I am submitting this complaint regarding an ongoing safety concern with my 2021 Honda Accord. Multiple lights indicate engine system problems which includes, check engine light, car jerks at acceleration, seat sensor recall. I took my vehicle to multiple shops including the Honda Dealership. However, they were unsuccessful to identify the cause. Multiple problems such as Turbo, belt, fuel system problem constantly comes up. I have attempted to take my vehicle for the recall, which was scheduled, however after waiting 4hrs, the dealership stated they did not have the parts. I am concerned about the potential risk to myself, my daughter and my passengers and believe these issues warrant further investigation to help prevent possible accident or injury.
Vehicle: 2021 Honda Accord Sport Subject: Intermittent Electrical Malfunctions, Multiple Safety Warnings, and Loss of Acceleration I am reporting ongoing safety concerns involving my 2021 Honda Accord Sport. On multiple occasions, my vehicle has displayed several dashboard warnings, including brake system, electric parking brake, vehicle stability assist, hill-start assist, adaptive cruise control, and electric power steering warnings. These warnings have appeared intermittently in the past and sometimes disappeared on their own. On October 18, 2026, while traveling on the highway at approximately 65 mph, these warnings had already been displayed for approximately 30 minutes to an hour when my vehicle suddenly began struggling to accelerate. Despite pressing the accelerator, the vehicle continued losing speed, and the RPM gauge remained around 2,000 RPM. It felt as though something was holding the vehicle back. After several moments, the vehicle suddenly regained power, accelerated, and the RPM increased. This was particularly concerning because I was traveling at highway speed. I also experienced electrical problems with the interior lights. While the vehicle was running, neither the automatic interior lights nor the overhead lights near the rearview mirror would turn on. They remained inoperative after I turned off the vehicle. Later, the lights began working again, but I noticed the right overhead light fading while the left remained illuminated. The battery was replaced earlier this year, in 2026. I am concerned that these intermittent malfunctions could result in a sudden loss of power or failure of important vehicle safety systems while driving. I am requesting that these problems be documented and investigated for a potential safety defect or recall. I have photographs of the dashboard warnings available as supporting documentation.
Electrical problems that causes damages to the engine and problems with acceleration.
Infotainment System glitching: "ghost touch"; the screen is not registering my touches and randomly selecting/pushing buttons all on its own. Took it to Honda dealership and they have no answer or fix. This happens from time to time while driving, causing a huge safety concern. Extremely distracting to me, the driver. Everywhere I go to get answers, I only get "issue is known to manufacturer but no fix available; they are currently working on a fix". This has happened multiple times intermittently since I've purchased the vehicle in 2021.
The contact owns a 2021 Honda Accord. The contact stated that while his wife was driving at 65 MPH, the accelerator pedal was depressed, but the vehicle failed to accelerate as intended. The vehicle hesitated and then accelerated; however, the vehicle then failed to accelerate above 20 MPH. Several unknown warning lights illuminated. The contact's wife drove the vehicle to a safe location and parked. While attempting to start the vehicle, the engine started and then shut off. The dealer was notified of the failure, and the vehicle was towed to the dealer, where it was diagnosed with a fuel pump failure requiring fuel pump replacement. The contact referenced a recall with a similar failure description; however, the contact's VIN was not included. The manufacturer was notified of the failure, and a case was filed. The failure mileage was approximately 152,100.
I bought my Accord in February of 2026 with only 79k miles (now at 93k). 3 days after purchasing all of these warning lights popped up regarding emissions, brake system, power steering, etc. I brought it back in and they said that it needed a new fuel injector on cylinder one (which didn’t fix the issue). I have replaced all 4 spark plugs and kept up with maintenance since it has been in my ownership. Now I’m having issues with the car going in limp mode, decreased acceleration, decreased mpg, and constant misfire codes! I do not feel safe in this car anymore since it has almost left me stranded as well as it disabling the safety features which could put me at risk. Honda desperately needs to fix this issue seeing as it’s putting their customers at risk, along with others on the road. NHTSA please do something!
Was at a stoplight, came to a complete stop once at a complete stop car completely shut all the way off, losing all electricity no hazard lights. You cannot press down on the brakes to restart the car. You could not do anything. Car lost 100% of power 5 to 10 minutes later, Vehicle shut back on automatically and engine started to run automatically.
head unit touch screen will occasionally, uncontrollably create multiple phantom touches all over the screen, multiple per second continuously. driver cannot resolve while in motion. driver cannot cannot click away while the issue is occurring as the issue seems to mask real input. very distracting not only visually, but hearing as well. worst case the car could inadvertently contact emergency services via phones or cars sos capabilities. occurs at any / all speeds. multiple reported issues online and honda has done nothing.
