Daimler Trucks North America LLC (DTNA) is recalling certain 2013-2016 Freightliner Cascadia vehicles equipped with Hendrickson 5" drop axles. In certain front axle, brake lining, and brake spider combinations, braking may cause high vibrations, resulting in early failure of the tie-rod tube.

A tie-rod failure would cause a disconnect between the front wheels, resulting in a loss of steering ability, thereby increasing the risk of a crash.
Remedy

If the brake lights fail to illuminate, there would not be a signal to other motorists that the vehicle is slowing, increasing the risk of a crash.

The affected vehicles have a tailpipe that allows hot exhaust to exit near the CNG tank pressure relief device (PRD).

Risk
Hot exhaust in close proximity to the PRD may cause the PDR to activate and CNG to be unexpectedly released from the tank. Discharge of CNG in the presence of an ignition source can increase the risk of a fire.
Remedy

Daimler Trucks North America (DTNA) is recalling certain model year 2013-2014 Freightliner Cascadia trucks manufactured December 11, 2012, through November 27, 2013 and equipped with a Meritor Wabco electronically controlled air suspension (ECAS) system. In the affected vehicles, the wiring harnesses for the ECAS may have been reversed.

If the wiring harnesses are connected in reverse, it may adjust the suspension axle height incorrectly, reducing driving traction and parking brake effectiveness. Either result increases the risk of a crash.

The brake caliper mounting bolts may be incorrectly torqued, which could result in the bolts loosening.

The bolts may not provide adequate clamping force between the brake caliper and brake anchor plate, resulting in the caliper detaching and thus reduced brake effectiveness. Reduced brake effectiveness increases the risk of a crash.
Remedy

In the affected vehicles, when the ignition is off and the door is opened without the parking brake applied, the dashboard does not display the correct message of "Park Brake Off Door Open."

For 1 1/2 years I have gone to the local dealer with the complaint that if the roadway IS wet and it's necessary to apply the brakes, the steering will pull sharply and hard to the left. there has been multiple "fixes"; none have solved the issue. freightliner corp has been directing the dealer with procedures to try and none of them solve the issue. currently, my truck has been at freightliner nw, olympia, wa for nearly 10 days while I pay for a rental truck to meet my customer's needs. as of today, freightliner IS baffled and IS looking for another idea to try. apparently this IS not the only instance of this situation. last month when I spoke with a rep, it was stated that "there was a recent directive" for another procedure to use for this complaint. I'm not new to this occupation as I've driven and/or owned my own tractor for over 50 years. I've never had a vehicle have this type of problem and not be solvable. the intensity of the steering wheel being pulled to the left IS such that I will not allow my wife to drive the truck if it IS raining. I believe it would jerk it from her control. It IS not a steering mechanism issue as it only does this when brakes are applied and wet roadway. I've looked back in my records and I see my initial complaint was made in may, 2019

TL* the contact owns a 2014 freightliner cascadia. The contact stated that the front springs of the brakes were rubbing against the outside of the hubs on both the driver and passenger sides. The vehicle was taken to bakersfield truck center (8140 golden state hwy, bakersfield, ca 93308, (661) 393-6950) where the dealer replaced the hubs, brake shoes, and drums for a total cost of over $1,900. The contact was concerned because the springs were installed that way originally from the manufacturer; however, the dealer stated that they had not seen this type of failure. The failure was noticed by the dealer while being repaired per an unknown recall regarding the front axle. The manufacturer stated that they did not know if there were any recalls for the failure. The contact was also concerned if the failure occurred with other customers. The failure mileage was 650,000.

The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The failure mileage was 538,123.

This IS the first time I have experience an immediate stall with no restart in the 45 years of professional driving. I was just beginning to go down hill at mm138 on interstate 5, when all the sudden the dash lights and gauges started to quiver as if the electrical system was loosing current. more importantly, the truck lost all engine and transmission functions to include jake and power braking, loss of power steering. I was able to maintain control all the way down till the road leveled out. after waiting several minutes, I was able to restart and continue drivning. on saturday I had the truck inpsected and advised the dealer I had been in contact with nhtsa who advised the dealer to inpsect all fuses and the pndb for any loose connections.

