rod and ball joint and that they should not have passed inspection. I have tried several times to contact the dealer and he now seems to be avoiding me. I have four kids and need the van fixed and safe asap. *tr
Drivers reclining seat broke. replaced seat with a used seat and it has failed also. *tr
TL* the contact owns a 1994 plymouth voyager. While parked, the contact noticed that the rear passenger side sliding door would not close. The vehicle was not taken to the dealer for diagnostic testing and was not repaired. The approximate failure mileage was 145,000.
The #16 fuse- ignition feed was blowing intermittently but now does it as soon as you turn the key on run position. Short cannot be found please help me. I am on fixed income and cant afford any more expenses after towing it 3 times in one week. *Tr
Tl*the contact owns a 1994 plymouth voyager. while closing the lift gate, the lift bar failed and shattered the rear window. the contact sustained cuts and bruises on his hands. the insurance inspector stated that the cause was due to a mechanical failure. recall # 99v213000 (structure:body:hatchback/liftgate:hinge and attachments) was found. the current and failure mileages were 114,000.
1994 voyager tire blew out. *ts the consumer lost control of the vehicle and struck a construction sign. the vehicle sustain damage to its hood, passenger door, roof, right front tire and right rear tire. *nm
No known events other than use and rust lead to failure.. the problem was discovered when passenger side rear tire began rubbing wheel well. please help, both leaf springs snapped/broke on our 1994 voyager! please be advised, be on watch for any other dodge/plymouth vans doing this, could cause dangerous loss of control and or accident/injury! this should not have happened! Warning! this part # 80556pn/1504* main leaf springs IS defective and amazingly broke completely into at the front mounts. I have many pictures to send. please recall asap. vin: rr809177 mileage: 184000 I am very concerned that this could be just the "tip of the iceberg" and many vans could have the same problem with disastrous results if leaf springs break at highway speed! you folks need to get after this asap! I got a good used 95 whole axle with leafs and had the old axle replaced with the used one. Old parts/leaf springs with old axle have been saved and are available. *jb
Dt*: the contact stated the vehicle was purchased two weeks ago and the transmission failed. the transmission began to make a banging noise while driving at 50 mph. When the vehicle was moved off the road and placed into park, the gear position indicator shifted out of park . However, the vehicle would not move in either direction. the dealership was contacted, and they determined the transmission needed to be replaced. according to the contact this transmission was notorious for its high rate of failure and poor quality. the contact felt the seller knew about these problems and failed to solve them. The consumer checked the internet and discovered there were over 1,000 irate people who had similar transmission problems. The manufacturer should acknowledge the problem and issue a recall. updated on 05/30/2006. *Ak
Dear sir/madam: safety issue: my plymouth van caught fire somewhere in the lower dashboard on driver side. We had just driven the car and parked it, when minutes later the horn sounded and we say smoke and flames. The fire department was called. The car doors locked themselves, one front light went on, rear tail light was on during fire. Fortunately no one was in the van. Why did the fire start and why did the van doors lock. this IS a real safety issue. Please send a reply as to what you plan to do about this safety issue. thank you.
Dt: the caller said that while driving transmission locked. He said that at 55 or 60 mph and without warning the transmission l locked in first or second gear. The caller's vehicle lost speed and had to get off the road. He drove the vehicle at slow speeds to get to the dealer. Dealer and manufacturer offered no help for free.*ak
Dt: the contact stated the parking brake will inadvertently release after locking it. This has been happening for over 4-5 years. It has gotten progressively worse over the years. It will click as if it was locked, then it will jerk. It will make a very loud popping noise before it jerked. The dealer told him the car was too old. The manufacturer said it was out of warranty. *Ak *jb
The head lights become cloudy because of ultraviolet damage to the plastic. The plastic needs ultraviolet protection, like our eyeglasses have ultraviolet protection. The headlight develop cataracts, literally, becoming cloudy and very difficult to see clearly at night. I have to drive very slowly. I have found maguire's pastx does help for a few months and then the headlamps or lenses become cloudy again. Of course each time I use this plastic cleaner and polisher, I am diminishing what little uv might be left in the plastic. This cloudiness of the plastic also IS bad enough to prevent the passing of state inspections in pennsylvania - it's regarded as a possible hazard obviously according to the state inspection. *jb
Driver's seat suddenly reclined to full back position without warning. after returning seat to upright position, found that seat recline mechanism does not secure seat, moderate pressure on the seat back causes it to suddenly recline. *jb
The liftgate support bracket recall 99v210000 remedy failed. This resulted in the liftgate opening while driving.*ak
Brake line failure, resulting no brake pressure to stop vehicle was able to stop vehicle with emergency brake 2ND time in 2 years parts replaced at garage. ak
The ABS braking system malfunctioned. As a result, when applying the brakes a squeaking noise was heard. Also, a hesitation before the brakes worked. *Ak
Nar 06/19/2003. *mr the consumer was involved in an accident and the vehicle was totaled. The car was occupied with two airbags and at the time of the accident none of the airbags worked. *cb *jb
Please be informed of a potential auto fire defect on a 1994 plymouth voyager van with a 3.3 engine.. cause: \tuse of a metal cable restraint cleat that holds the high amperage battery cable harnes, next to the alternator. Cleat IS located before the main battery fuse link, condition agrivated with thermal warming, located above the exhaust manifold. Cleat cuts into the high amperage cable and ignites a fire in the cable harnes. Burning harnes IS located directly below a flexable fuel line. It may not be possible to extinguish the resulting fire without removal of the battery clamp, as the fuse link affords no protection and the fire will re-ignite. Fault discovery: owner observed fire soon after ignition and used a fire extinguisher to put out the flames, but the cable continued to incandess until the battery clamp was removed. Followup examination of the recovered metal cleat showed indications of arc-burning. failure mode: the failure mode may explain the cause of other incidents where the vehicle was either parked unattended or being driven at higher speed, and evidence of cause could not be determined due to more extensive fire loss.
I have been working on this peeling paint problem for two years. I discussed this with the dealership several times - they offered to give me a free estimate for a new paint job! I have a chrysler case #9230347. The zone rep has looked at the van and offered to assist with repainting. I was told to get an estimate from maaco which was $1400 to repaint the roof and hood. He offered to pay $400. I decided to take them to small claims court and had another estimate to repaint the entire van for $3040.79. We went for our pretrial conference on june 10, 2003. I was told I need to prove that this dealership knew in 1994 that the paint would fail... I can't find a service bulletin. Better business bureau only keeps records for 3 yrs. we went to mediation - me, the lawyer from the dealership, and the zone rep for chrysler (bill sedick). The lawyer was a stereotypical sleaze... He said the statue of limitations was 6 yrs and this case should never have been brought to court. He tried to get the judge to throw it out before we even went to mediation. Then he threatened to make me pay thousands of dollars in legal fees... I dismissed the case against the dealership because I learned several months ago that I shouldn't have gone that route anyway. But I hope that sleazy lawyer doesn't think it was because of him... I told him he wasn't telling the truth - that there was no statue of limitations under the magnuson moss fed. Law... He was such a creep - he even made the court clerk mad because he said their dismissal form was bogus. Then the chrysler guy offered $1,500.00 for a paint job. He even apologized for the lawyer guy. I'm supposed to go to a paint shop for an estimate and mr. Sedick said he'd work with them and me to get the van repainted. He was pretty nice actually. He said he didn't want a dissatisfied customer and he hoped I would do business with them again - I said probably not...anyway thanks for your help.

NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
Mr. Larry a. Sackey submitted a petition to the national highway traffic safety administration (nhtsa) by letter dated april 18, 2003, requesting that nhtsafurther investigate the defective collapsible steering shaft system on 1987-1995 vehicles manufactured by the daimlerchrysler corporation.in part because of 49 cfr ? 30120(G), nhtsa has decided to limit the scope of the petition vehicles to 1993-1995 models, excluding the 1993 jeep grand cherokee and 1994-1995 dodge RAM series trucks that were recalled (93v210 & 96v230) for steering column failure.if the analysis of these vehicles had identified a potential problem, the scope could have been expanded in an investigation.a review of odi's data base shows no recent complaints and a lack of a complaint trend concerning the subject issue on the subject vehicles as a whole or by individual models.therefore, the petition IS denied.for details, see federal register notice volume 68, no. 130, july 8, 2003.

Cfas sent a follow-up letter on december 12, 1996, to change the intent of the petition to airbag aggressivity and deployment variability.nhtsa received the letter on 12/12/96; accordingly, the petition due day was changed to april 12, 1997.cfas petitioned the agency to conduct an investigation to determine whether the airbag crash sensor and/or deployment systems for the 1991-92 chevrolet corsica/beretta, 1990-92 ford taurus, and 1994-96 chrysler minivans are defective.cfas allege that information on low speed crashes here occupants have suffered fatal injuries suggests [that the above vehicles] have defective airbag crash sensor and/or deployment systems.

Liftgate latches in the subject vehicles have failed in low and moderate speed impacts resulting in occupant ejections, injuries, and fatalities.there are two failure modes associated with latch failure: fork bolt-detent lever bypass, and inertial unlatching.nhtsa analyses of chrysler and peer minivans took the form of static and dynamic crash testing, engineering evaluations, complaint and field analyses, and real world data analyses.a technical report discussing these analyses can be found in the engineering analysis technical report in the public viewing file.chrysler has advised nhtsa that it will conduct a campaign to replace the latches on all the subject vehicles.the chrysler campaign will consist of notification letters to all owners; free replacement of all latches with stronger, safer latches regardless of the age of the minivans; national advertising to promote customer awareness of the campaign; quarterly reports regarding campaign completion rates; as well as additional measures to ensure a high completion rate, above and beyond the actions that normally would be taken in a formal safety recall.these include follow up written and telephonic notifications to owners who do not bring their vehicles in for corrective action.nhtsa firmly believes that the crash performance of the latches on these minivans raises serious safety concerns, and that the campaign addresses these concerns.the chrysler corrective action will increase the strength of the liftgate latches and decrease the potential for inertial unlatching, thereby reducing the risk of crash-related openings of the liftgate and possible occupant ejection through the liftgate opening.owners of chrysler minivans will get a stronger, safer latch free of charge at the earliest possible time, without the need for further technical investigation and complex and time-consuming litigation with an uncertain outcome.based on the above, odi IS closing this investigation.
