Toyota IS recalling certain model year 2004-2010 passenger vehicles. the accelerator pedal can get stuck in the wide open position due to its being trapped by an unsecured or incompatible driver's floor mat.
A stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.
Toyota filed an amended defect report on november 25, 2009, stating that dealers will modify the accelerator pedal and, on certain vehicles, alter the shape of the floor surface under the pedal. these changes address the risk of pedal entrapment due to interference with the floor mat. redesigned accelerator pedals will become available beginning in april 2010 and dealers will replace any modified pedal with the new pedal if desired. also, dealers will replace any genuine toyota or lexus all-weather floor mats with redesigned all-weather mats, or repurchase the previous mats from owners who do not want the new ones. additionally, software modifications will be installed on camry, avalon and lexus ES 350, IS 350 and IS 250 models that will ensure that the brake overrides the accelerator in the event both brake and accelerator pedals are applied. toyota will begin mailing letters to owners on december 31, 2009. owners may contact toyota at 1-800-331-4331, lexus at 1-800-255-3987.

The cause of the failure was not yet determined. The local dealer and manufacturer were not notified of the failure. The approximate failure mileage was 131,000.
My dashboard IS melting, since it IS melting it IS causing the dashboard to be really shiney, which IS causing a huge reflection on my windshield, which IS making it hard to see out. The dashboard IS also really sticky from melting, and im also concerned about the airbags because of this.
Re: tsb-l-sb-0144-11 sticky/melting dashboard the car IS 6 months beyond the date of the warranty enhancement period. Since the condition of shiny stickiness of the dashboard did not come to my attention until after the enhancement period, I believe that I am nevertheless entitled to the repair of the dashboard by toyota and that coverage should be also extended to the replacement of certain ancillary parts related to the replacement that might be damaged as a result of the warranty repair. There IS no valid reason that the warranty enhancement should have expired as this condition IS dangerous and needs to be corrected on all affected vehicles regardless of an arbitrary period set by toyota. *js
Melting dashboard and driver's side visor does not stay up. The melting dash causes a glare on the windshield causing visibility to be obstucted. I was told that I missed the warranty period but have never been sent anything about this problem.
TL* the contact owns a 2009 toyota camry hybrid. While driving 45 mph and depressing the brake pedal, the vehicle failed to stop. The contact shifted into low gear and was able to coast the vehicle to the side of the road. In addition, the ABS, traction control, and brake warning indicators illuminated. The vehicle was towed to the contact's residence. The vehicle was not taken to a dealer or an independent mechanic for diagnostic testing. The vehicle was not repaired. The manufacturer was notified of the failure and did not assist. The approximate failure mileage was 137,000.
ABS and traction control are on the vehicle slips when taking of the vcs stability IS on it's very scary to drive the vehicle.
The rubber button to open the trunk melted, down to the rear bumper of my car (black).
The hand brake light keeps coming on and off while driving. The car feels like it's coasting and have no traction control. All of the sudden, all lights turn on dashboard and check vsc system comes on, brakes are hard to engage and steering control IS difficult. Car drives like that for about 5 minutes and then all goes back to normal. Few days later it starts up again with the break lights coming on and off. It happens during different driving conditions: straight city street, while turning, going down a hill or attempting to come to a stop. This makes it really scary as I am in a very dangerous situation when I am trying to break and the break do not engage and steering control IS affected. I have taken it to mechanic after the incidents and they cannot diagnose the issue and there are no failure codes. However, the issue keeps going on making me terrified to drive the car.
My vsc light IS on along with the ABS and the brake light. While taking a 1 1/2 hour tript the light went off and stayed off for a few days. While taking another highway trip traveling the speed limit, the light was off until after driving for two hours I stopped at a restaurant. The lights came back on, and have been on ever since.
Vsc system keeping appearing on the dash board
On my 2009 camry hybrid, as I was going to work, when I tried to slow down the car the brakes went hard and can't stop the car totally I saw the brake light, traction control, and ABS lights turned on, and check vsc system displayed. I immediately slow down the car took the exit and stop the car. I turned off the car and start the car again which fixed all the above issues. it happened approx two weeks ago and took the car to toyota of grapevine tx 76051 and they did not find any issue related to brake or any code. I am scared to drive the car and taking the car to the same dealer to check again.
