I am submitting this complaint on behalf of my stepfather regarding a safety incident involving a 2014 Ford F-150. After coming to a complete stop at a red traffic light while on a curve, the truck shut off completely and would not restart. The hazard lights continued to function. The truck was towed to our home and has remained there because it is not drivable. We discovered Ford Safety Recall 24S37, NHTSA campaign number 24V444, which concerns a potential failure of the transmission output speed sensor (OSS). This failure may cause an unintended downshift into first gear, potentially resulting in rear-tire sliding and loss of vehicle control. The recall remedy involves updating the powertrain control module (PCM) software and checking for diagnostic trouble codes. We do not believe this recall repair has been completed, but this has not yet been confirmed. Although the shutdown does not exactly match the condition described in the recall, we are concerned about a potential safety-related defect and request that this incident be documented and investigated to determine whether it may be related to the recalled condition or another vehicle defect. A repair invoice dated March 13 also documents work that appears to involve cutting and programming a vehicle key. We request that this repair history be considered when investigating the cause of the shutdown. We request that the vehicle's recall status and repair history be verified and that the cause of the shutdown be investigated. We also intend to contact Ford to request assistance with towing the inoperable vehicle to an authorized dealership and a complimentary diagnostic inspection, if possible.
In the last month this has happened twice. While driving down the freeway my vehicle just downshifted on its own, speedometer dropped to 0, and I lost steering power but vehicle was still running. The wrench for service came on and the battery light also. As I was trying to get to a safe location the infotainment system went off first followed by my dash lighting. On the first time it happened after letting the vehicle sit for about an hour upon restart everything reset and was ok, but the engine light is still on after second incident and I'm afraid to drive it now.
I own a 2014 Ford F-150 equipped with the 6R80 six-speed automatic transmission (VIN: [XXX]), currently at approximately 133,000 miles. On XXX, while driving at ~70 mph on [XXX], my truck experienced an abrupt, unintended downshift — the precise failure mode described in Recall 24S37. The software patch prevented it from making it worse and a orange wrench came across the dashboard, but still it required some evasive maneuvering. I proceeded directly to the dealer to investigate; however, they were booked for the day. Not understanding the full impact I left to come back in the morning. The vehicle eventually became unsafe to operate, would not start, and had to be towed to Gullo Ford. The dealer then pulled diagnostic trouble codes, confirmed the transmission lead frame had failed, and replaced the lead frame at my expense of $1,977.04 (2127.04 with a coupon of 10% off) (invoice and repairs completed enclosed). INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
My truck has had repeated transmission issues, and has been "addressed" by two separate recalls (24S37 and 26S45). However, I am still having issues. I believe that "24S37 Supplement #1" specifically covers the issues I am having, yet Ford still continues to disregard my request. Tis is a major safety issue. It is my son's truck, and I am driving it now since I don't want anything to happen to him.
During a panic stop the brakes failed and the pedal went to the floor resulting in a rear end collision. The vehicle is currently with the mechanic but he believes it a failed brake booster. When I checked for related problems it seems the same brake booster (EL3Z-2140-E) in my 2014 F150 V8 5.0L was used in the same year 3.5L but yet the recalls (16S24 and 20S31) didn't include my vehicle
Brake master cylinder failed, leaking all Brake fluid into the Brake booster. I find recalls for this year model, but not my truck.
I was driving up the Interstate I565 heading towards Decatur Al from Huntsville Al. I was cruising at about 80 mph. All of a sudden my trucks transmission downshifted into first gear causing my back end to fish tail and the RPM's maxed out in the red and the engine lost all power. After that it would not start and I had it towed to the Ford Dealership. They did nothing and they charged me. They went ahead and took care of the recall then: because an intermittent loss of signal from the output shaft speed (OSS) sensor / internal lead frame can cause an unexpected downshift to first gear regardless of vehicle speed. The manufacturer was notified of the failure by phone. The failure mileage was approximately 145,500.
Keeps going into limp mode, codes are pointing to lead frame, a lot of sensor A codes. Rough shifting from 1st to 2nd as well as sudden down shiffting.
Ford did recall on transmission, it failed on my way home. I took it back and they refuse to fix trying to separate event to warranty issue. 3weeks later I have no truck and being stalled by ford. Even Elena Ford has it on her desk(computer) 4 days no response. They quickly processed the recall so they can imply it never had one. Lead module(connector) is reason and part of recall.
