Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. The recall began December 3, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. Note: This recall supersedes recalls 13V130, 14V362, 14V773 and 15V869.

An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Remedy
Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate inflator, free of charge. The recall began July 6, 2018. Mailing to the affected RX-8 owners began July 6, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. Note: With the addition of the RX-8 vehicles, this recall fully supersedes recall 16V354.

This may result in the bolt(s) snapping off during use or without a load. the unsecured rack and load can then come loose while driving creating a potential road hazard, possibly resulting in a vehicle crash.

On certain passenger vehicles, the right and/or left rear seat belts may not lock properly due to missing retaining pins on the housing.

The locking mechanism of the seat belt retractor may malfunction, increasing the risk of death or serious injury to the rear seat passengers in the event of a crash. passengers should not sit and child seats should not be used in the right or left rear seats until this defect has been corrected.
Remedy
Dealers will inspect the rear seat belts and replace defective seat belt retractors. the recall began on december 27, 2004. owners should contact mazda at 1-800-222-5500, option #4.

The contact replaced the bulbs, but the failure persisted. The vehicle was taken to the local mechanic who diagnosed that the electrical wires had shorted out due defective wire coating. The mechanic attempted to fix the failure by covering the electrical wires, but the failure persisted. The local dealer was notified of the failure. The manufacturer was not yet contacted. The failure mileage was 165,000.

Repeated throttle body issues, car while driving will drop to a crawl with erratic revving. Did this in the middle of the express way, nearly causing an accident, had to be towed away. Will not start do to revving loop caused by throttle body. Multiple tsb have been issued for this problem, so it seems the throttle body issues plague many customers.

I took my vehicle when I received the first notice and went to jim falk mazda in kahului,maui,hawaii and they told me sorry we don't have the part to replace yet but will call me when it comes in and that was about a year ago.now I received the 2ND and final notice and I called two days ago to the same place and again they told me they were going to look into it and call me back. Well no call back yet and I am getting very upset that they don't feel that this IS important to call people back to fix this important problem. Being that the airbag IS important you would think that they would order the part asap and call you to come in and have it replaced.

In january 2016 I called mazda to get this scheduled and went in late february once the parts came in. I was told both my driver and passenger airbag would be replaced that day. when I picked up my car I was told only the drivers side had been done and I'D be contacted about the passenger side soon. one month later I get a call saying my other airbag part was in. I called to make an appointment and then was told, no wait, it wasn't. a few months later I received a call confirming my appointment for my airbag. great but I didn't have an appointment. I called and they didn't know what I was talking about but would look in to it. 3 days later I was called again and said my part was in to make an appointment. so I did. I skipped work to bring my car in, show up, and was told that there was no part and there never was. I was told the part wasn't even available and not to drive my car with a passenger until its fixed. I then received calls from mazda's chicago office and letter in the mail stating indeed, parts weren't available yet and reiterated not to drive with a passenger until its fixed. that was over 4 months ago at this point. it's absurd that a recall that can severely injure myself or my passengers has taken 11 months with no solution. and to be told "don't drive with a passenger" IS even more absurd. I use my car for day to day life, that IS not an option. I just want my recall fixed. I know many people that had both their sides fixed, why IS mine taking 11 months an counting with multiple times being screwed around and over 4 months of silence now? I purchased new tires and had installed at costco on august 1ST 2016. I went to have the tires retorqued at costco on august 12TH. On sept 1ST I was driving northbound on a city parkway street so mostly straight roadway with some gentle curves at approx. 50-55 miles per hour, when suddenly my steering wheel started to shake, then weaving left to right, I tried to regain control by steering straight and apply brakes to slow down but that just made it worse and the car began spinning out of control across the roadway until it finally landed to a stop on the bank off the side of the road. Several people stopped at the scene to make sure I was ok. A couple that lived near by said they heard a loud boom...a police officer also came and stayed with me while I waited for the tow truck. The driver had said that he believed the front tire may have blown, it IS currently flat and my vehicle IS no longer driveable.