I was driving on the interstate (395N into dc) and the car's brakes failed. The car immediately displayed vehicle control system (vcs) failure and a number of other warnings. I was forced to swerve out of traffic and to utilize the emergency brake to stop the car.
Brake comes on, ABS light comes on and it becomes difficult to brake (brakes are hard). then after an hour or so the lights go off and brakes return to normal. I noticed that when it returns to normal there IS a heavy bump, it actually feels like someone bumped you from the rear. this has happened off and on for about 3 months now. I took it to my mechanic thinking it was something minor, pads etc. And it was not that. on 11/28/18 I took it to the dealership. they found nothing wrong. not sure what else can be done. this IS very frustrating and potentially dangerous. because this problem can happen at any time with no warning, it has happened while driving and also starting the vehicle. my personal mechanic did find some information that may pertain to this problem (warranty enhancement program - ZE1 2007 - 2011 model year camry hybrid vehicles extension of warranty coverage for brake actuator assembly and skid control ecu/break pedal sensor) service campaign bulletin ref# t-cp-ze1, date of issue july 1, 2014.
When I driving back yesterday jan 1ST 2019 from friends house suddenly the check vsc system turned on and other icons turned on in the car dashboard. I tried to press the brakes at a stop sign and they do not engage or they would engage on a delay, vehicle took long time to stop. It was hard driving back and I was scared of an accident happening even after turning on emergency blinkers on my drive back home. Toyota needs to investigate it as it IS a safety issue.
TL* the contact owns a 2009 toyota camry hybrid. While driving approximatey 10 mph and attempting to stop, the vehicle would not stop or slow down when depressing the brake pedal. During the failure, several warning messages and indicators displayed and illuminated including "check vsc system" (vehicle stability control), ABS, master warning, and brake failure. When the contact turned off and restarted the vehicle, it operated normally. The vehicle was taken to passport toyota (5001 auth way, suitland, md) to be diagnosed, but the failure could not be duplicated. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 106,571.
While driving straight at about 40 mph (in a 50 mph zone on a back road) I stepped on the brakes when approaching a stop sign. The brakes failed to engage. The check vbs system warning came on as well as all of the dash board warning lights. The car was checked and the brake pads were fine, the brake fluid level was normal, and no computer codes came up when that was checked. The brake light keeps coming on and off and the brakes work most of the time, but out of no where they won't engage. It was very scary, especially because my kids were in the car. Thank goodness I wasn't going faster than I was because I don't think we would have stopped in time.
Brake, ABS and warning lights came on, check vsc message came on and my brakes stopped working. I was leaving my parking lot and got halfway down the block when this occurred. I could not brake so I let the car roll to a stop and used the curb to make a complete stop. Car I had not moved since I drove it really slow to house.
While I was driving at about 65 mph on a four lane divided highway, the driver's visor suddenly and unexpectedly rotated completely down, blocking my view out of the windshield so that I could not see ahead. It was a dangerous and frightening situation! I had to keep control of the car while holding the visor up so that I could get home safely and then get the car to the repair facility. toyota knew that this safety hazard existed and was serious because it did a "warranty enhancement notice" for 2007 to 2011 camry hybrid sun visor brackets, but this expired for my car in may of 2016.
TL* the contact owns a 2009 toyota camry hybrid. While driving approximately 30 mph, the ABS warning indicator illuminated. The vehicle was taken to the dealer (autocentro toyota in san juan, puerto rico, 787-999-9115) where it was determined that the vehicle had a leak in the ABS. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 165,000.
Driver side sun visor falls down unexpectedly. will not stay in place except for full down position. this has been since the car was new. was searching for how to fix it and found link to here. I understand that this IS a common problem with all toyota visors and IS due to bad design and poor materials. toyota does not stand behind it and the replacement visors are the same junk as original.
My dashboard IS melting. When it solidifies it becomes extremely reflective and obstructs the windshield view.
The check vsc system was displayed in my totoya camry 2009 hybrid, this morning. The warning, ABS, brakes, parking brake and skid control lights have turned on. From test driving the vehicle, it appears that the brakes seem less responsive.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation. After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first. In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles. The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims. The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness. In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that a safety-related defect does not exist. For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
