Brakes that pull to one side during an ACC braking event may cause a sudden change in vehicle direction, increasing the risk of a crash.
Remedy
Dealers will inspect and replace the front brake modulator valves, as necessary. Some vehicles will have a software update which will alert driver when a replacement is required. All repairs will be performed free of charge. Owner notification letters were mailed beginning October 16, 2023. DTNA's number for this recall is FL-958. This recall expands and replaces previous recall 20V-390. All vehicles previously repaired under recall 20V-390 will need to have the new remedy performed.
Daimler Trucks North America LLC (DTNA) is recalling certain 2017-2021 Cascadia P4 vehicles. The brake lights may remain illuminated after the brake pedal has been released.

Daimler Trucks North America, LLC (DTNA) is recalling certain 2017-2022 Freightliner Cascadia vehicles equipped with halogen headlight assemblies. The halogen headlight bulb may dislodge during replacement and contact other vehicle components, possibly causing them to melt.

The single brake modulator valve may be affected by chemical corrosion which can slow down the release timing and cause the brakes to pull to the affected side during automatic braking events.

In certain driving conditions, such as on a long down-hill grade, the fuel line may burst if the fuel pump cooling circuit screen becomes restricted.

Risk
If the fuel line bursts, fuel may leak onto the road resulting in a roadway hazard for other motorists. The engine may also stall without warning, resulting in the vehicle's inability to restart, increasing the risk of a crash.
Remedy

The Automatic Emergency Braking System has activated at least 3 different times while I was driving. At each occurrence there was nothing in the roadway or travel lane. The vehicle came to an abrupt stop twice and slammed the brakes on but then released them a moment later once. In each event a very near collision with other vehicles was almost caused. There are other drivers that use this truck that have reported similar events (5 or 6), but I cannot say for certain what the exact circumstances were in those events. This vehicle is available for inspection if requested. A dealer has not confirmed the problem, each time they report that the system in functioning correctly. The vehicle has not been inspected by any entity other than the dealer. No symptoms prior, only a red collision warning light in the gauge cluster and a warning beep at the immediate time of the event.

Unsure what system or component failed. Our driver was making a delivery on the morning of 4/29/23. He was parked at the consignee location, engine off, and reported hearing a loud pop sound from the tractor while he was offloading merchandise from the trailer. A fire started under the hood, he attempted to extinguish it with the fire extinguisher and failed. The tractor burned to the ground. If the driver had been in the cab when it started, he could've been killed in the fire. Per online articles I found, it seems there were recalls on this same tractor year/model where fires were involved, but this unit shows no open recalls. Wreckage is available for inspection. CHP came to the seen, but to my knowledge there has been no inspection performed by anyone in any official capacity yet. No warning lamps or messages, or symptoms reported in advance of incident, just the loud pop sound reported by the driver.

After experiencing numerous issues with Jost 5th wheels, specifically JSK36N model. The Wear ring bolts (NT0000005) are constantly getting loose making the Wear ring (SK 2105-19) move in the 5th wheel plate, what we consider unsafe. While working on hundreds of trucks we have noticed that this is this issue is very popular. The drivers have been complaining about the trailer king pin being loose in the 5th wheel, causing a hit/push on start/stop when driven in combination (truck + trailer). Because of not having the solid connection, the ‘’hits’’ are affecting the drivetrain (differentials and transmission) on the long run. Some on the drivers are panicking and not concentrated on the road, all because feeling unsafe due to the excessive 5th wheel free play. After the detailed inspection and understanding the problem, our technicians replaced hundreds of the Ring bolts, applied thread locker and torqued properly. Unfortunately the issue comes back again. The only way to get this issue resolved is to replace the whole 5th wheel assembly. This can get very pricy considering that the replacement of the 5th wheel costs thousands of dollars. It hardly hurts the bigger fleets. The situation described above was noticed mostly on 2017-2019 Freightliner Cascadia’s

Had same issues on 2018-2020 freightliner cascadias (7 different units), chassis harness rubbing against transmission and the metal bracket by the air compressor. Active fault codes for coolant level , tcs, transmission ecu - drivers not able to drive on automatic mode/ only manual mode available. One time the driver was not even able to start the truck in the morning, had to get the unit towed to the dealer and have the harness repaired. All of the faults previously described could happen in motion and/or when vehicle was stationary.

Semi was traveling westbound on interstate 80 when it entered north ditch of the roadway. Semi rolled at least once after entering ditch. Driver was ejected from the vehicle while it was rolling. It came to rest on passenger side facing westbound. Driver was pronounced dead at hospital. at the scene of the accident the mechanic of the company mentioned there was a problem with the brakes. The air bag did not deploy and the seat belt alarm continued to sound even when the belt was connected. This happened only shortly after driver started shift. Driver died from blunt force trauma and had no medical issues that would have impaired his driving. The trooper that completed the accident report and did not advise a technical report because the driver and his semi were only effected by the incident. It had been a dry morning with clear visibility. There has been rumors the semi in question had been known for having issues with the braking system and would have been going in for diagnostic the following day when driver was to take a two week vacation. The family spoke with the companies attorney who agreed to preserved the semi until it was further investigation. The semi IS contained on the companies property and has signs of tamper or being disassembled.