Suddenly lost power steering dash lights for traction control and power steering assist it takes a lot of strength to steer it no prior warning and I recently had spinal surgery
Odometer Fraud. The contact purchased a 2014 Ford F-150. It was discovered that there was a mileage discrepancy. The vehicle was a dealer sale. At the time of purchase, the vehicle mileage was 154,279. The contact stated that the mileage listed on the paperwork was 153,945.

The contact stated that the brake pedal was depressed to the floorboard but failed to respond as intended. The contact enabled the parking brake to stop the vehicle. The vehicle had experienced catastrophic brake failure. The vehicle was taken to the dealer, where it was diagnosed with a leak in the brake master cylinder. The contact was informed that the brake master cylinder, brake booster, and brake vacuum sensor needed to be replaced. The vehicle was not repaired. The manufacturer was contacted by phone, and a case was filed. The failure mileage was approximately 106,000.
While accelerating to pass another vehicle, on numerous occasions, and at speeds of 45-65 MPH, transmission suddenly downshifts to first gear, and engine over revs, followed by a very loud slam in the drivetrain. This has happened about twenty times, once with my wife along. There is a recall on 2014, F-150’s, but my VIN was not included. The dealership is refusing to investigate because they haven’t been able to reproduce this intermittent problem. I, essentially, have been told to pick up the vehicle that I’m afraid to drive. Must I wait until I have an accident because the rear wheels lock up?
We purchased this 2014 Ford F-150 SuperCrew (3.5L EcoBoost) used on Dec 31, 2022. Just two days after taking it home, the vehicle began experiencing sudden, violent, uncommanded downshifts from 6th to 1st gear at highway speeds. This occurred throughout our ownership. Because this was our family's only form of transportation, we had no choice but to continue driving it under these terrifying conditions, repeatedly navigating a lethal safety hazard with our three children in the vehicle until the powertrain suffered a catastrophic failure. This matches the exact defect investigated under NHTSA Campaign 24V444 / Recall 24S37 and expanded under Recall 26S45. On Dec 23, 2023, a final highway downshift forced the powertrain into 1st gear, creating immense kinetic stress and engine-braking that instantly shattered internal components and caused a severe engine knock. Less than a mile later, while trying to get our family home in life-threatening -40 degree Fairbanks winter weather with our three children inside, the engine completely blew, leaving us stranded. The Fairbanks Ford dealership initially stated that Ford would be liable for the consequential damages. However, it later backtracked, claiming that a local customer satisfaction program had "timed out" at the 150,000-mile threshold. A diagnostic certification from a local Nissan dealership has documented the failure. I was an original reporter aiding the push to reopen this failed software recall investigation, and Ford's corporate delays allowed this defect to destroy my engine before a real fix was issued. Due to this defect, our family has suffered extreme financial hardship: we spent $6,513.68 out of pocket for a 5-month Turo rental starting Christmas Day 2023; I was forced to leave my job due to a lack of transportation in a sub-zero climate; and I had to liquidate my retirement early. We have paid on time for 3 straight years on a broken vehicle and currently owe $19,633.44 on a financial ghost. Ford Motor Co
The contact's mother owns a 2014 Ford F-150. The contact stated that while his wife was driving 65 MPH, the brake fluid warning light illuminated. The brake pedal was depressed and went to the floorboard. The vehicle failed to stop. The contact's wife engaged the parking brake, and the vehicle was able to slow down, and the vehicle was pulled over and parked in a parking lot. The manufacturer was made aware of the failure and informed the contact that the vehicle was previously repaired under NHTSA Campaign Number: 16V345000 (Service Brakes, Hydraulic), and that the warranty had expired. The failure mileage was approximately 119,000.
I have a 2014 F150 manufactured in 5/2026 and recently had my electronic power assist steering fail. Ford issued recall 14S09 / NHTSA 14V-316. The recall was for F150's made between 5/26/2014 and 6/19/2026 that covered a steering gear motor position sensor magnet that was installed incorrectly. The result of this problem was a failure of the electronic power assist steering. Ford just diagnosed my F150 with an internal binding/failure of the steering gear, but when I searched for the recall using my VIN it wasn't included in the recall. I find this more than coincidental that my truck was manufactured in May of 2014 and just had the same failure stated in the recall.
