TAKATA RECALL PARTS DELAY - MANUFACTURER REPAIR FAILURE.I am filing this complaint because Chrysler/FCA and its authorized dealer network have failed to provide the necessary remedy parts for an active, critical safety recall within a reasonable timeframe. My 2004 Ram 1500 is subject to the urgent Takata airbag recall. I contacted the manufacturer's official recall helpline and multiple local authorized dealerships to schedule the repair. I was formally informed by all parties that they are experiencing severe supply issues sourcing the required replacement parts. The last communication I received from the manufacturer regarding this issue was on July 1st, 2026. Since that date, there has been a total lack of communication, updates, or a timeline for when these safety-critical parts will become available.Because of this parts shortage, I was forced to park my truck completely idle for over two consecutive months, as the vehicle could not pass its required state safety and emissions inspection without the recall being resolved. This supply chain failure has completely stripped me of the use of my vehicle, creating a severe logistical hardship while leaving an unaddressed, potentially lethal explosive airbag inflator in my vehicle. I am requesting that NHTSA investigate FCA/Chrysler's compliance with their legal obligation to supply safety recall remedy parts in a timely manner.
The contact owns a 2004 Dodge Ram 1500. The contact stated that the vehicle failed an inspection because the VIN was included in NHTSA Campaign Numbers: 16V352000 (Air Bags) and 15V313000 (Air Bags); however, the parts to do the recall repairs were not yet available. Four dealers were contacted and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The manufacturer was notified of the issue, but no assistance was provided. The contact had not experienced a failure.
Vehicle: 2004 Dodge Ram 1500 I was the driver and sole occupant of my 2004 Dodge Ram 1500 and was wearing my seat belt. I was traveling westbound on [XXX], shortly after passing [XXX], when I was involved in a three-vehicle collision. The first vehicle stopped abruptly, resulting in the collision. I was traveling approximately 50–55 mph before the collision, and my truck sustained a significant frontal impact. Despite the frontal impact, the driver’s frontal airbag did not deploy. There were no passengers in the vehicle. The component/system of concern is the driver’s frontal airbag and Supplemental Restraint System (SRS). Prior to the collision, I was not aware of any airbag warning lights, warning messages, or other symptoms indicating that the system was malfunctioning. The exact reason for the airbag non-deployment is currently UNKNOWN. The vehicle has not yet been inspected or altered to determine the cause. The truck is currently secured at my residence in its post-collision condition. It has not been repaired, dismantled, modified, salvaged, or otherwise disturbed since the collision and is available for inspection upon request. I am reporting this incident because the driver’s frontal airbag failed to deploy during a significant frontal collision. I am requesting that the airbag non-deployment be documented and investigated to determine whether a defect or malfunction in the airbag/SRS system contributed to the failure to deploy. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
I am the owner of a Dodge Ram truck that is currently under an active and urgent Stellantis "Stop-Drive" safety warning due to the defective Takata airbag inflator recall. I am writing to file an official complaint regarding the manufacturer's failure to provide a remedy within a reasonable timeframe, as required by federal law.I contacted an authorized dealership to schedule the recall repair. I was initially told the wait for the replacement part would be two weeks. It has now been over three months, and the dealership still has not received or provided the necessary repair parts.Because of the official "Stop-Drive" order issued by the manufacturer, I cannot legally, safely, or responsibly drive my vehicle. However, I am still forced to pay ongoing auto insurance premiums for a truck that has been rendered completely unusable for a quarter of a year due to this parts shortage. The manufacturer is failing its statutory obligation to remedy this critical safety defect in a timely manner, and they have not proactively provided me with alternative transportation or a mobile repair solution to resolve this dangerous situation.