On May 26, 2023, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE23010) to assess reports of false positive activation of the Automatic Emergency Braking (AEB) system in model year (MY) 2017-2022 Daimler Trucks North America (DTNA) vehicles. The reports alleged that activation of the AEB system occurred while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. DTNA indicated that it received of a total of 315 reports that may relate to the alleged defect in MY 2017-2022 DTNA vehicles. DTNA stated that the vast majority of false positive activation of the subject systems occur in very limited durations and result in a five mile per hour deceleration. ODI considered four AEB systems offered by DTNA: Detroit Assurance 2.0, Detroit Assurance 4.0, Detroit Assurance 5.0, and Wabco OnGuard. ODI’s analysis shows that of the Detroit Assurance versions, all are failing at similar rates. The OnGuard AEB systems equipped on the same model and model year vehicles have experienced a significantly lower claim rate. To date, ODI has received a total of 32 consumer complaints of inadvertent activation of AEB in MY 2017-2022 DTNA vehicles. In total, ODI reviewed 347 unique (i.e., non-duplicative Vehicle Identification Numbers) reports that may relate to the alleged defect, which include 24 reports involving a crash, 13 reports involving an alleged injury and one report with a fatality. PE23010 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations in MY 2017-2022 DTNA vehicles. The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate reports of engine electrical wiring harness chafe on the subject vehicles.ODI analysis found thousands of unique harness part numbers, spanning the two generations of vehicle chassis assembled at multiple vehicle assembly plants in two different countries, with multiple sub-models, incorporating over thirty powertrain choices.Each wiring harness could contain several dozen individual wires for power, ground and communication purposes depending on the multitude of equipment options available for custom vehicle configuration. ODI identified three primary areas where a wiring harness bundle may chafe: the air compressor support bracket, the transmission case, or the coolant surge tank. In each case, the root cause was some combination of inadequate design clearance, installation variability and relative motion between the wiring harness and the point of contact, such as from vibration. The chafing could result in a range of consequences from none to various diagnostic fault codes, telltale illumination, driver warning center messages, erratic electrical component operation, difficult or no engine start condition, engine de-rate (power reduction), engine shut down, inoperative transmission gear selection, or an unexpected shift to neutral.These potential consequences vary depending on the specific damaged wire in the harness and the exact failure mode, such as intermittent short circuit to ground or open circuit. Generally, the chafing resulted in a progressive series of driver warnings with high levels of detectability over the course of several days and ignition cycles. Drivers usually heed warnings before an engine shutdown condition occurs and the driver may override an engine shutdown countdown timer using the override switch. If the driver allows the shutdown event to occur, the engine can almost always be immediately restarted. Incident rates for an engine shutdown with no warning or a transmission shift to neutral while driving are both very low. ODI received one complaint (NHTSA ID Number 11366557) from a fleet mechanic, who reported in a subsequent interview that he repaired at least six subject vehicles that experienced the alleged defect condition. However, there have been no new Vehicle Owner Questionnaire complaints received by ODI since opening this investigation, and there are no known crashes, fires or thermal events, or injuries or deaths related to the alleged defect. Accordingly, no safety-related defect trend has been identified. Based on available information, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. In its process for evaluating Early Warning Reporting data, the Office of Defects Investigation (ODI) found twenty-four (24) field reports involving headlight fires on Freightliner Cascadia trucks, model years 2017 and newer produced by the vehicle manufacturer, Daimler Trucks North America (DTNA). ODI contacted DTNA and learned that it was aware of halogen bulb-related thermal events. DTNA stated a belief that the issue was due to incorrect bulb replacement during maintenance. However, some vehicles experienced fires within the original equipment halogen bulb's expected life range.In July 2019, DTNA released a service bulletin and a training video covering proper headlight bulb replacement. There have been 17 fires, with 3 total vehicle losses, since DTNA released this extra training information.On February 25, 2021, ODI opened a Preliminary Evaluation (PE21-006) and sent an Information Request (IR) letter to DTNA.After the PE opening, DTNA reevaluated the data and found additional thermal events at low mileages and decided to file a recall. On May 14, 2021, DTNA submitted recall 21V-357 for 72,508 vehicles covering 2017-2022 models (exceeding the population and model years of this investigation). The planned owner and dealer notification date is July 12, 2021.For the remedy, DTNA will open up the 2-inch access into a 4-inch access hole into the inner fender to provide better access for the maintenance replacement of the bulb, and add a label that provides a visible warning to aid in the correct installation during the change. Additionally, a new foam grommet will replace the flammable rubber boots for both the high and low beam bulbs, improving access to the interface point where the bulbs install into the socket of the reflector. ODI believes that the scope of DTNA recall 21V-357 is sufficient, and accordingly, PE21-006 is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other models or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.

On August 31, 2020, the Office of Defects Investigation (ODI) opened a recall query, RQ20001, to investigate the allegations of water intrusion into the truck cab signal-detect and activation module (SAM cab).The investigation further evaluated the effectiveness of the original recall remedy, and the installation of an updated drip shield as it relates to continual water intrusion resulting in the loss of electrical functions and potential fire.The potential hazard was the increased risk of a crash and injury in certain model year (MY) 2007-2021 Freightliner Cascadia P3 vehicles manufactured by Daimler Trucks North America (DTNA). ODI sent DTNA an information request (IR) letter on September 29, 2020.DTNA's November 20, 2020, IR response detailed how the company had implemented various improvements through recall 09V024 and multiple technical service bulletins (TSBs) to mitigate water intrusion.The effectiveness of new windshield seals and revisions to the drip shield is evident by the downward trend in claims rate from an average high of 7.64% in the beginning product life of Cascadia P3 (MYs 2007-2009), to 1.79% average for MYs 2011-2018, and down to 0.49% towards the end of the Cascadia P3 production.Moreover, the data shows that most failures occurred within the first three years of service with a significant downward trend thereafter. In view of the current overall low failure rate, the nature of early-life failure, and failure rate trending downward since model year 2017, this Recall Query (RQ) is closed. The Agency reserves the right to take additional action if warranted by future circumstances.