Vehicle infotaiment screen glitches and therefore is not making user access all car warnings ans switches screen during driving specially if using the navigation feature.
The infotainment center went bad. It has been replaced... twice and is still bad. The screen jumps from XM radio to compass to clock to settings to FM radio to maps to XM radio to _____. It may do that 30-40 times a minute. And with each change there is a beep. This cannot be turned off. It is verrrry distracting. Honda says there is nothing that can be done to fix this. ?!?!?!?! They said I would have to wait until they come up with an idea to try, though it may be a year to two. Going online reveals that many others have the same issue. This cannot be safe. Beep..... beep.... beep.... beep. Ans the screen changing. You are constantly trying to touch the screen to get it back there you want it. It may takes 50-60 touches to do it. All the time you are trying to watch the road. CAN YOU PLEASE HELP???
My 2021 Honda Accord Touring 2.0T has an intermittent touchscreen/infotainment malfunction commonly described as “ghost touch” or “phantom touch.” The touchscreen generates inputs on its own without the screen being touched, causing menus and functions to change unexpectedly while the vehicle is being driven. The malfunction is distracting because the infotainment system begins operating by itself and requires driver attention to correct or stop the unwanted inputs. The same center display is also used for vehicle information and the rear-view camera, which makes reliability of the display a safety concern. Honda Corporate has been contacted regarding the problem. I have been advised that Honda is aware of the issue but is not currently authorizing a repair because a permanent repair/fix has not yet been developed or released. I have an extended warranty that can cover a repair, but Honda has not provided an approved permanent repair at this time. The problem is intermittent and may occur unpredictably. The factory touchscreen remains installed and the vehicle is available for inspection. I am submitting this complaint because the defect can create significant driver distraction and affects a display that is integrated with important vehicle functions.
The contact owns a 2021 Honda Accord. The contact stated that while driving with the driver assist enabled, the vehicle veered over to the left and crashed into the median. The contact stated that the vehicle was uncontrollable. No air bags deployed. The vehicle was towed to a local tow yard. A Police report was filed; however, the information was unavailable. The contact was transported to the local medical center. The contact sustained bruises and pain to the body. The manufacturer was contacted by phone, and a case was filed.

The infotainment/touchscreen system in my 2021 Honda Accord is malfunctioning and experiencing what is commonly referred to as “ghost touching.” This issue has been occurring since approximately March 2026. While I am driving, the touchscreen will randomly make selections on its own without me touching the screen. This is especially concerning when I am using GPS/navigation because the system can change screens, select different options, and interfere with the navigation information I am trying to view while driving. This creates a safety concern because the unexpected touchscreen activity is distracting and causes me to take my attention away from the road to correct what the system is doing. The issue happens without warning and can occur while the vehicle is moving and I am relying on GPS for directions. I took the vehicle to a Honda dealership regarding the problem. The dealership was made aware of the ghost-touch issue and stated that there is currently no fix available for it. The touchscreen and related components are still installed in the vehicle and are available for inspection upon request. The vehicle has not been inspected for this issue by police, insurance representatives, or any other outside party. There are no warning lights or dashboard warning messages before the malfunction occurs. The primary symptom is the touchscreen suddenly registering inputs and making selections on its own even though I am not touching it. The problem first began around March 2026 and has continued intermittently since then, including while driving and actively using GPS/navigation.
The contact owns a 2021 Honda Accord. The contact stated that while her son was driving 55 MPH, the vehicle shut off with several warning lights illuminated on the instrument panel. The contact's son pulled off to the shoulder of the highway and had the vehicle towed to a dealer. While at the dealer, the mechanic discovered coolant leaking into cylinder #2, causing an engine misfire. The dealer replaced the head gasket. After the repair, the failure returned immediately upon driving the vehicle home from the dealer. The vehicle was taken back to the dealer and the failure persisted upon return of the vehicle. While checking the repair order, the contact discovered that the service report was blank. The dealer was unable to duplicate the failure or determine the cause of the issue. The contact was not notified of the issue after retrieving the vehicle. The manufacturer was notified of the failure, and the contact was provided a case number. The vehicle remained with the dealer unrepaired. The failure mileage was 43,881.
Sudden braking; car comes to a complete stop and brakes so Hard. Most likely fault from Collision Mitigation Brakking system
The car's infotainment system constantly glitches out and changes channels, outputs, texts, videos while car is being driven. It's very distracting and dangerous. It will literally do it 100's of times within a short period. It's much worse than any text messaging distraction, which is against the law. Several people with this car have the same issue. Also the car will break when there's actually nothing in the road that should make it break but it will just slam on the brakes.