The V6 motor in my mazda 6S has been leaking oil for almost 10 of the 12 years I've owned it. The oil leaks out of the faulty seals on the timing chain cover in front of the passenger, and the oil drops onto the catalytic converter. This oil vaporizes and that vapor enters the cabin in slow speed driving and when stopped at intersections. I complained about a burning oil smell many times while the car was still under warranty, and the mazda service techs always pretended like they didn't find any problem. Years down the road, I am finding this IS a rampant problem with all ford and mazda cars and suvs with this "duratec" 3.0 V6 which was made by ford. It IS installed in many ford, mercury and mazda vehicles. I carry a fire extinguisher with me in the event I have an under hood fire, and I'm very worried about my health after all these years of breathing this vapor. The cost to repair this seal failure IS daunting. I hope mhtsa can see that ford has been covering up this defect, endangering lives and the health of drivers as well as creating a tough financial burden on the owners to fix the issue. I gave a date on the form below, since it was required, but this has been going on since 2006 or so.

After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant…
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

The Office of Defects Investigation (ODI) opened PE14-016 in June 2014 based on six inflator rupture incidents involving consumer owned vehicles produced by five vehicle manufacturers.All six vehicles were operated in Florida or Puerto Rico at the time of the rupture and for the majority of their service life, and were equipped with inflators produced by Takata, a tier-one supplier of automotive air bag systems.During the course of PE14-016, ODI determined that five additional vehicle manufacturers used inflators of a similar design and vintage also supplied by Takata. No evidence of field failures was found in vehicles produced by these five additional manufacturers.Nonetheless, at ODI's insistence, all 10 vehicle manufacturers initiated a regional recall within approximately two weeks of the opening of the investigation.The regions recalled initially included Florida, Puerto Rico, Hawaii, and the U.S. Virgin Islands, areas with high absolute humidity and climatic conditions believed to be a significant factor in the inflator ruptures.As part of the recall actions, inflators removed from remedied vehicles are to be returned to Takata for testing.Takata's initial test results on passenger inflators from remedied vehicles indicated a much higher than anticipated rupture frequency for inflators returned from Florida.Accordingly ODI requested all 10 manufacturers expand the regional recalls for passenger inflators to include other geographic areas where high absolute humidity conditions exist, including the Gulf States and other coastal areas.Takata's testing of the passenger inflators to date continues to indicate this geographic area as having the highest risk, with no ruptures occurring from inflators returned from outside the expanded recall regions.During PE14-016 four additional passenger inflator field events occurred, all in vehicles from the same expanded geographic region.Also during PE14-016 four additional driver inflator field events occurred including two in vehicles from regions not known for high absolute humidity, specifically California and North Carolina.Accordingly, ODI requested all five of the affected vehicle manufacturers currently using the subject Takata driver inflators expand to nationwide recalls.Significantly, neither of the affected vehicle manufacturers or Takata provided any explanation to account for these two driver air bag inflator ruptures outside the area of high absolute humidity.Takata testing of returned driver inflators indicates a lower rupture frequency as compared to passenger inflator testing.All test ruptures reported by Takata to date have occurred on inflators returned from high absolute humidity areas.The investigation now includes all manufacturers and vehicles known to be affected at this time.ODI's investigation will focus on, among other things, root cause analysis, other potential defect consequences, identification of affected vehicles scope, and adequacy of the remedy.The five ODI reports cited above can be reviewed online at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10537899, 10568848, 10585224, 10605877, 10651492

In an effort to achieve a higher level of Customer Experience and retain the value of Mazdas vehicles, MNAO Warranty Operations is allowing all Mazda dealers to utilize a third party, SPECMO Enterprises, Inc., for Instrument Panel Cluster (IPC) odometer setting service. With the use of SPECMOs services, it is possible that the odometer will be set between 10 to 31 miles over the odometer mileage at the time of replacement for both warranty and customer pay repairs. Please make sure that the customer is aware that there may be a slight variance to the odometer, in which case the customer may have to accept the slightly higher mileage. Since odometer setting service is not part of the warranty repair, it is not warrantable nor required, but optional. However, Mazda Warranty policy provides after- warranty assistance (AWA) or DSA that can be utilized to cover this service expense.

The most common cause of repeat A/T failures is lack of lubrication to the front and rear planetary gears. This is usually caused by an accumulation of metal particles and debris (from a previous A/T failure) on the internal mesh type baffle of the oil cooler. Power flushing will remove the restriction by back flushing, followed by forward flushing to dislodge a restriction, then back flushing again. This will in turn ensure full ATF flow for the transmission when it is operating.