The contact owns a 2014 Ford F-150. The contact stated that while driving 50 MPH, the RPM increased with the transmission stuck in second gear. There was an abnormal sound coming from the transmission. The vehicle was pulled over, the ignition was turned off, and the vehicle was restarted, and the vehicle operated as intended. No warning light was illuminated. The vehicle was taken to the dealer to be diagnosed, and the contact was provided an estimate for the diagnostic test. The vehicle was then taken to an independent mechanic to be diagnosed, and the contact was informed that the output shaft speed sensor and the valve body needed to be replaced. The vehicle was not repaired. The vehicle was serviced under the NHTSA Campaign Number: 24V444000 (POWER TRAIN) by the owner of the vehicle, but the remedy failed to prevent the failure. The contact stated that the failure had occurred after receiving the recall repair. The manufacturer was notified of the failure and informed the contact that, since the recall was already performed, the additional repair would be an out-of-pocket expense. The failure mileage was 160,000.
After transmission had programming update it never shifted properly. Took it to the dealer for diagnostic and they said nothing wrong. Transmission failed at only 82000 miles.
My 2014 Ford F-150 was previously included in Ford Safety Recall 24S37 regarding unintended transmission downshifts caused by an intermittent Output Shaft Speed (OSS) sensor issue. The vehicle was inspected by a Ford mobile technician on January 20, 2026. No qualifying codes were found at that time, and the recall was closed under my VIN. The recall involved the risk of an unintended downshift into first gear that could result in loss of vehicle control. On August 20, 2026, while my husband was driving approximately 40-50 mph, the truck suddenly downshifted and rapidly lost speed. The vehicle would not accelerate beyond approximately 10-15 mph and had to be moved into a parking lot to avoid remaining in traffic. After parking, the truck would no longer move under its own power. The wrench warning light illuminated and the vehicle became undrivable. The symptoms experienced appear substantially similar to those described in Recall 24S37. The dealership has indicated that the current failure may involve the transmission lead frame and speed sensor system, but I am concerned that this event may be related to the same transmission downshift condition addressed by the recall. I am requesting that NHTSA document and review this incident.
The contact owns a 2014 Ford F-150. The contact stated that while driving at an undisclosed speed, the wrench warning light illuminated, and the transmission unexpectedly downshifted to third gear. Additionally, the vehicle immediately went into LIMP Mode and failed to accelerate as needed. The contact associated the failure with NHTSA Campaign Number: 24V444000 (Power Train); however, the VIN was not included. The dealer was contacted and diagnosed the vehicle with molded leadframe failure. The vehicle was repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.
On July 30, 2026, while operating my 2014 F-150 under normal driving conditions, the dash cluster information screen displayed a "Brake Fluid Level Low" warning. Moments later, the brake system experienced a sudden/catastrophic loss of front braking performance. The brake pedal became severely spongy and traveled completely to the floorboard, requiring drastically increased distance to bring the vehicle to a stop. Due to ineffective braking, I missed the driveway of the parking lot I chose to inspect/determine what the issue was and subsequently could not stop the truck within the confines of the parking space. This brake failure put me, my family as well as oncoming traffic and pedestrians in jeopardy of injury or death due to collision. A mechanical inspection of the vehicle revealed the rear cup seal inside the brake master cylinder had failed. This internal seal failure allowed hydraulic brake fluid to bypass the cylinder and leak directly into the vacuum brake booster, completely contaminating the booster assembly. Without effective braking, the truck was deemed too dangerous to drive so it was parked and unused until replacement parts could be installed. This specific component failure, internal seal degradation, and resultant fluid contamination of the booster is identical to the safety defect detailed in NHTSA Campaign #16V345000 (Ford Recall #16S24). However, this specific vehicle was excluded from that safety campaign solely because it is equipped with the 6.2L V8 engine, rather than the recalled 3.5L engine, this despite using the identical Hitachi master cylinder. The Ford Corporate Consumer Relations was unwilling to assist with the purchase of replacement of the master cylinder, booster but did offer to record my complaint and issue a case number for engineering review. The defective master cylinder and contaminated booster assembly have been retained for inspection if required or requested.
The contact owns a 2014 Ford F-150. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 26V378000 (Power Train); however, the failure occurred after the recall repair was performed. The contact stated that while driving at an undisclosed speed, the transmission unexpectedly downshifted with the check engine warning light illuminated. The vehicle was taken to an independent mechanic, where a diagnostic trouble code for the transmission was retrieved. The vehicle then failed to restart and was towed to the dealer. The dealer determined that the molded lead frame needed to be replaced. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 127,000.
