On September 4, 2025, I brought my 2001 Toyota Tundra to Midas at 5325 Olde Wadsworth Blvd, Arvada, CO 80002 for an oil change and inspection. Midas advertises a “Closer Look Vehicle Check,” which includes checking steering, suspension, and wheels. Their inspection report marked the steering system as “OK” and only noted mild shock wear. Less than a month later, while exiting the highway, the lower ball joint failed completely, causing the front suspension and wheel to detach. I immediately lost steering control. Fortunately, this occurred at low speed; had it happened moments earlier on the highway, it could have caused a major collision or fatalities. The failed component (lower ball joint and related suspension parts) is still available for inspection. An independent mechanic later confirmed that the joint was severely worn and the failure appeared to be due to long-term deterioration that should have been detected during Midas’s inspection. No warning lights or indicators appeared before the failure. The incident endangered my life, my passengers’ safety, and other drivers on the road. The business has refused to take responsibility or cover repair costs, which currently total over $2,400, despite their advertised inspection and guarantee. This raises serious concerns that Midas and potentially other service locations may be failing to properly inspect critical suspension components, which poses a major public safety hazard.

Toyota Corporate is refusing to acknowledge my Tundra's eligibility for remedy under a safety recall and campaign. My Tundra is experiencing the failure mode outlined in the safety recall 90M and extended safety campaign Tundra BXD. The spare tire crossmember of the frame is almost entirely rotted through with rust and is barely connected at this point. My Tundra's VIN number falls within the VIN range table's indicated in the recall and campaign documentation, and meets the criteria of currently being registered in an eligible cold-climate state. By all accounts, it should be eligible to receive remedy under this campaign. However Toyota has on multiple occasions just told me that "Safety Recalls are Service Campaigns are VIN-specific" and that VIN does not report as eligible. I've provided photo evidence of the issue and the documentation, yet Toyota refuses to acknowledge my truck's eligibility for repair under this program. Safety recalls are to prevent safety incidents from occurring due to known design or manufacturing defects. My truck is experiencing a known and documented defect. Toyota has a process for fixing it. However they're refusing to fix mine because of a mis-match in the VIN database. Safety issues don't affect "VIN numbers", they affect vehicles, the people who drive them, and others out on the roads.

The contact owns a 2001 Toyota Tundra. The contact stated while driving approximately 70 MPH, the steering wheel suddenly pulled to the right and front driver's side wheel detached from underneath the vehicle, causing the vehicle to skid across the highway before coming to a stop. The vehicle was later towed to the local dealer who diagnosed the vehicle and determined that the front driver's side ball joint had failed and needed to be replaced. The contact stated that there was damage to the subframe of the vehicle caused by the detached wheel. The vehicle was not yet repaired. The local dealer and manufacturer were notified of the failure, but no assistance was offered. The failure mileage was 233,500.
- Wheels
Driving straight at 30mph and wheel came off causing near deadly collision. Upon inspection it is evident that the wheel separated from its ball joints. Further research strongly suggests that this is a known defect of this year/make/model, even though the recall was issued only for the subsequent years vehicle. This is a potentially deadly design flaw and not an isolated manufacturing defect as Toyota claims.

Recently my mechanic brought to my attention frame damage. I went for an inspection from a local frame specialist, who told me my frame was completed rusted through. Cost for replacement materials and labor total cost 9227.30. Toyota early on identified known rust issues with the tundra with a recall many years ago. After waiting over a year after the recall my truck was undercoated with a big one year warranty. Apparently was unaffective.

Low mileage, two year old front tires beginning to separate at inner sidewalls, making vibrations above 50+ mph and shaking vehicle so bad almost undriveable at highway speeds. Alignment shop said get them checked. Re-balancing tires had no effect, moved front tires to rear then bed of truck would vibrate at speed. Replace the 2 tires in question and problem resolved. Had tire shop look at the tires and they found tread separation at inner sidewalls of both suspect tires. Called manufactured for warranty replacement or refund and they needed a rma form filled out by an authorized dealer who sells this brand of tire before a return could be accepted. Original purchase was online from walmart for pickup at local store. Walmart had no record of the tire seller who supplied online walmart sales. Manufacturer referred me to a 'discount tire' store locally that manufacturer had previous contact with and dealt in this brand of tire. Contacted the suggested store and salesman needed to contact manufacturer first to know how to proceed. After a callback from store, salesman tried to diagnosis the tire problem over the phone without actually looking at the tires and said tires were probably out of round. I have the two tires and still pondering for a solution.

