My 2021 F-150 PowerBoost hybrid repeatedly lost propulsion while traveling on the highway in early October 2026, including four incidents on one trip around October 3. At approximately highway speed, the tachometer dropped to zero while the selector remained in D, and the truck coasted as if in neutral. There were harsh shifts around 9th/10th gear and difficulty accelerating beyond approximately 70 mph. A vehicle warning appeared. Disconnecting and reconnecting the battery temporarily restored operation. Sudden loss of drive power in highway traffic created a collision risk. The underlying cause has not been independently established. A Ford dealer's October 2026 inspection recorded an internal transmission hydraulic leak, clutch-disconnect trouble codes suggesting a hybrid drive unit concern, a recommendation to replace the transmission with the hybrid drive unit, and high-voltage battery cell-balance DTCs. More recently electric-driving mileage has remained at zero and the gasoline engine continues running while stationary. Severe shifting and vibration had varied between 2H and 4A modes. Around June 2025 the vehicle also stopped operating and was towed to the same dealer; I understand a 12-volt battery was replaced. Photographs from that period show Stop Safely Now and other warnings around 70,000 miles. I request investigation of whether that earlier incident and the present faults are related; I do not claim the same root cause has been proven. Exact diagnostic codes and earlier service details should be verified from the dealer records. The October intake odometer was recorded as 99,632; mileage at the highway incidents is not precisely known.
I had an oil change performed earlier in the day. Drove 90 miles on the oil change when I lost complete oil pressure and than had my engine lock up. After the truck was towed back to the dealership and some surface investigation was done they told me it would be 2K to take the oil pan off to get a definitive answer to the issue. For them the only fix was replacing the entire engine at 14k. They believed it was just bad luck timing the occurrence of the loss of oil pressure and engine failure. The event happened with 135,556 miles on the vehicle. At 122,000 miles I had my drive shaft detach on the expressway. At the time I was still under extended warranty on the truck. The fix for that was a new drive shaft and to replace the rear axle. The issue I have is believing that 2 catastrophic events on the vehicle would occur within 12k miles and the three events not being connected. The manufacture wasn’t able to provide me with any answers as to what failed or why making it incredibly difficult to make a huge financial decision on what to do. I struggle with a vehicle that has that low of mileage would just completely implode and be undrivesble.
Passenger side LED DRL outage on horizontal and vertical LED strip
The cdf is outdated and the truck would get stuck in the wrong gear and not work. I was only 3,800 miles over the 60k mile warranty and having to pay out of pocket for a updated cdf drum that ford has created because they know the old ones are faulty

In July of 2024 I took my truck into Cleburne Ford in Cleburne, Texas for transmission issues. The vehicle registered 39, 581 miles at that time. My complaint was mainly clunky issue regarding shifting when down and upshifting. The dealer test drove and advised completing transmission rebuild IAW with TSB 24-2176. The truck stayed at the dealership for approximately one month closing out the work order on 8/6/2024. I drove the truck for approximately two years until experiencing a serious issue that was extremely dangerous. On a road trip to to El Paso from Glen Rose with the cruise control set travel at approximately 80 miles per hour, which is the legal limit on that stretch of road. The vehicle started lunging, lurching, and hunting! it was extremely unnerving and could have caused a lesser driver to lose control of the truck, because at that speed it only takes one small issue to happen and a grave accident could have occurred. The service engine soon indicator light then came on the dash. The cruise control was deactivated and we were near a rest stop where we stopped. When I turned the truck back on the service engine soon indicator light was off. For some reason I thought maybe it has something to do with the cruise control, so I did not turn it on again for the remainder of the trip there and back. The truck seemed to be ok, but I wanted to take it in again to have it checked. I took it to Triple Crown in Stephenville Texas, because I felt upset to think I might have experienced another transmission issue just two years ago. They system identified code P 07F7 and determined TSB 26-2226 was required. They commenced to do another rebuild replaced everything again. They did not find evidence of the CDF or trash can being bad. They said its also important to replace the pump and valve body to mitigate the possibility or reoccurrence. Ford paid approximately $1,800, because of a request from the dealership and I paid approximately $2,200 out of my pocket.
The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated, and a alert was received through the mobile application. The vehicle was taken to a dealer, where a diagnostic inspection revealed that the knock sensor failed and required replacement. The dealer informed the contact that water intrusion on the intake manifold caused the knock sensor failure. The vehicle was scheduled for repair at the dealer. The contact attributed the failure to the factory omission of the engine cover and the cowl design directly above the engine compartment, which allowed water to collect on the manifold. The manufacturer was notified via telephone, and a case was filed. The failure mileage was approximately 64,900.
My daytime running light has burned out. I also see a lot of other f150 around town with the same exact problem. I also don’t want to get stopped from the police for having a bulb burned out. I have researched this problem and you can’t fix it unless you buy a whole brand new headlight assembly for $1500.
I am getting a Steering Assist Fault message on my dash as well as code C1B00:49-2F. I took my vehicle to the Ford dealership and the deemed I have a faulty steering rack. I noticed there was also a recall out for other 2021 F-150's experiencing similar issues. My VIN was not included.
I have experienced ongoing issues with transmission. Two separate dealerships have done partial "rebuild" to no avail. The vehicle has lost power several times which include XXX at approximately 3:30 pm. while pulling onto a busy highway in heavy traffic. As soon as I got home, I parked the truck in the carport where it remains as of this writing. There have never been any warning lights, symbols, etc. on dash when these and other multiple problems with this vehicle happened. Multiple technicians have duplicated all of the issues that I have reported to the dealerships. I have been in touch with Ford Motor Company several times and have an open case with nothing from their representatives except to say how sorry they are. Second dealership has stated both verbally today, XXX, and in writing on XXX that he personally oversaw a complete teardown of this transmission on XXX when C-clutch was replaced see service order#XXX although parts list doesn't support his statement. Please refer to service order #XXX which is a documented failure of the E-clutch in my vehicle. The service manager at [XXX] O[XXX] states that this document is "speculation" on the part of the technician. I am in possession of his email stating such. I am at this time requesting any investigator be assigned to my case/claim # XXX, which I was given by [XXX] (last name unknown) Respectfully yours [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
While driving truck, it had an electrical fritz that caused the car to become undrivable, with systems not responding. when stopped to power cycle, the car would accessory on but not attempt to start. the horn would go off randomly. was able to manually put the truck into neutral and push into a safe spot, at what point a battery clear was attempted with no results, and required towing to dealership where water had intruded into the BCM