This set of circumstances occurs every time I pass under the railroad overpass, going southbound, on a week day (between 10 and 20 times so far). Freightliner always acted like they had never heard of this before. When I told them to check the gps and they would see that it always occurs in the exact same location they'd give me some mumbo jumbo. On several occasions freightliner overrode the fault codes as I was talking to them. But certain things were changed by freightliner. After about the first 10 of the events I no longer received emails from virtual technician regarding them. The engine shut down light no longer came on and the derate began disappearing within 30 minutes. that was the case until the evening of monday, 6/22/15. As I passed under the overpass the warning lights came on. 15 miles later the engine derated. The lights and derated stayed on until the evening 6/23 at which time the shutdown light came on and, on the highway 99 freeway, in the dark, I was reduced to a maximum operating speed of 5 mph. I called virtual technician and they told me the only course I had was to get towed to a freightliner shop. I went to sleep for the night and called mc & sons towing out of pixley, ca, next morning. As I awaited their arrival I called detroit daimler and described my situation. Just as the tow truck arrived detroit overrode the codes

We were asleep in the truck with the curtains closed. The engine was running smoothly and the air conditioning was blowing cold air. We were awakened by someone pounding on our door. When I opened the curtain, the left side of the windshield was a sheet of flames coming out from under the hood. Flames were also coming out of the left side hood latch access hole and covering the left side wing window. we exited the truck by the right side door and took a picture of the fire within two minutes of getting out. This picture IS available. Another picture was taken between 4 and 6 minutes after the first picture. This picture IS also available. my wife was deeply asleep, but I was sleeping lightly having gone to bed shortly before the fire. My wife and I were both awakened by loud voices outside the truck. Our dog was also awakened by the loud voices. Our dog gave two sharp barks of warning, and the voices stopped. We thought it was a parking lot altercation. We all lay our heads back down to return to sleep. Our dog IS quite frightened of fire, or the smell of smoke. None of us detected anything amiss at all; no alteration in the engine sound, variation in the temperature of the air conditioning, or other noises or odors. In the course of about 30 minutes, the truck burned, literally to the ground. Pictures of the burned out carcass are also available. the company's insurance adjuster said that there was not enough of the truck left from which to draw any forensic conclusions. *tr

I pointed out to the freightliner service writeup that I noticed that a frame mount was loose and showed it specifically to him but they "say" they checked it out but I'm still hearing the popping/twisting when carrying a loaded trailer. the frame mount/bracket IS still loose and I've actually turned the bolt in the mount a couple of times by hand. the same exact mount on the passenger side IS not loose and the bolts sit properly in the mount grooves like they're supposed to with no visible signs of looseness or movement as opposed to the drivers side. it even has a visible rust trail of where it seems like water IS getting in between the loose parts. I've also noticed some very unusual signs of tire wear especially on the drivers side and most importantly on the drivers side front steer tire which (in my opinion) could possibly be related to the loose frame mount and the popping/twisting. freightliner did replace a spacer underneath the drivers side leaf spring in the front to help correct the "lean" on that side but the truck still leans to the drivers side the same as when I put it in the shop with no significant change. I do know one or two people with the same model, make and year of truck that have been complaining of the same popping/twisting sound when loaded as well. I am truly concerned for my safety, as well as the publics and that IS the reason why I'm taking the time to address my concerns in this manner. *tr

This behavior IS not predictable to either the driver or nearby vehicles, and will eventually lead to a crash or a dangerous shifted load. I have experienced this in 4 different freightliner cascadias of recent vintage (2012 or newer), all equipped with the onguard cruise control system. I have not driven a cascadia that did not experience this problem. *tr