Chassis rust and holes in the frame. Obviously large rust holes in the direction of the frame behind the front driver side wheel. Frame holes cause great concern for the overall operational safety of the vehicle. I consider this degree of rust degradation excessive and a craftsmanship defect. Heavy steel vehicle frames shuld not exhibit rust holes to this degree. Upon recall notice and inspection by toyota, the vehicle received a cnc undercoat to much of the chassis/frame as a measure to prevent further/future rust formation. Obviously, the undercoat was insufficient at preventing further rust formation on this part of the frame. I do not know exactly when the rust holes appeared. I entered today's date when answering the question of "when did this happen", but it IS reasonable to assume the rust formation has been in existance for many months to years.

My vehicle was supposed to have been repaired under a previous recall for frame rust, with a coating, but it IS apparent that toyota did not repair all area's as today driving down the highway my shockmount completely rusted away, causing me to almost hit several cars, and pieces of the mount flew behind me into traffic. from the outside this truck looks to be in excellent shape, however the tow driver looked underneath and said he could not believe the rust damage. my vehicle has now been deemed unsafe and not roadworthy. my fear IS how many unsuspecting owners are there that thought the recall repairs were done correctly, and could have an accident that could lead to serious injury or death if these go untreated, repaired or replaced. I was driving on the main outerbelt freeway in columbus, I-270 at 60-65 mph when the part came off.

This investigation was opened based on 20 reports of model year (my) 2000 and 2001 vehicles experiencing severe corrosion of the frame, resulting in either under-body mounted spare tire separation or rear brake failure.the failure report counts shown above are for my 2000 and 2001 vehicles only.by letter of november 16, 2009 toyota has notified the agency that it will conduct a safety recall (09v-444) of approximately 110,000 model year (my) 2000 through 2003 tundra vehicles.the affected recalled vehicles were sold or are currently registered in the district of columbia and the states of ct, de, il, in, ky, ma, md, me, mi, mn, nh, nj, ny, oh, pa, ri, va, vt, wi, wv.toyota alleges high amounts of road salts are used in these areas and excessive corrosion of the rear cross-member (to which the spare tire IS mounted) can occur and cause the spare tire to separate from the vehicle and can also lead to rear brake failure.toyota stated: toyota has tentatively concluded that the root cause IS a combination of factors, including usage in areas where road salt IS applied, inadequate vehicle maintenance (i.e., not following the recommendations in the owner's manual), the design of the rear cross-member, and manufacturing issues.the design of the rear cross-member IS such that it IS open on the top side and susceptible to accumulation of dirt, water and road salt.manufacturing issues of the rear cross-member related to the coating process also affect its corrosion resistance.toyota also states that owners of my 2000 through 2003 vehicles in states not covered by the recall will be notified on how to obtain the same inspection and repair as the recalled vehicles, free of charge.further toyota states: please note that while toyota has not determined a defect exists in 2004 through 2006 my tundra vehicles, toyota plans on initiating a field action in the near future to prevent future corrosion on those vehicles originally sold or currently registered in the affected region at no cost to the owner.by letter of january 8, 2010 toyota has notified the agency that it amended its defect information report for safety recall 09v-444 to include the fuel tank mounting system (i.e., fuel tank support straps and fuel tank mounting cross-members) in the safety recall.toyota states: toyota has not decided that the fuel tank mounting system in those vehicles contains a safety-related defect within the meaning of federal motor vehicle safety law.rather, toyota IS taking this action at odi's request and to avoid future disagreement with odi with respect to this issue.my 2007 and newer tundra vehicles are not included in any of toyota's actions as the rear cross-member in those vehicles have been redesigned so that it IS open on the bottom.the actions taken by toyota are sufficient to resolve the issues raised by this investigation.accordingly, this investigation IS closed.the closing of this investigation does not constitute a finding by nhtsa that a safety-related defect does not exist in the subject vehicles.the agency will take further action if warranted by the circumstances.