Louisville Chrysler Dodge Jeep Ram. 5311 Dixie Hwy, Louisville, KY 40216. I took my truck into this dealership to have the airbags replaced due to a recall and to have a keyless entry remote programmed. The service tech told me it was going to take a day or two to get the airbags replaced and warned me about possibly breaking my dashboard (without replacement or reimbursement) due to the known cracking dashboards on this truck. I told them to be careful (as the dashboard was in pristine condition) and go ahead with the programming and airbag recall. This was my only ride as I live an hour away, and they did not have a courtesy car for me to use, so they ended up calling an Uber to get me home. A few hours later, I got a text from the service tech saying everything was done and I could come pick up the truck. I asked if everything was done, remote programmed and airbags replaced. They said no...The next day I tried to text the tech to get a ride back to the dealership but no one responded. I called the dealership multiple times and finally got ahold of a person who had no clue what was going on. They then told me that the tech went on vacation and reassigned my truck to another tech. They couldn’t get an Uber back to the dealership, so I ended up driving there with my wife. They gave me a report of the inspection that they did on the truck and it said that the battery terminals were OK even though it has a sheetrock screw holding it in place. On arrival, the truck looked like it wasn't touched. Cover screws still had dust on them and the driver airbag cover was the same. I called them out on it and they reassured me that it was indeed replaced and the driver airbag cover was reused. Many months later I got a Carfax report from this dealership and I looked at it to find out that I still have a passenger airbag recall on the truck. I got a letter also saying I should stop driving the truck because of the airbag recall.
The contact owned a 2004 Dodge Ram 1500. The contact stated that while making a right turn into oncoming traffic, the vehicle rear-ended a semi-truck in front, where it came to a stop. No warning lights illuminated. The contact stated that the air bags failed to deploy during the collision. Additionally, the seat belts became inoperable and failed to restrain the contact, causing the contact to strike the windshield. The contact sustained a 2-inch cut that required 15 stitches, an eyelid injury that required 7 stitches, multiple bruises, and whiplash injuries, and was transported to a local emergency room for medical treatment. A police report was filed. The vehicle was destroyed. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 200,000. The VIN was not available.

I tried to get the service done Iam a one vehicle owner the dealership told me I had to pay out of pocket for the rental car with no set day of service The dealership didn’t care whether I got the recall done cause of the year of the vehicle they provided no transportation solution
I was driving to work that morning traveling about 40 into the city when all of a sudden I heard a loud noise, I barely could handle the steering wheel to coast it to the side before it came to a complete hault, I look out my window and my drive shaft broke in 2 pieces was passing me on the main road. Front end was on the side shoulder, back end was out in the road. I jumped out of the truck and grabbed up both ends of my drive shafts and put in the back of my 04 dodge ram truck, then started directing traffic on this busy road of morning commuters. Fluid was everywhere on the road so I'm trying to put sand from the side of the road onto the fluid that was on the road that had leaked out my truck all while directing traffic to keep from creating an accident. An law officer came ask was I alright, I answered explained what had happened and he never took a report instead replied with "Seems like you got everything handled" and got back in his car and drove off. I called for a tow service for their assistance and requested it be took to a shop. Where it now sits unrivaled. It was inspected by the mechanic and learned that the pinyin nut had came off cause my rear end to lock up which caused my drive shaft to come out and when it hit broke it smooth into 2 pieces that caused damage to my transmission case. This could have ended very badly for me that day, I'm thankful that it didn't. But now my truck is tore up do to a faulty manufacturers pinyin nut. And I need my truck fixed so I can work and support my family. This was not my doings. I love my truck very much and desperately need it back up and running
Refuse a loaner car while the recall gets done
Got in a accident and the airbags did not deploy
The driveline driveshaft fell out there’s a pinion nut that came loose and suddenly locked my rear end up without warning. Cant move it
On 3/24/2022 my mother, Helen Mallazzo was involved in an accident in Queen Creek, Arizona. Police were called and a report was filed. When this accident occurred the seatbelts failed to restrain her and her dog and the airbags did not deploy, resulting in both of their heads hitting the windshield and leaving two large cracks in the windshield.