The contact owns a 2021 Ford F-150. The contact stated that the LED daytime running lights was flashing quickly before burning out. The contact stated that while driving 30 MPH and then stopped at an intersection, another driver informed the contact that the passenger's side headlight was flashing quickly. The contact towed the vehicle to the residence. The vehicle was taken to the dealer, where the passenger's side headlight assembly was replaced under warranty. The dealer informed the contact that the driver's side headlight might fail as well. The contact stated that the failure also occurred on the driver's side. Upon inspection, the contact became aware of a burning plastic odor coming from the headlights. Upon researching the failure online, the contact learned that the failure resulted from the headlights overheating and melting. The manufacturer was notified of the recent failure but denied coverage because the vehicle had exceeded 100,000 miles. The dealer advised the contact to report the second failure to the NHTSA Hotline. The failure mileage was approximately 99,000.
Vehicle: 2021 Ford F-150 Tremor Mileage: Approximately 103,000 miles My 2021 Ford F-150 Tremor developed a severe shudder and vibration during acceleration. The condition progressively worsened and became significant enough that I had the vehicle professionally inspected. The vehicle was diagnosed with a failing torque converter as the cause of the acceleration shudder/vibration. The truck has been well maintained throughout my ownership. It is garage kept and receives regular maintenance through an authorized Ford service facility. I have maintenance records documenting the vehicle’s service history. My concern is that the torque-converter failure produces a severe vibration/shudder during acceleration and has progressively worsened. I am concerned about the potential safety implications if the torque converter or transmission continues to deteriorate while the vehicle is being driven. I contacted Ford regarding the failure and requested assistance with the repair. The vehicle is outside Ford’s original powertrain warranty at approximately 103,000 miles, and Ford declined to provide assistance. I am submitting this complaint so NHTSA has a record of this torque-converter/transmission failure and the severe acceleration shudder occurring in my 2021 Ford F-150.

The vehicle was towed to the local dealer, who diagnosed that the parking pawl solenoid had failed and needed to be replaced. The contact was notified of NHTSA Campaign Number: 26V402000 (POWER TRAIN); but was informed that the part to do the recall repair was not yet available. The failure mileage was 50,000.
The contact owns a 2021 Ford F-150. The contact stated that while driving 45 MPH, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The vehicle was shaking abnormally and the check engine warning light illuminated. The vehicle was driven to the shoulder of the roadway. The contact turned off the vehicle. The vehicle was restarted. The vehicle was driven to the residence. The dealer was contacted by phone and made aware of the failure. The vehicle was taken to the dealer; however, the failure was not duplicated and the failure reoccurred. The vehicle was taken back to the dealer, and the contact was informed that the spark plugs needed to be replaced. The vehicle was repaired. The contact was informed that parts were not yet available for NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline). The contact stated that the vehicle was unsafe to drive. The manufacturer was contacted by phone and email and a case was filed. The failure mileage was approximately 51,000.
This vehicle was purchased new. It currently has about 56000 miles. Over time there have been numerous incidents where the battery was suddenly dead. The dealer repeatedly replaced the battery with out addressing the cause. This was done under warranty until recently when there was a charge of $1138. This supposedly resulted in the electronics being reset to factory settings. On a subsequent trip of about 2000 miles , there were numerous incidents (at least 8-10) where the Google Maps program, installed by the dealership, malfunctioned. My question is, what other electronic issues were installed with the factory settings. This has been reported to the dealer, with no response. On August 31, 2026, a request was made to the dealer for copies of service records for this vehicle so that this problem and lack of correction could be challenged. There has been no response, other than their saying “they will make it right “. They are apparently unable to provide service records.
NHTSA’s Office of Defects Investigation opened these reviews into vehicles including this one.
On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. In view of the recall action being taken by Ford, ODI is closing this Engineering Analysis.