- 11007445The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. If these contaminants settle and remain on a painted vehicle surface, especially the horizonal areas of the hood, roof, and decklid, significant damage can occur. This damage is the result of actual etching of the paint and appears as pitting or water spots. As acid rain droplets on the vehicle surface evaporate, the concentration strength of the acid increases, causing deeper and more rapid damage. This evaporation and corrosive action also occur more rapidly on dark colored cars as direct sun heat increases. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
- 11007444To prevent brake rotor rust from forming during transportation and storage, wheel film will be used instead of a cardboard type of anti-rust cover. The purpose of the wheel film is to shield the disc brake rotor from weather elements and initial rust before the vehicle is delivered to the customer. Consequently, the film should remain on the wheel for as long as possible.
- 11007443Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. If these acidic compounds settle on an exposed vehicle, especially the horizontal areas such as the hood, roof, and decklid, significant damage to the painted surfaces can occur. Acid rain damage can typically be identified on vehicles by the presence of stains on the paint surface that resemble hard water spots. Unlike water spots however, acid rain damage cannot be removed by regular washing procedures. Also, because acid rain can etch and soften the paint, normal buffing or polishing repair procedures should not be attempted. This can cause further damage and result in visible depressions in the paint surface. The following are the three major categories of acid rain damage: Minor damage: requires only buffing to repair. Moderate damage: usually requires neutralizing, color sanding, and buffing. Severe damage: extending beyond 1/2 mil of clearcoat on a pearl, metallic, or solid color, requires neutralization, sanding, and repainting. In cases where acid rain damage is minor, neutralization and buffing with a liquid-type paint finessing product may provide an adequate repair. Only specially formulated products outlined in this bulletin should be used for that purpose. Unfortunately, other than minor damage, there is no simple method of determining the actual extent (depth) of acid penetration other than color sanding a representative affected area until there is no visible etching or depressions, followed by measuring the amount of paint removed with either a magnetic or digital-type film thickness gauge. The procedures in this bulletin are intended for use by qualified body/paint technicians and should not be attempted by inexperienced personnel. It is the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.

- 11005153Toyota vehicles are currently protected with RapgardTM protective film designed to protect the horizontal painted surfaces. This material protects from acid rain, environmental fallout, and rail contamination.
- 11005149Vehicles may occasionally be subjected to contamination by airborne iron particles shed from railroad tracks, train wheels, exposure to heavy machinery facilities, grinding, welding, etc. This type of contamination can be identified by the presence of small, red or brown particles on the paint surface. These particles are often difficult to see on dark color paints but can be easily felt when brushing a hand across horizontal body surfaces such as the hood, roof, or deck lid.

Consequently, the film should remain on the wheel for as long as possible. SUPERSESSION NOTICE The information contained in this bulletin supersedes Service Bulletin No. PD017-04. - 11003644Acid rain results from rainwater or other airborne moisture that become acidic due to industrial chemical impurities in the atmosphere. While the quality and durability of paint on Toyota vehicles is continuously being improved, it remains the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to the first sale. To read, While the quality and durability of paint on Toyota vehicles is continuously being improved, it remains the dealers responsibility to protect and maintain the quality of the vehicles paint finish after receipt at the dealership prior to first sale. Perform frequent vehicle washing, as often as daily, during high heat and humidity periods to minimize the potential for paint damage due to acid rain exposure. This is especially important in geographical areas known for high frequency and concentration of acid rain and industrial fallout.
- 11003594The condition known as acid rain is caused by airborne chemicals or particles in the atmosphere, which mix with rainwater, nighttime dew, or high humidity to form acidic compounds. While Toyota paint quality, durability, and resistance to environmental factors is continuously being improved, it remains the responsibility of the dealer to protect and maintain the vehicles paint finish from the time of arrival at the dealership until retail delivery to the customer. In areas known for high frequency and/or concentration of acid rain (see Figure 1), frequent vehicle washing during high heat or humidity periods will minimize the potential for paint damage caused by acid rain. It is further recommended that either reverse osmosis or deionized water be used to prevent water spotting.