I tried to schedule with Landers jeep Chrysler Dodge in Little Rock, Arkansas to fix the airbag recall in my truck on two separate occasions in the first occasion I brought it in. They told me it would take four hours on the phone when I got it there they told me it may take a few days and they don’t even know if they have the parts for it and they told me to reschedule in the second time that I brought it in. The service manager told me he couldn’t service it right now.
Mechanic said TIPM went bad. All gages guit working, check engine light stays on wind shield whipers turn on & off, engine stalls, all lights randomly turn on and off head lights turn signals exc. Transmission and 4 wheel drive say repair. Anti lock breaks acting up. Mechanic said this is happening alot for 2004 Dodge Ram trucks.
I recently figured out a problem with my truck that is an extreme danger. When you fill up the fuel tank at any gas station, it seems that the fuel enters the vapor canister and makes the engine stall for about a mile before it starts running properly. After several close encounters after filling the tank, I looked upon other threads of Dodge Ram owners and found this to be a common problem that was never addressed by Chrysler and never acted upon by NHTSA. Please look into this and advise a course of action to have Chrysler repair this problem.. It is a matter of time before either I or another with the same problem are seriously injured or killed due to this defect. Many Thanks In Advance!!
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers. All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation. The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region. Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas. The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy. The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

Virgin Islands, areas with consistently high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures. As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly, ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016, four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region. Also during PE14-016, four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing. All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas. The PE is now closed/upgraded to an Engineering Evaluation (EA15-001) to include all manufacturersand vehicles known to be affected at this time.ODI's EA investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy. The recalls related to this PE are: 14V343, 14V344, 14V348, 14V351, 14V353, 14V655, 14V700, 14V701, 14V752, 14V763, 14V770, 14V773, 14V787, 14V802 and 14V817.The number of vehicles affected are an estimate since some vehicles may have both the driver and passenger side inflators recalled. Analysis of the complaint, field report and warranty data shows that the majority of stalling incidents in the subject vehicles appear to be related to the engine calibration concerns that daimlerchrysler addressed with service bulletins releasing revised pcm software.analysis of the complaints and odi's survey of owners of vehicles that received engine management software updates under tsb #18-013-05 show that these stalling incidents have the following characteristics: - they occur at low speeds, often during parking lot ordriveway maneuvers; - they occur during decelerations, such as braking for a stop sign or traffic signal; and - the operator IS almost always able to immediately restart the vehicle.when compared with other safety defect investigations and recalls of conditions that result in stalling during low-speed deceleration with immediate engine restart possible, the failure rate in the subject vehicles IS low.analyses of complaint and warranty data also show a declining trend.accordingly, this investigation IS closed.the closing of this investigation does not constitute a finding by nhtsa that a safety-related defect does not exist.the agency will continue to monitor complaints and other information relating to the alleged defect in the subject vehicles and take further action in the future if warranted.see document file for additional information regarding this resume.
On may 16, 2005,odi opened preliminary evaluation pe05-027 to investigate complaints of engine stalling in model year 2004-05 dodge durango suvs and RAM 1500 series pickups equipped with the 5.7L V8 engine.information provided by daimlerchrysler during pe05-027 indicated that a large percentage of the complaints are related to an idle undershoot condition that may cause the engine to stall during turning maneuvers.in february 2005, daimlerchrysler revised the powertrain control module software in production vehicles and issued a technical service bulletin (tsb 18-013-05) releasing the new software as a service remedy for the idle undershoot condition.the bulletin was superceded by tsb 18-013-05a in april 2005.daimlerchrysler believes that the failure rate for stalling while driving due to the idle undershoot condition IS low and that the bulletin has adequately addressed the problem. Since pe05-027 was opened, odi has continued to receive complaints from owners of the subject vehicles who allege experiencing engine stall incidents under all types of driving conditions.in some instances the stalling has continued after the vehicle received the service bulletin repairs.this investigation has been upgraded to an engineering analysis (ea05-018) to further assess the scope, frequency, and safety consequences of the alleged defect in dodge durango and RAM 1500/2500/3500 series pickup trucks.