COMPONENT: Visibility--Exterior Rearview Mirror, Passenger Side. While driving normally, the passenger-side exterior rearview mirror glass/holder assembly suddenly shattered and completely detached from the mirror housing. There had been no known collision or contact with another vehicle or object. The passenger window was open. The detached mirror glass/holder assembly entered through the open passenger window and landed on the passenger seat. Numerous small fragments of mirror glass were also deposited inside the passenger compartment, including on the passenger seat and dashboard. After the incident, the passenger-side mirror housing remained attached to the vehicle and remained in its normal extended operating position. The housing did not fold from the event and does not exhibit obvious significant collision damage. The detached mirror assembly and photographs have been preserved. Photographs show the exterior mirror housing,m the exposed internal adjustment mechanism, the shattered mirror assembly on the passenger seat, the shattered glass and the rear of the detached mirror holder/adjustment assembly. I am concerned that this may represent a failure of the mirror glass/holder or its attachment mechanism rather than ordinary collision damage. Because the mirror assembly and glass fragments entered the occupied passenger compartment, I believe the event presents a potential safety hazard to vehicle occupants. I am requesting that NHTSA retain this complaint for comparison with similar reports involving Honda Accord exterior mirror glass shattering or detachment.
Vehicle started flashing all lights on dashboard. I drove it for awhile and then it eventually went off. A few days later same lights came back on a car started misfiring. Was told by Honda that is was the Turbo that's going bad. Today is just cut off on me. Luckily there wasn't anyone behind me. I see a lot of people are having these same issues and a recall should be put out immediately.
Door lock actuator(s) randomly failing to lock/unlock. Multiple reports on forums for same exact issue. Replace one, another fails shortly after. Issue involves both locking and unlocking, putting safety individuals at risk for unexpected access to car or inability to lock car once inside. No messages, warnings, or suggestions from car to tell me that the locking had failed.
Unable to use cruise control Unable to use lane control Unable to use tire pressure gauge Unable to use auto brake system Car shakes due to a sudden decrease in engine performance. Turbocharger issues Powertrain System issues ECM Failure
Dash lights on for Brake system malfunction. Emergency parking brake light on as well as TPS system. Lights around Park, Neutral and drive not illuminated. Battery and alternator are in good standing.

SUBJECT: A REQUEST FOR HONDA CORPORATE INVESTIGATION AND GOODWILL ASSISTANCE - 2021 HONDA ACCORD I am submitting a formal complaint regarding serious and recurring electrical and safety-related problems with my 2021 Honda Accord. 1. Numerous dashboard warning lights and safety-system warnings illuminate simultaneously. 2. The vehicle repeatedly displays messages instructing me to "See Dealer." 3. The power door locks repeatedly click and cycle as though they are attempting to lock or unlock on their own. 4. The stereo, on its own, scrolls through buttons and bluetooth blinking through all the stereo options on its own. 5.An OBD diagnostic code reader is unable to properly communicate with the vehicle and reports "Unable to Connect/Vehicle Not Found." 6. Sudden brake activated without any object being in front of the vehicle. 7. Factory Rims Extensive finish failure The simultaneous occurrence of these problems strongly suggest that this may involve an electrical, communication-network, body control module, wiring, power-supply, or other vehicle system defect rather than ordinary wear and tear. I am particularly concerned because the dashboard warnings involve vehicle systems that may affect braking, steering, stability control, driver-assistance features, and other safety functions. When numerous safety warnings illuminate at the same time, I have no reliable way of knowing which vehicle systems are actually functioning correctly. The repeated activation of the door-lock system is also concerning. The locks should not continuously activate on their own during normal vehicle operation. Combined with the diagnostic communication failure and multiple warning messages, this raises concern regarding a larger electrical or vehicle-network malfunction. Although the vehicle is currently outside the factory warranty period, this type of widespread electrical and safety malfunction should not reasonably occur in a 2021 vehicle as the result of normal use.
The contact owns a 2021 Honda Accord. The contact stated that while driving approximately 30 MPH, the message "Emission System Problem" was displayed. The vehicle was taken to the dealer, where it was diagnosed and determined that the turbocharger had failed and needed to be replaced. The contact was informed that the failure was a common issue. The vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 108,579.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles. The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations. However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury. The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources. In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged. In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. PE22-003 has been upgraded to an Engineering Analysis to further assess the scope, frequency, and potential safety related consequences of the inadvertent AEB activations. The scope has been expanded to include assessment of model year 2020-2022 Honda CR-V and Accord vehicles. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.