While driving, the malfunction indicator light (yellow wrench) will illuminate, followed by a complete loss of communication between the instrument cluster and the transmission. The speedometer immediately drops to zero even though the truck is still moving, and other information, such as the fuel range, goes blank. The truck will also struggle to accelerate from a stop, almost as if it is starting in second gear. If I shut the truck off and restart it, everything returns to normal. Before the malfunction light comes on, the transmission will often make a very hard downshift or remain stuck in first gear and refuse to shift as speed increases. After a few seconds, the warning light appears, communication is lost, and the transmission shifts again, often very harshly. This seems to happen more often at lower speeds. This creates a serious safety risk because I lose all knowledge of my speed while driving, and the transmission may not respond properly when pulling into traffic or trying to reach a safe place to pull over. The dealer is aware of the problem. Recall 24S37 was supposed to correct it, but it did not. Recall 26S45 acknowledges that the original repair was not effective for all vehicles, but my VIN is not included even though I am experiencing the same failure. I have reported the issue to the dealer multiple times by phone and through their online service system. A dealership representative told me I am not the only customer experiencing this problem, but the dealer has not inspected the truck since completing Recall 24S37 and has not taken action or returned my calls. The malfunction indicator light consistently appears about four seconds after the transmission begins acting abnormally and immediately before the instrument cluster loses communication with the transmission. I am now nervous to drive the truck, especially from a stop or at lower speeds, because I never know when the problem will occur.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. On May 23, 2016, Ford Motor Company (Ford) submitted a Defect Information Report (DIR) to NHTSA regarding a master cylinder defect that could result in loss of the front brake circuit in approximately 225,012 model year 2013 through 2014 Ford F-150 vehicles equipped with 3.5-liter GTDI engines and built at the Dearborn Truck Plant from August 1, 2013 through August 22, 2014; and the Kansas City Assembly Plant from August 1, 2013 through August 31, 2014 (NHTSA Recall #16V-345, Ford Recall #16S24).Ford indicated that the rearmost cup seal in the brake master cylinder may roll, resulting in leakage from the brake fluid reservoir to the brake booster.According to Ford's DIR, Depending on the amount of brake fluid loss from the brake master cylinder into the brake booster, the driver will experience an audible chime, message center alert, red brake warning light in the instrument cluster and may begin to experience a change in brake pedal travel and feel.In the event that a loss of brake fluid is substantial enough to reduce brake function to the front wheels, the driver may experience longer pedal travel, increased pedal effort, and extended stopping distance, increasing the risk of a crash.Full braking function would remain in the rear wheel brake circuit.Ford's recall will replace the master cylinder and, if the master cylinder is leaking, also replace the brake booster.Seventy-three (73) of the complaints received by ODI contained vehicle identification number (VIN) information required to determine date of build and recall applicability.ODI's analysis found that 66 complaints to ODI and 278 complaints to Ford involving vehicles built in the range covered by 16V-345, resulting in an overall complaint rate of 152.9 incidents per 100,000 vehicles.Twelve (12) crash incidents were reported, including 11 with VIN information. All 11 crash incidents with VIN information reported involve vehicles covered by Ford's recall.ODI's analysis identified 7 ODI complaints and 36 Ford complaints in the 194,139 subject vehicles built outside of the build range for 16V-345, resulting in a complaint rate of 22.1 incidents per 100,000 vehicles.Ford indicated that warranty return analyses identified issues with minor internal leakage between circuits due to internal contamination caused by supplier assembly plant cleanliness concerns in these vehicles.Ford assessed the internal leak condition as an early-life condition with minimal safety risk that does not result in loss of fluid or circuit failure.ODI will continue to monitor complaints in subject vehicles not included in 16V-345.No complaints have been identified in MY 2013 through 2014 Ford F-150 vehicles equipped with other engine options. The recall action initiated by the manufacturer appears to address the safety risks identified by this investigation. This preliminary evaluation is closed. The ODI reports cited above can be reviewed at http://www-odi.nhtsa.dot.gov/owners/SearchSafetylssues under the following identification numbers:10870768, 10870747, 10870511, 10870395, 10870055, 10869981, 10865843, 10865521, 10865342, 10865043, 10864862, 10864714, 10864360, 10863982, 10863527, 10862333, 10862047, 10861735, 10861190, 10860378, 10860118, 10855381, 10855356, 10854785, 10853751, 10853295, 10853012, 10852349, 10852134, 10852037, 10851317, 10851268, 10851109, 10850015, 10849862, 10846488, 10846335, 10846193, 10846006, 10840414, 10840377, 10839994, 10839985, 10839981, 10839833, 10839825, 10839816, 10838782, 10838510, 10818273, 10817706, 10809601, 10788886, 10788369, 10785526, 10779854, 10776834, 10766291, 10762034, 10761412, 10760663, 10758997, 10750065, 10749886, 107