On August 31, 2020, the Office of Defects Investigation (ODI) opened a recall query, RQ20001, to investigate the allegations of water intrusion into the truck cab signal-detect and activation module (SAM cab).The investigation further evaluated the effectiveness of the original recall remedy, and the installation of an updated drip shield as it relates to continual water intrusion resulting in the loss of electrical functions and potential fire.The potential hazard was the increased risk of a crash and injury in certain model year (MY) 2007-2021 Freightliner Cascadia P3 vehicles manufactured by Daimler Trucks North America (DTNA). ODI sent DTNA an information request (IR) letter on September 29, 2020.DTNA's November 20, 2020, IR response detailed how the company had implemented various improvements through recall 09V024 and multiple technical service bulletins (TSBs) to mitigate water intrusion.The effectiveness of new windshield seals and revisions to the drip shield is evident by the downward trend in claims rate from an average high of 7.64% in the beginning product life of Cascadia P3 (MYs 2007-2009), to 1.79% average for MYs 2011-2018, and down to 0.49% towards the end of the Cascadia P3 production.Moreover, the data shows that most failures occurred within the first three years of service with a significant downward trend thereafter. In view of the current overall low failure rate, the nature of early-life failure, and failure rate trending downward since model year 2017, this Recall Query (RQ) is closed. The Agency reserves the right to take additional action if warranted by future circumstances. On April 23, 2018, the National Highway Traffic Safety Administration (“NHTSA”) opened Audit Query (AQ18002) to assess the timeliness and scope of four recalls filed by Daimler Trucks North America, LLC (“DTNA”). NHTSA subsequently expanded AQ18002 to examine the timeliness and scope of three additional DTNA recalls and compliance with early warning reporting and manufacturer communications reporting requirements. NHTSA and DTNA entered into a Consent Order on December 29, 2020, to resolve NHTSA’s assertions that certain recalls within the scope of AQ18002 were untimely, DTNA failed to timely submit certain manufacturer communications to NHTSA, and DTNA failed to submit field reports to NHTSA as required. As part of the Consent Order, DTNA agreed to a total civil penalty of thirty million dollars ($30,000,000), of which ten million dollars ($10,000,000) was paid on January 12, 2021. Also, of that total civil penalty, DTNA agreed to spend five million dollars ($5,000,000) on the specific performance obligations to develop advanced data analytics capabilities to enhance its ability to detect and study emerging safety-related defect trends on its vehicles (“Safety Data Analytics infrastructure”) and an advanced data analytics and reporting platform that integrates data on emerging safety-related defect trends on its vehicles and recall lessons learned (“Safety Communications Platform”). The Consent Order also held fifteen million dollars ($15,000,000) of the civil penalty in abeyance pending DTNA’s satisfactory completion of the requirements of the Consent Order, and its compliance with the Safety Act, and regulations thereunder. NHTSA and DTNA agreed to a two-year term for the Consent Order—with an option for NHTSA to extend the Consent Order for an additional year, if warranted. In the Consent Order, DTNA agreed to numerous performance obligations, including monthly meetings with NHTSA to discuss potential safety issues, recent manufacturer communications, and recall decision-making, among other things; quarterly meetings with NHTSA to discuss DTNA’s progress on the performance obligations under the Consent Order, including the Safety Data Analytics infrastructure and Safety Communications Platform, and any issues related to compliance with the Consent Order, Safety Act, or regulations; development of written procedures and employee training materials to help DTNA comply with its legal obligations to timely identify and report safety defects and noncompliances, submit complete and accurate recall filings, and timely submit manufacturer communications and field reports to NHTSA; and identifying and reporting previously unreported manufacturer communications and field reports. Also, via the Consent Order, DTNA acknowledged that the failure or inability to identify a specific root cause, develop an adequate remedy or repair, or confirm the affected population of vehicles are not bases for delaying filing a recall with NHTSA. On December 22, 2022, NHTSA extended the Consent Order for a term of one year due to concerns about DTNA’s compliance with its recall-related obligations. Specifically, NHTSA expressed concerns about the completeness of DTNA’s recall reports and issues identified with owner notification letters during the term of the Consent Order, as well as the length of time that DTNA took to consider a straightforward noncompliance issue before filing a recall and DTNA’s interactions with NHTSA during the term of the Consent Order. NHTSA addressed these concerns with DTNA during the third year of the Consent Order. The extended term of the Consent Order expired on December 28, 2023. Based on the Consent Order, this investigation, AQ18002, is closed. Closure of this AQ has no bearing on the terms of the Consent Order or DTNA’s ongoing legal obligations, and it does not represent a determination by NHTSA as to DTNA’s performance under the Consent Order.

On April 23, 2018, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE18-004) to investigate certain alleged defects affecting the Powernet Distribution Box (PNDB) and associated wiring on certain model year (MY) 2011-2016 Freightliner Cascadia (P3 platform) truck tractors that may result in disruption or loss of electrical power to the PNDB itself and other electrical circuits, thermal events, and/or vehicle fires.On May 7, 2018, ODI sent an Information Request (IR) letter to Daimler Trucks North America LLC (DTNA). On June 19, 2018, ODI received an IR response from DTNA that included vehicle production, consumer complaint, field report, warranty, manufacturer actions, changes and other data related to the alleged defects. On April 8, 2019, ODI received a supplemental IR response from DTNA that included their assessment of the alleged defects. Furthermore, ODI engaged in discussions with the manufacturer about the alleged defects on multiple occasions during the course of this investigation. ODI analysis of all available data indicates there is not an apparent defect trend. The most recent field report data reviewed was from 2017Q1 and the most recent complaint data was received by ODI in 2019Q1. The complaint rate is low. Most warranty claims occurred early in the vehicles' service life, and with the newest of the subject vehicles being over four years old, they are all now beyond that point. The warranty claim rates for loose input or output terminals (0.16%), loose fuses (0.25%), PNDB power (B+) supply cable corrosion or improper PNDB power (B+) supply cable routing and/ or securement (0.00%) are all low.The most serious consequence in the warranty claim data alleged engine shutoff with a vehicle in motion. While some vehicles did experience a loss of electrical power resulting in loss of motive power with no immediate restart capability, the bulk of the warranty claims were for vehicles equipped with an optional cab power disconnect switch solenoid with an automatic reset feature that may exhibit a brief, intermittent loss of power. If the input and output voltages differ by an unacceptable amount, potentially due to foreign object debris or corrosion on the solenoid's contacts, the PNDB senses the difference and enters an auto-reset mode in an attempt to correct the condition. The internal contacts open for one second, stopping the flow of electricity, and then close, restoring the flow for two seconds. This cycle is repeated for up to five times; after a maximum of thirteen seconds, the PNDB defaults to the on position and electrical power is restored. This condition exhibits a transient loss of power but it does not cause any permanent engine disablement. As a precaution, the solenoid-equipped PNDB part was modified in August 2017 to eliminate the auto-reset feature. Since 2010, there has been only 1 report of vehicle fire and zero reports of crash, injury or death. Accordingly, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if warranted by the circumstances.

On February 26, 2016, Bergstrom Inc. notified NHTSA of a safety defect in approximately 10,819 of its ParkSmart auxiliary electrical temperature management units. These units were installed on Freightliner Cascadia Class 8 trucks that may overheat due to high resistance, resulting in possible fires. One of the contributing factors which led to this recall was a plastic vented cap covering the compressor connectors allowing increased flow of air to the overheated connectors, which increased the risk of a fire.ParkSmart units equipped with non-vented caps, according to Bergstrom, were not affected. See Bergstrom recall 16E-017.On March 7, 2016, Daimler Trucks North America (DTNA), the parent company of Freightliner, filed its corresponding defect decision as to the impacted Freightliner trucks.DTNA identified 8,865 model year 2015-2016 Freightliner Cascadia trucks installed with the ParkSmart units.On September 18, 2017, the Recall Management Division (RMD), opened this investigation to resolve the discrepancy between the larger number of units Bergstrom recalled with the smaller number of trucks DTNA identified as impacted, and to pursue additional recall actions as needed. RMD issued an information request to DTNA on Oct. 26, 2017 inquiring about the discrepancy, as well as field experience information.DTNA filed several responses of different dates that provided shipping and distribution information as well as field reports.DTNA explained that their recalled population was comprised only of vehicles they had produced for the U.S. market.They identified that 1,282 vehicles were produced for the Canadian market, and 122 vehicles for the Mexican market.DTNA said that the remaining balance of parts were likely supplied to parts depots for service (service loops) for other purchasers, such as Freightliner dealers sourcing custom vehicle requests. RMD also requested and reviewed shipping and distribution materials from Bergstrom. These materials identified Park Smart units intended for MY 2014 Cascadia trucks (e.g., vehicles outside the DTNA recall population).RMD confirmed with DTNA that the MY 2014 trucks had the non-vented cap design used on them and so were properly excluded from the recall population.In addition to collecting information and documentation from the involved companies, ODI reviewed early warning reporting information. This search identified nine reports of thermal events. All events, however, occurred on MY 2014 Cascadia trucks that, for reasons explained above, were not part of the DTNA recall and the event linked to other causes. In view of the information available to it at this time, RMD is satisfied that the scope of the DTNA recall is adequate.Accordingly, this RQ is closed.

- 10193160Beyond routing and wiring concerns, the PCC system as previously implemented suffers from two timing / reporting issues. One stems from the long boot-up time of the PCC module as compared to the CPC (a.k.a. the 6N4C timer issue), and the other stems from the potential of some PCC modules to lock-up (a.k.a. the NAND flash issue). The timer concern is considered to primarily affect EPA10 units (OBD13 already had a longer timer) and most can be resolved by simply refreshing the CPC software (even if an update is not available). The module lock-up software solution is currently being validated, so the PDC stock is not clean yet. However, lock-up is anticipated to only be present in approximately 8% of PCC modules (based on testing), so (ahead of a campaign), here are instructions on how to reactivate a system, verify proper operation, and drive service actions.
